Harmful alcohol consumption and its predictors among drivers with Crash Related Trauma (CRT) in LAUTECH Teaching Hospital, Oshogbo, Osun State, Nigeria. 尼日利亚奥孙州奥肖博LAUTECH教学医院车祸相关创伤(CRT)驾驶员有害酒精消费及其预测因素

Background: Alcohol impaired driving is a leading cause of Crash Related Trauma (CRT) in Nigeria. Information regarding pattern of alcohol consumption among drivers presenting CRT is scarce. This study explores the pattern of harmful alcohol consumption, its demographic correlates associated with risky driving behaviour among recuperating drivers in a tertiary hospital in Oshogbo, Nigeria. Methods: The study crosssectional study of recuperating patients presenting Crash Related Trauma (CRT) at the accident ward and the physiotherapy unit of LAUTECH Teaching Hospital, Oshogbo, Osun State, Nigeria. The recuperating drivers responded to the Risky driving behaviour scale, Alcohol disorder screening tool and sociodemographic questionnaire. Data was analysed using descriptive statistics, chi-square test, Pearson correlation and multiple regression analyses at p and #8804;0.05. Results: Findings revealed that less than half of the victims were daily drinkers and 28% were heavy drinkers. The prevalence of hazardous drinking was 37.3%. Age of driver, gender, religious affiliation and driving experience were associated with harmful alcohol intake. Gender (male sex); driving experience, high alcohol consumption rates and alcohol related problems predicted risky driving behaviour. Conclusion: There is high prevalence of alcohol dependency among drivers recuperating from CRT. Drug rehabilitation for Alcoholics should be inculcated in the management of CRT victims.

Refractory Chylothorax ; A rare presentation of Systemic Lupus Erythematosus ( SLE ) 难治性乳糜胸;罕见的系统性红斑狼疮(SLE)

5-10% of SLE patients can present with pleuritis as their initial manifestation. However, chylothorax is a rare finding in patients with SLE. A 27 year old lady presented with refractory bilateral chylothorax of unknown etiology and was later diagnosed as SLE. Her chylothorax resolved with immunosuppressive therapy, however had a recurring course throughout her follow up.

LARGE THYROGLOSSAL DUCT CYST WITH ATYPICAL ENDOLARYNGEAL PRESENTATION - A RARE ENTITY. 具有不典型喉内表现的甲状舌管大囊肿-罕见病例。

Thyroglossal duct cysts are the commonest cystic neck swellings, usually seen in pediatric population. They are typically found in midline, and mostly infra-hyoid. Very rarely thyroglossal duct cysts are found with atypical endolaryngeal presentation. Literature search revealed 15 cases of endolaryngeal thyroglossal duct cysts reported in literature. In this article, we report the 16th case of thyroglossal duct cyst with endolaryngeal extension. This being a very rare case of a large cyst which presented with dysphonia in a 41-years-male who was referred to us for laryngoscopic examination by a speech language pathologist (SLP) His primary complaint was dysphonia and further inquiry revealed a painless, slow growing cystic neck swelling of submandibular triangle. It was diagnosed as a thyroglossal duct cyst with endolaryngeal extension following laryngoscopy and Computed tomography. Cyst was surgically removed with Sistrunks procedure with no recurrence and normal phonation without any therapeutic intervention by SLP post operatively. Literature search revealed that TGDCs with endolaryngeal extension are very rare.

Pre-pubertal juvenile nasopharyngeal angiofibroma with intracranial extension 伴颅内扩张的青春期前青少年鼻咽血管纤维瘤

Juvenile nasopharyngeal angiofibroma (JNA) is a rare benign vascular tumor predominant in adolescent and adult young male. It represents only 0.05 to 0.5% of all head and neck neoplasms. Clinical presentations of JNA are dominated by recurrent and unprovoked epistaxis, and nasal obstruction. More rarely other unusual clinical manifestations can be observed, especially in forms invading oropharyngeal structures and skull base. Headache and exophthalmia are exceptional. The JNA occurring in the pre-pubertal period can pose a diagnostic challenge for clinicians because of their often-atypical clinical presentations, and the absence of conventional radiological features at this age. We report an original observation of JNA with endocranial extension in pre-pubertal boy revealed by persistent headache and unilateral exophthalmia.
