Insecticides Susceptibility of Malaria Vectors in Okitipupa, Ondo State, Nigeria Using WHO Susceptibility Test 利用世卫组织药敏试验对尼日利亚翁多州奥基蒂帕疟疾病媒的杀虫剂敏感性进行研究

: Pyrethroid insecticides are recommended for use in long-lasting insecticidal nets (LLINs) and indoor residual spraying (IRS) due to their low mammalian toxicity and fast action. Currently pyrethroid resistance has been reported in western and eastern Africa, therefore monitoring resistance is important in all malaria endemic countries. The overall goal of this study is to monitor resistance levels in malaria vectors in Okitipupa. Susceptibility of malaria vectors to insecticides was determined in Ayeka, Idepe, Igbodigo and Okitipupa districts of Okitipupa town using WHO test. Three sentinel sites from each district were selected and mosquitoes were collected from each sentinel site. The collected Anopheles mosquitoes were reared to adults in the insectary of the Department of Biological Sciences of Olusegun AgaguUniversity of Science and Technology, Okitipupa. Two to five days old An. gambiae mosquitoes were assessed for resistance levels to Pirimiphos-methyl (0.25%), Lambdacyhalothrin (0.05%), Bendiocarb (0.1%), Permethrin (0.75%) and Propoxur (0.1%). Knockdown time (KDT) was recorded up to 60 minutes and maintained for 24hrs post-exposure on 10 % glucose solution, after which mortality was recorded. The susceptibility test showed that mosquito mortality after 24 hrs for Pirimiphos-methyl was 25%, Lambdacyhalothrin was 0%, Propoxur was 75%, Bendiocarb was 100% and Permethrin was 50%. This study revealed development of resistance to Pirimiphos-methyl, Permethrin and Lambdacyhalothrin in An. gambiae s. l. across all sentinel sites. The implication of this was discussed and suggestion for monitoring was made before this problem becomes widespread in Okitipupa town.

Inhibitory Effects and Potential Mechanism of Yokukansankachimpihange on SDS-induced Itch-associated Responses in Mice Yokukansankachimpihange对sds诱导小鼠瘙痒相关反应的抑制作用及其潜在机制

Yokukansankachimpihange (YKSCH) is a Chinese formula proven in Japan to be an effective treatment for pruritus in patients with atopic dermatitis or chronic urticaria and dry eczema with psychoneurotic symptoms, such as insomnia. Sodium dodecyl sulfate (SDS) may cause skin irritation in both humans and animals. Adverse effects, such as skin dryness, barrier destruction, dermatitis, and pruritus, developed following the repeated application of 10% SDS to a local site. Therefore, the present study investigated the antipruritic effects of YKSCH on skin irritation induced by a repeated SDS stimulation in mice and attempted to elucidate the underlying mechanism of action. ICR mice were stimulated with 10% SDS and co-treated with oral YKSCH extract for four days. Only oral YKSCH extract was administered on the fifth day. Scratching behavior was observed 24 h after the last application of SDS. Intraepidermal nerve growth was investigated by an immunofluorescence analysis. NGF concentrations in the epidermis were measured using an enzyme-linked immunosorbent assay. The expression of amphiregulin (Areg) and semaphorin 3A (Sema3A) was assessed by quantitative real-time PCR. The oral administration of YKSCH (200-400 mg/kg) dose-dependently suppressed pruritus, significantly reduced intraepidermal nerve growth, and down-regulated epidermal Areg mRNA expression in SDS-treated mice. These results suggest that YKSCH exerts antipruritic effects against SDS-induced pruritus in mice. The mechanism of action of YKSCH may involve reductions in intraepidermal nerve density due to the down-regulated expression of Areg.

Pharmaceutical Practice of Ceftazidime-avibactam Combined with High Dose Meropenem in XDR-Klebsiella Pneumoniae Bloodstream Infection 头孢他啶-阿维巴坦联合大剂量美罗培南治疗广泛耐药肺炎克雷伯菌血流感染的药学实践

Background: The literature of ceftazidime-avibactam for bloodstream infections is limited, and whether meropenem (MIC ≥ 16 mg/L) can be used is also a perplexing issue for clinicians. Aim: To observe the clinical efficacy and safety of ceftazidime-avibactam combined with high dose meropenem in the treatment of extensively drug-resistant (XDR) klebsiella pneumonia (Kp) bacteremia. Methods: In 1 critically ill patient with XDR-Kp diagnosed through blood culture, tigecycline combined with cefoperazone-sulbactam and tigecycline combined with polymyxin B were successively given, and both showed poor anti-infection effects. The clinical pharmacists recommend ceftazidime-avibactam combined with high dose meropenem for anti-infection therapy. Through literature review, clinical pharmacists dynamically adjusted the dose according to the creatinine clearance rate of patients. Clinical pharmacists analyzed whether meropenem (MIC ≥ 16 mg/L) could be combined with ceftazidine-avibactam for XDR-Kp and recommended the dose of meropenem for 2g q8h and intravenous pumping for 4 hours. Results: Ceftazidime-avibactam combined with a large dose of meropenem pumped into the vein for a long time made XDR-Kp rapidly negative conversion, with clinical symptoms, signs and inflammatory indicators such as body temperature, PCT, and CRP significantly improved, without adverse reactions such as liver and kidney function. Conclusion: Ceftazidime-avibactam combined with high dose meropenem has a significant therapeutic effect on XDR-Kp and the efficacy is safe and reliable.

Determinants of Cannabis Use Among Psychotics in Kinshasa, Democratic Republic of the Congo: A cross-sectional study 在金沙萨,刚果民主共和国精神病患者大麻使用的决定因素:一项横断面研究

Background and aim: The relationship between cannabis use and psychosis is well established, but the determinants of this use in psychotics in developing countries are lacking. The aim of this study is to identify the determinants of cannabis use among psychotics in Kinshasa, Democratic Republic of Congo. Methods: This cross-sectional and analytical study included 321 psychotics carried at the Neuropsycho-pathological Center of the University of Kinshasa out from January 1, 2018 to December 31, 2019. The variables of interest were socio-demographic parameters, cannabis use and diagnoses of psychosis. A logistic regression model identified the determinants of cannabis use. The threshold of statistical significance was p<0.05. Results: The mean age of cannabis users was 30.9 ± 8.7 years with a sex ratio of 12 M / 1F. They represented 37% of psychotics of which 51% were of no religion and 55% were schizophrenics, 77% had tobacco-alcohol co-use, 83% had high education level and 78% were single. The determinants of cannabis use were male (aOR=2.01, 95% CI: 1.27-3.18; P<0.003), not belonging to a religion (aOR: 2.43, 95% CI: 1.24-4.80; p=0.010), tobacco co-consumption (aOR=9.12 95% CI: 4.35-19.13; p<0.001), alcohol (aOR=4.39, 95% CI: 2.22-8.71; p<0.001) and schizophrenia (aOR=2.29 95% CI: 1.15-3, 19.13; P=0.019). Conclusion: The prevalence of cannabis use among psychotics was high. They were young male schizophrenics, without religion, single with co-consumption of tobacco and alcohol. The determinants were male gender, non-religious affiliation, tobacco-alcohol co-consumption and schizophrenia.

Establishment of a Method to Allow Microbial Limit Testing of Iodine-containing Preparations 含碘制剂微生物限度检验方法的建立

Background: Contamination of pharmaceuticals with microorganisms may lead to deleterious effects on the therapeutic properties of the drug, and may potentially cause injuries to intended recipients. Iodine can inhibit and kill microorganisms in pharmaceutical preparations. The risk of microbial contamination of cellular products can be reduced when cultured in the presence of iodine. This however, may impact the sensitivity of microbiological tests. Iodine-containing Preparations does not guarantee sterility but may just reduce the proliferation rate of microorganisms, microbiological testing of medicinal products remains obligatory. Thus, an appropriate method to test for microbial contamination of iodine-containing products has to be validated. Objective: To establish a method that would allow microbial limit testing of four iodine-containing preparations (concentrated iodine tincture, compound iodine oral solution, iodine tincture, and iodine glycerin). Methods: To specifically determine the degree of contamination in the four in-house iodine preparations, sodium thiosulfate was used to reduce the iodine molecules to iodide ions, thereby eliminating the intrinsic antibacterial property of the preparations. Then, the suitability of this method was evaluated according to the regulations for non-sterile microbial limit tests. Results: The neutralization method effectively eliminated the interference of the antibacterial iodine component, rendering the control bacteria detectable. The recovery ratio of the test strains met the required standards. Conclusion: The neutralization method with sodium thiosulfate as the iodine neutralizer is suitable for microbial limit testing of concentrated iodine tincture, compound iodine oral solution, iodine tincture, and iodine glycerin, with the results being accurate and reliable.
