In removable partial dentures, abutment teeth are essential for support, maintenance, and stability of partial dentures, and are an important factor in the fabrication of functional dentures. If the number of periodontally sound abutments is sufficient and they are located on both sides, it will be easy to manufacture functional dentures with good support, maintenance, and stability. But on the contrary, if the number of teeth is insufficient or if they are located on one side, it is biomechanically disadvantageous and relatively difficult to make functional denture. On the other hand, recently, implant-supported removable dentures, which reinforce the three elements of maintenance, support, and stability by using implant surveyed crown, have been frequently selected as treatment plans. In particular, by adding an implant surveyed crown when only a small number of residual teeth remain, mastication efficiency, esthetics, patient comfort, and pronunciation can be improved. In this case, bilateral posterior extension partial denture was fabricated using implant surveyed crown as abutments by placing implants in the premolars area opposite to the residual tooth unilaterally in the mandible. As a result, the asymmetry of the abutment was resolved, and the maintenance and stability of the denture was achieved. (J Korean Acad Prosthodont 2022;60:19-28)

The development of digital technology is causing great changes in dentistry. This digital workflow combines various 3D data in the prosthetic treatment area for diagnosis and prosthetic manufacturing. The planned diagnosis and the fabrication of prosthesis in a virtual patient formed by synthesizing digital data can simulate the results of prosthetic treatment more intuitively than conventional methods, thereby increasing the predictability of aesthetic prosthetic treatment. In this case report, functionally and aesthetically satisfied clinical results were obtained by fabricating a fixed partial dentures through a digital workflow on congenital missing teeth in the maxillary anterior region. For a full-mouth fixed prosthetic treatment of the edentulous patient, it is essen-tial to confirm the proper tooth position and thorough evaluation of the remain-ing alveolar bone and soft tissue before surgery. CAD-CAM dentistry and guided implant surgery have such advantages of providing simultaneous planning of surgery and prosthetic treatment to ensure pre-knowledge of the treatment. In this clinical case, using the digital technology, digital temporary denture fabrication, esthetic evaluation before fixed prostheses treatment, and guided surgery planning was possible. After the surgery, previously obtained data was used for fabricating fixed temporary prostheses. Definitive zirconia prostheses transferred from the provisional prostheses were fabricated and functionally and esthetically satisfying results were obtained. With the development of digital dentistry, it is being applied in various ways of dental treatment. This case report presents the definitive prosthesis designed in advance with a re-established vertical dimension and the digital technology, which determined the amount of tooth preparation, in order to preserve as much tooth structure as possible in a patient with pathological wear of the posterior teeth and loss of vertical dimension. For accurate tooth preparation, the guides of the occlusal and axial surfaces were digitally and additively manufactured. Then, aesthetics and anterior guidance were established at the provisional stage. The information of the provisional restoration was delivered to the definitive stage by double scanning. The digital technology, including the virtual planning and the guided tooth removal, produced the definitive restorations satisfactory to both the patient and clinician. Installation of dental implants at optimal angles and positions is critical in long-term stable implant-supported restorations. Surgery and prosthodontic proce-dures should be performed accurately as the treatment is planned. In this clinical case, Computer aided design and manufacturing technology was used not only to establish a precise surgical plan, but also to fabricate both provisional and definitive fixed prostheses. A surgical guide was designed to install the implants at proper positions for the definitive prostheses. The patient’s esthetic information, which was necessary for the new provisional and definitive fixed prostheses, was obtained from the existing temporary dentures. Finally, the complete mouth fixed implant-supported rehabilitation using monolithic zirconia provided the patient with functionally and esthetically satisfactory prostheses.
