Linfoma No-Hodgkin primario de sacro. Presentación de un caso clínico y revisión de laliteratura. 原发性非霍奇金淋巴瘤。临床病例的介绍和母乳喂养的回顾。

Introduction Bone lymphoma is a very rare entity. It constitutes less than 2% of lymphomas and the sacrum is an unusual primary location. Only 11 cases have been described to date. Objective To describe a case of primary sacral lymphoma ina 46-year-old patient after reporting lumbar pain with radiculopathy of one month's evolution. Material and methods A 46-year-old female patient with lumbar pain of one month's evolution of mechanical characteristics with radiculopathy S1 of the right leg. No acute bone injury was observed in the X-rays performed. A CT scan was performed in a private center that reported a lytic lesion of 28 x 97 mm diameter in the right sacral wing with malignant characteristics. MRI is requested, which confirms the lesion with extension to soft tissues. Tumor markers and plasma proteins were not elevated. Biopsywith histopathological analysis reported a diffuse large B-cell lymphoma with positivity for cMYC, BCL2, BLC6, CD20, CD79a, PAX5, MUM1, with a Ki67 index of 75%. Results After diagnosis of primary diffuse large B-cell lymphoma without distant metastasis, chemotherapy treatment was started with 6 cycles of R-CHOP followed by radiotherapy. Conclusion Primary bone lymphoma is a very rare entity, and even more rare at the bone level, but knowledge of this entity is necessary to include it as a differential diagnosis of low back pain with radiculopathy.

Musculoskeletal disorders are one of the most frequent causes of disability, handicap and demand for medical attention. Knowledge of musculoskeletal medicine (MSM) should be essential for most specialties. Unfortunately, many medical interns (MIR) admit to a lack of confidence and competence in this area. The aim of this paper is to perform a narrative review of the level of knowledge and skills acquired in musculoskeletal pathology of medical graduates in different countries. The data show that the teaching of MSM is deficient during the academic period. It’s essential to plan training actions that lead to an increase in knowledge and skills in this pathology in medical students. Optimisation of pregraduate teaching in MME will help to improve healthcare, make the system more agile, and allow better screening of specialised referrals.

Fractura de estrés del cuello del fémur. A propósito de un caso. 股骨颈应力性骨折。关于一个案例。

A fatigue fracture, also known as a stress fracture of the femoral neck, is an uncommon type of fracture that happens when a patient engages in strenuous physical activity or overuse. This case involves a 31-year-old woman who consults the hospital on several occasions with creeping pain in her groin and inner thigh. Following normal X-rays and theabsence of clinical improvement, an MRI and a fresh X-ray were conducted, revealing a loss of continuity of the cortical bone of the femur's neck, which was classified as a stress fracture. The most common consequence of this type of fracture is femoral head necrosis due to delayed diagnosis. As a result, it's critical to get a diagnosis as soon as possible.

The luxation of the astragalus are severe but uncommon injuries. These luxations associates a great sort of complications, so they can be a challenge for the surgeon. Tipically are consequence of high energy impact in young people. Less frequently these injuries happend as a result of low energy impact in people with a higher laxity of their joints or malformations.They often associates talus or ankle fractures, tendon rupture, o neurovascular damage. These lesions are named pure–luxation of the astragalus when there is not bone fracture. This article has the aim of describe the three types of pure–luxation of the astragalus by three case reports.

Fracturas de cuello de radio Jeffrey Tipo II. Revisión de la literatura a propósito de un caso. 杰弗里桡骨颈骨折II型。关于一个案例的文献综述。

Introduction: The elbow is the most frequently dislocated joint in children, while radius neck fractures account for 1% of all pediatric fractures. In the literature, two types of fractures have been described in this location associated with elbow dislocation in which the head is completely displaced with respect to the neck, both types being infrequent. One of them was described by Jeffrey, who postulated that the displacement and fracture were generated from the spontaneous reduction of the transiently dislocated elbow. Case description: Following his description, we are going to present the case of a 9-year-old girl who suffered an isolated radial head fracture-dislocation. After closed reduction, a 180º inversion of the radial head was observed with respect to its metaphysis, requiring a subsequent open reduction and fixation withKirschner wires. Discussion: Since Jeffrey described this fracture mechanism, multiple authors have published cases that reaffirm his hypothesis. In the same way, new maneuvers have been described that allow the correct reduction of this fracture while being the least bloody possible. Conclusion: Our purpose is to carry out a review of the literature regarding this case in order to highlight the importance of a correct initial assessment that Will lead us to the most appropriate treatment at all times.

Wrist fractures are one of the most common fractures in the emergency areas of our hospitals, accounting for 14% of all limb injuries and 17% of fractures treated in the emergency room. The surgery of the same using volar plates has increased inrecent years due to the possibility of blocking the screws, this being the surgical treatment of choice in our country. Among the complications of this type of treatment is tendon injury, due to the synthetic material, both flexors and extensors, being extremely rare the rupture of the Extensor Indicis Propius (EIP) and Extensor Digitorum Comunis of the index finger (EDCi).We present a case of rupture of both tendons after treatment with volar plate, its causes, its resolution and its possibleprevention.

As you well know, obesity is a current health problem. According to the National Health Survey in 2017, 17.43% of Spaniards suffer from it. Furthermore, in our consultations, patients with concomitant arthropathy and obesity are a recurring problem. Currently, there are safe and effective medications for weight loss, which we as orthopedic Surgeons, with adequate knowledge, can prescribe in search of improving the health of patients and surgical results. In this short summary we try to bring this knowledge to specialists in our field.

One of the complications most feared by orthopedic teams is post-surgical infections. In our paper we present the case of a patient who, after a hip osteosynthesis due to a pertrochanteric fracture, suffers an infection that is finally solved thanks totreatment with calcium sulfate beads coated with an antibiotic. This treatment is presented as an option to consider in those cases that are not resolved with conventional therapies since it presents a controlled release of antibiotic directly on the site ofinfection.

Objectives: To analyze the differences in length of stay, readmission and infection in patients undergoing total knee arthroplasty before and after the implementation of a fast-track protocol. Material and methods. Retrospective observational follow-up study in 777 patients. The variables under study were: length of stay, readmission and total number of Emergency Department (ED) visits and rate of infection. Results. A homogeneous sample of 777 patients was obtained with women predominance and mean age of 70 years. The values before and after fast-track implantation were: length of stay 5,1 vs 3,2 days (p<0,001), patients underwent readmission 12,2% vs 21,9% (p<0,001), ED visits 74 vs 118 (p<0,001). Rate of infection was similar in both groups for all levels. Conclusions. Fast-track protocols improve length of stay without affecting the rate of infection. Special care should be given to the increased readmission.

IMPACTO DE LA PANDEMIA POR COVID-19 EN LA EPIDEMIOLOGÍA, MANEJO Y SUPERVIVENCIA DE LOS PACIENTES CON FRACTURA DE CADERA COVID-19大流行对髋关节骨折患者流行病学、管理和生存的影响

The COVID-19 pandemic has forced a reorganization of health resources towards the care of patients with severe respiratory syndrome. While the incidence of traumatic pathology was reduced, this was not the case with hip fractures, and their management could be compromised. The objective of this study is to evaluate the consequences of the pandemic on the epidemiology, management and outcomes of hip fracture. Material and methods: A retrospective observational cohort study was done including patients with hip fracture treated between March 2019 and February 2021, establishing two groups for comparison, before and after the onset of the pandemic. Demographic variables, comorbidities, history of fragility fracture and hip fracture, type of fracture and type of treatment performed, surgical delay, length of hospital stay, and PCR result for SARS-CoV2 upon admission were recorded. In-hospital mortality rate andsurvival at 1 and 6 months were analyzed. Results: 251 patients were included, 137 (54.58%) PRE-COVID group and 114 (45.42%) POST-C group. There were no differences in demographic variables, comorbidities, type of fracture, or treatment performed. The absolute incidence was reduced by 16.8% and the relative incidence was reduced by 17.6%, this difference was significant during lockdown period and in the months o January and February, the period of maximum incidence of COVID-19. The incidence of PCR + on admission was 1.75%. No significant differences were found insurgical delay, hospital stay and mortality, survival at one month or at 6 months. Conclusion: With the COVID-19 pandemic, specific reductions in the incidence of hip fractures have been observed, without changes in its epidemiology,indicators of hospital management and in patient survival at one and 6 months.
