Tibio-Fibular Fusion by Posterolateral Approach to Treat Nonunion of Tibia 后外侧入路胫腓骨融合术治疗胫骨骨不连

A 30- year- old male has sustained open fracture of left leg and undergone 4 surgeries for four years (1. debridement and external fixator, 2. removal of external fixator, 3. vascularized contrafibular autograft plus change new plate/screws of ipsifibula, 4. flap surgery to cover the wound). At the time of examination: unability to walk, abnormal movements of leg, tibial bone loss 8 cm, especially the soft tissue of almost total anterior leg was so bad: no more subcutaneous tissue, skin very next to bone decouraging any attempt anterior approach. Diagnosis: pseudarthrosis of left leg with tibial large bone loss in the status of seriously damaged soft tissue of anterior leg surface. Technique operation chosen: Posterolateral. Appoach, Tibiofibular Fusion with massive autocancellous bone graft harvested from posterior iliac pelvis. Casting 4 months (from midthigh to metatarsal head). Postoperative walking with two crutches, Full Weight Bearing allowed after 14 weeks. Removal of cast after 16 weeks. Full knee flexion obtained 8 weeks after cast removal. Review at tenth month: normal walk, slight running, complete knee Range of Motion, 5. degree loss of dorsiflexion of ankle. X-rays: good fusion proximal and distal tibiofibular spaces and partly fusion the tibio-tibial gap. By this case,we can resumably conclude Tibiofibular Fusion through Posterolateral Approach should be strongly indicated for the leg nonunion in which the soft tissue of anterior surface is not favorable for nearby approach.

Use of Electromagnetic Radiation (EMR) in Preservation of Fruit Juices 电磁辐射在果汁保鲜中的应用

This study shows the bactericidal effect of Electromagnetic Field on fruit juice microbes. Short shelf-life period of fruit juice caused by spoilage organisms has limiting factor for its economy value. The Eighteen microorganisms isolated from both fresh and spoilt fruit samples (Pineapple and Apple), and identified during the study include, twelve (12) bacteria and Six (6) fungi, out of which only the bacterial isolates were exposed to electromagnetic field of 0mG, 500mG, and 5000mG for thirty minutes. The bacteria species were Leuconostoc mesentroides, Bacillus species, Lactobacillus brevis, Microbacterium species, Clostridium species, Bacillus cereus, Acetobacter aceti, and Staphylococcus aureus. The Gram negative bacteria isolates were Erwinia carotovora, Erwinia ananas, and Proteus species. Exposure of the isolates to an electromagnetic field of 0mG, 500mG and 5000mG showed a decrease in some electromagnetic field magnitude. This study shows reduction in growth range among most bacterial species tested at 500mG electromagnetic radiation exposure, but the growth of many of these bacterial species were triggered at 5000mG electromagnetic radiation exposure. This may mean an initiation of: adaptation mechanism, growth mechanism in some microorganism, and sugar content of the fruit juice from which they are being isolated. The exposure of the bacteria to electromagnetic field elicited detectable responses therefore depends on the adaptation mechanism of each bacteria and sugar content of the fruit from which it is being isolated from. Thus, future research can be done to optimize the limits specified for target microbes that are strength and frequency of this EMF in diseases control.

Proper S6K /BTK and PLCγ2 synthesis (which regulated by Ser phosphorylation pathway) are main regulations for thromboxane-A “TXA2” synthesis, and necessary for B-cell maturations and T-cells modulations and functions. The main reasons for causing Osteoarthritis “OA” and diabetes diseases (that are linked together) are the deficiency of Ser amino acids and decreasing of Ser phosphorylation signalling pathway which necessary for proper S6K productions, where normally the Ser phosphorylation signalling pathway is the basis of Ser /Thr phosphorylation signalling and is necessary for proper Akt, S6K1 synthesis and necessary for RORs and IFNs synthesis and also necessary for running proper BTK for PLCγ2 productions , where S6K is main regulator for ATPase and for proper PLCγ1 and for PLCγ2 synthesis which necessary for bone growth and for modulating immune efficiency. Osteoarthritis “OA” is characterized by a sharp expression in Gamma-Phospholipase C-1 “PLCγ1”, with decreasing “or inhibition” in PLCγ2 “PLC beta” productions due to inhibition or mutation in S6K and then in BTK. The increasing in PLCγ1 with Deficiency in Ser amino acids will lead to deficiency in Ser phosphorylation signalling, and decreasing in synthase activity that will reflect down regulations in BTK pathways and lead to inhibition in PLCγ2 productions which will reflect diabetes (inhibition in Estrogen with the production of Androgen instead of estrogen) and can reflect Osteoarthritis “OA” prognosis depend on the percentage of Deficiency or inhibition in basic amino acids and its basic necessary signaling pathways.

This study analyses the current state of a serious and persistent social emergency, the shortage of organs, and discusses possible alternatives for a solution. It should be mentioned in this regard that society is currently experiencing a terrible epidemic with high rates of contamination and critical mortality, COVID 19. This serious situation has generated changes in the medical-social approaches and in the therapeutically resources of habitual solution. Of course organ and tissue transplantation has had to undergo this health crisis. The persistent presence of “organ shortage” before the pandemic showed unsatisfactory statistical results of organ transplants worldwide. Based on the results, the following key factors have been suggested as potentially responsible for this reality, and ideas and conclusions

Molluscum Contangiosum Diagnosis, Manifestation and Management: A review Article 接触性软疣的诊断、表现及治疗综述

Introduction: Infectious skin disease with clinical manifestations of benign papules, which is often caused as a sexually transmitted disease in adults, caused by the molluscum contagiosum virus, a member of the poxvirus. Manifestations of the disease are asymptomatic, discrete, smooth papules. It usually develops from peduncled lesions up to 5 mm in diameter. The incubation period for Molluscum contagiosum is from one to several weeks to 6 months. Molluscum contagiosum is a viral infection that can heal spontaneously. There are 4 main subtypes of Molluscum Contagiosum 4 Virus (MCV), namely MCV I, MCV II, MCV III and MCV IV. These four subtypes cause similar clinical symptoms in the form of papular miliary lesions that are limited to the skin and mucous membranes. MCV I is known to have a greater prevalence than the other three subtypes. About 96.6% of molluscum contagiosum infections are caused by MCV I. However, in patients with decreased immune status, the prevalence of MCV II is 60%.

Cat scratch disease (CSD), caused by the bacterium Bartonella henselae, is usually self-limiting, presenting with low-grade fever and tender lymphadenopathy. With delayed diagnosis or reactivation of latent disease, CSD is associated with severe debilitating symptoms. We are presenting a patient who proved adiagnostic challenge, before being found to have reactivation of Bartonella henselae, requiring three-months IV Trimethoprim-Sulfamethoxazole andHydrocortisone.

The Research and Implications of Organoid Technology 类器官技术的研究与启示

Background: The induction of the formation of organoids derived from induced pluripotent stem cells (iPSC) is a new technology that is being well studied and aims to improve the clinical condition of patients with various diseases and even patients undergoing certain types of procedures. Methods: Systematic review, using the PRISMA protocol. The study period was from 2010 to 2020 and the keywords included “Organoids”, “iPSC”, “Ethic” and “Evidence-Based Medicine”, one at a timeand then combined with the Boolean operator “AND”. Results: 44 registrations were found. Of this total, 15 articles were excluded because they have not approached organoids and stem cells objectively, often attending to other phenomena or details that are not of interest to the review.

Acute Benign Hepatitis Due to Glycogen Hepatopathy 糖原性肝病所致急性良性肝炎

Glycogen hepatopathy is a very rare and forgotten complication of poorly controlled type 1 diabetes mellitus. Glycogen hepatopathy may also present in type II diabetes mellitus, especially when managed with high doses of insulin. Although it is a benign condition, it is rarely diagnosed in a timely manner. It is characterised by hepatomegaly causing abdominal pain due to stretch on the liver capsule causing capsulitis, and derangement of liver enzymes. In this article we report a patient who presented with severe abdominal pain, hepatomegaly and transaminitis, the symptoms persisted for one year before the diagnosis of glycogen hepatopathy was made.
