急性冠症候群の発症機序 においては, 冠動脈壁のプラー クが破裂 し, その内容物が血中に暴露 されることによる血 栓の形成が重要であることが示されている1,2). また, 剖検 例などの検討から, プラーク破裂の組織学的な特徴が検討 され, 破裂 しやすいプラークの特徴 としては, 単球 ・マク ロファージなどの炎症細胞浸潤が強いことが明らかにされ ている3,4). すなわち, 単球の内皮細胞への接着, 内皮下へ の浸潤は, 動脈硬化の初期病変の形成, さらにはその終末 像であるプラークの破綻に重要な役割を果たしている5). 血管内皮細胞は, 生理的な状況下では凝固 ・線溶系や血 小板などと相互に作用し, 血管の抗血栓性を維持 してい る. 一方, 病的状況下, すなわち炎症や感染などでは一時 的に向血栓性が上昇する. そこで, 単球 と内皮細胞のイン ターアクションの急性冠症候群への関わ りについて, 凝 固・線溶の面から検討した6). 方法としては, 内皮細胞はヒ ト臍 帯 静脈 よ り, 単 球 は末 梢 血 よ り分 離 し, 内皮 細 胞 と単
