New Potential of Chronic Cough Therapy in Postnasal Drip 鼻后滴注治疗慢性咳嗽的新潜力

Respiratory tract diseases are often accompanied by cough. Cough may vary in duration and aetiology. The underlying causes of cough may be associated with upper and lower respiratory tract conditions. Besides, some types of coughs are not related to any respiratory disorder. Cough can be an underlying disease or only a symptom of a medical problem. It is no wonder why the antitussive drugs market is so huge. The main antitussive medications include antitussives themselves (centrally acting and peripherally acting drugs), drugs with an indirect cough suppressing action (bronchodilators, anti-inflammatory drugs, antiallergic agents, decongestants, etc.) and combination products. However, only few of them produce combined effects. This article presents a clinical study of the efficacy and safety of Rengalin, the released-activity combination drug having antitussive, anti-inflammatory and bronchodilator actions. The study enrolled patients with chronic cough arising from a postnasal drip syndrome. The study treatment was compared to the standard therapeutic strategy. The following parameters were assessed to determine statistical difference: daytime and night-time cough severity, a patient's general condition measured by the visual analog scale (VAS), and the number of patients with fully resolved cough by the end of the study treatment. Rengalin was found to increase the efficacy of treatment for chronic cough and improve quality of life in patients.

Medical Cybernetics for Continuous Risk Assessment and Management of Insulin Resistance and Related Complications 胰岛素抵抗及相关并发症持续风险评估与管理的医学控制论

Individualized precision methods are needed for continuous non-invasive monitoring of state variables of risk indicators not just for early detection of subclinical derangements but also to monitor progress of effect of lifestyle modification and medical therapies throughout lifespan. We envision a cloud based cyber-physical system where input data come from wearable sensors along with frequently or daily use of a body-composition/ hydration-analyzer/ photoplethysmography equipped stand-up scale and these sensor data are used for state space modelling of functioning in four domains with major implications for morbidity/ mortality: 1. Cardiometabolic, 2. Cardiorespiratory, 3. Cardio vegetative, and 4. Cardiovascular functioning. The metrics in these 4 domains allow to draw trajectories into the future and to continuously assess risks of endpoints. At subclinical stage of derangements cloud computing generated feedback on a mobile app may provide for automation and self-improvement to meat daily goals of therapeutic efforts. When abnormalities reach more significant level therapies by health care providers are in order along with continued self-management of modifiable risk factors with behavior modification. This article outlines how a Medical Cybernetics centered approach can be used for continuous risk assessment/ management of insulin resistance and its connected pathological processes such as endovascular inflammation and dysfunction.
