Abstract Introduction : Although it brings several benefits, simulation in emergency care can cause significant psychological stress and thus inhibit the learning process. The addition of an educational video (EV) to the briefing would better prepare students. However, the acceptability of an EV as well as its effects are unknown. Objectives : 1) to document the acceptability of an EV to the briefing in emergency care simulation with nursing students, and 2) to measure the effects of the EV on situational anxiety and teamwork. Methods : A randomized post-test experimental design with a control group was conducted with 125 nursing students recruited using a convenience sample, randomly divided into two groups: a) control (n= 50) without viewing of an EV and b) experimental (n= 75), with viewing of an EV. Questionnaires were administered to assess the acceptability of the EV, situational anxiety, and teamwork. Results : The majority of participants of the experimental group recognize the acceptability of the EV. However, no effect of the EV could be demonstrated on situational anxiety (p = 0.24) and teamwork (p = 0.95). Discussion and conclusion : It seems important that the EV is implemented in a supervised manner, including discussions about its content, in order to increase the benefits for the students. Future studies comparing the presentation of an EV with or without discussion surrounding its presentation, as well as before and after the simulation, would provide a better understanding of its influence on situational anxiety and teamwork.

Nurses’ Evaluations of the Feasibility and Clinical Utility of the Use of the Critical-Care Pain Observation Tool-Neuro in Critically Ill Brain-Injured Patients 护士对脑损伤危重症患者使用危重护理疼痛观察工具- neuro的可行性及临床应用的评价

Introduction: The Critical-Care Pain Observation Tool-Neuro (CPOT-Neuro) was derived from the original CPOT to assess pain in brain-injured patients in the adult intensive care unit (ICU). Objective: This study aimed to describe the nurses’ evaluations of the feasibility and clinical utility of the use of the French and English versions of the CPOT-Neuro in critically ill brain-injured adults. Methods: Fifty-nine ICU nurses from two university affiliated trauma centres (Montreal, Canada) were trained to use the CPOT-Neuro. Those who used it at the bedside during the study were invited to complete a self-administered questionnaire about its feasibility and clinical utility. Results: Twenty-seven of the trained ICU nurses (46 %) used the CPOT-Neuro during the study and completed the questionnaire. Feasibility: All nurses agreed that the training duration was sufficient, that the directives of use were clear, and that the tool content was simple to understand. Some nurses disagreed that the tool was quick to use (11.1 %), and easy to complete (7.7 %). Clinical utility: The tool was found useful by most nurses, but some of them disagreed that it was helpful for practice (7.7 %), that it influenced their practice (15.4 %) and that they would recommend it routinely (11.1 %). Responses did not differ between nurses from the two trauma centres who used either the French or the English version of the CPOT-Neuro (Mann-Whitney U tests, p>.05). Discussion and conclusion: The CPOT-Neuro was found to be feasible and useful by ICU nurses. Its implementation into daily practice could optimize pain assessment in critically ill brain-injured patients. Pain Management in the Context of a Nursing Help Line: Identification of Needs, Development of a Continuing Education Activity, and Evaluation of its Impact 护理帮助热线中的疼痛管理:需求的识别,继续教育活动的发展,以及其影响的评估

Introduction: Pain management by nurses in the context of a phone help line is a complex task. Continuing education (CE) is a potentially effective strategy to ensure development of this competence. Objective: The main objective of this study was to develop and evaluate a customized pain management CE activity by and for nurses working at a phone help line providing health information. Methods: A three-phase convergent mixed-method design was used: needs and preferred educational strategies assessment, conception of CE activity, evaluation. Based on a participatory approach, the CE activity was developed to meet participants’ expectations and needs. It included two components: 1) CE day and 2) individual clinical support. A quasi-experimental study with a single time series was used to evaluate the CE activity regarding participants’ knowledge and beliefs about pain management and their perceptions of their pain management activities. Data collection was performed using focus groups and questionnaires. Results: Participants’ knowledge about pain management increased after the CE day and remained stable after three months. Also, participants reported an increase in various patient-centered pain management nursing activities. Discussion and conclusion: This study illustrates the importance of involving nurses in designing a CE activity and supports its potential benefits in the context of a phone help line. The fast-track system is characterized by the early identification of patients presenting themselves at the ED with a non-urgent health problem but requiring an appropriate treatment in a reserved area. Objective : To describe the steps and outcomes of a knowledge transfer project aiming to implement and evaluate a fast-track system for children with acute respiratory problems at the emergency department of a university hospital. Methods : According to the Knowledge-to-Action Process Framework, the project included: 1) the implementation plan 2) the fast-track system and 3) the evaluation of the acceptability of it by the interdisciplinary team and the family as well as its potential for efficiency. Results : The results indicate a high acceptability of the fast-track system from the interdisciplinary team (nurses, physicians, respiratory therapists, patient attendants) and families, as well as a decrease in the average length of stay in the ED. Discussion and conclusion : The fast-track system can be considered as an organizational modality that can improve access to emergency services by leveraging the clinical skills of the nurse to effectively address the needs of the child with acute respiratory problems.

Effects of an Intervention Approach Based on the Meanings of Vocal Behaviours in Older People Living with a Major Neurocognitive Disorder: A Pilot Study 一种基于声音行为意义的干预方法对老年神经认知障碍患者的影响:一项初步研究

Introduction : Vocal behaviours (VB) are frequent in long-term care facilities (LTCF) and have many negative consequences. Interventions to decrease VB have limited clinical impact. Objective : This pilot study aimed to provide preliminary evidence on the effects of an intervention approach based on the meanings of VB in older people living with a neurocognitive disorder in LTCF. Methods : A mixed methods action research study was used. Fourteen triads (n=43) made up of an older person who manifested VB, a family member, and formal caregivers were included. An approach based on the meanings of VB was implemented in five LTCF. Semi-structured interviews took place with some participants. Tools were used at four-time points to measure five variables: the frequency of VB, the well-being of older people, the perceived disruptiveness of VB, the partnership-based decision-making and the empowerment felt by family and formal caregivers. Results : The approach improved the perception of family and formal caregivers toward older people. This led to habit changes that influenced positively the frequency of VB and the well-being of older people. Formal and family caregivers also perceived VB as less disturbing and felt more empowered relatively to VB. The attitude of formal caregivers toward families evolved positively. No other changes were noted on partnership-based decision-making. Discussion and conclusion : This study indicates the potential of this approach to improve the well-being of older people who manifested VB, their families, and formal caregivers. This approach could be implemented in LTCF, tested and adapted for other behaviours.
