運動誘発性喉頭閉塞症(exercise-induced laryngeal obstruction:EILO)は高強度運動時発作性に喘鳴と呼吸困難をきたす疾患である.楔状結節の余剰粘膜が内転する,かつ/または吸気時に開くべき声帯が内転することにより喉頭の閉塞が生じる事がその原因であり,誘発性喉頭閉塞症(inducible laryngeal obstruction:ILO)の病型の1つである.運動による誘発される声帯機能不全(vocal cord dysfunction:VCD),声帯奇異性運動(paradoxical vocal cord motion:PVFM),喉頭軟化症など様々な用語で表現されてきたが近年用語が統一された.診断は運動中の持続的喉頭内視鏡(continuous laryngoscopy during exercise:CLE)により確定するが施行可能な施設が限られている.
运动诱发性喉梗阻症(exercis -induced laryngeal obstruction, EILO)是一种高强度运动时发作性喘息和呼吸困难的疾病。其原因是由于楔状结节的剩余粘膜内翻和/或吸气时应打开的声带内翻而造成喉头阻塞。是诱发性喉头闭塞症(inducible laryngeal obstruction, ILO)的病型之一。运动诱发的声带功能不全(vocal cord dysfunction, VCD),声带奇异运动(paradoxical vocal cord motion)PVFM),喉头软化症等各种各样的用语来表现,不过近年来用语被统一了。诊断可以通过运动中的持续性喉内窥镜(continuous laryngoscopy during exercise, CLE)来确定,但可实施的设施有限。

食物蛋白誘発胃腸炎(Food protein-induced enterocolitis syndrome:FPIES)は非IgE依存性食物アレルギーの一病型であり,原因食物を摂取した1~4時間後に皮膚症状や呼吸器症状などのIgE依存性反応を伴わない繰り返す嘔吐で特徴づけられている.2017年の国際コンセンサスガイドライン公開により,世界各国でFPIESに関する報告が増えている.海外におけるFPIESの原因食品は,牛乳・米・魚が多く,鶏卵がそれに続いている.一方,日本ではこの数年で鶏卵,特に卵黄によるFPIESの患者が急激に増加しており,現在では原因食品として最も頻度が高い.このため,鶏卵FPIESの発症年齢,予後などの疫学面では日本を中心に解明が進んでいるが,原因抗原・感作経路・嘔吐の機序・免疫学的機序については不明な点が多い.診断の補助となる検査,安全で信頼性の高い食物経口負荷試験や長期管理の方法も確立されていない.今後これらが明らかにされることで,急増する鶏卵FPIES患者数に歯止めがかかることが期待される.
食物蛋白诱发胃肠炎(Food protein-induced enterocolitis syndrome)FPIES)是非IgE依赖性食物过敏的一种病型,其特征是在摄取致病食物1 ~ 4小时后,不伴有皮肤症状和呼吸道症状等IgE依赖性反应的反复呕吐。随着2017年国际共识指南的公开,世界各国关于FPIES的报告不断增加。在海外引起FPIES的食品中,牛奶、大米、鱼居多,鸡蛋紧随其后。另一方面,在日本这几年由于鸡蛋,特别是蛋黄引起的FPIES的患者急剧增加,现在成为出现频率最高的原因食品。因此,在鸡蛋FPIES的发病年龄、预后等流行病学方面,以日本为中心正在进行研究,但在致病抗原、致敏途径、呕吐机制、免疫学机制等方面仍有很多不明之处。作为诊断辅助的检查、安全可靠的食物口服负荷试验和长期管理的方法也尚未确立。今后通过明确这些情况,有望遏制急剧增加的鸡蛋FPIES患者人数。
