Pregabalin was synthesized in 1991 and approved for the treatment of neuropathic pain and refractory epilepsy in 2004 and It is one of two available a 2-d ligands, pregabalin and gabapentin, known as the gabapentinoids. Pregabalin and gabapentin share a similar mechanism of action with the difference related to pharmacokinetic and pharmacodynamic characteristics, and pregabalin has a faster onset time and a more predictable absorption profile than gabapentin. [1-2] Surgical and conservative treatments had longterm beneficial effects on sciatica symptoms in patients with lumbar disc herniation. Compared with conservative treatment, surgical treatment relieved back pain faster, but no relevant clinical difference was observed after 3 months. Surgical treatment may thus be attractive to patients with debilitating pain symptoms who seek quick relief, or who did not experience satisfactory improvement with conservative treatment. Fat embolism syndrome (FES) is a rare clinical syndrome characterized by signs and symptoms resulting from fat emboli mostly after orthopedic trauma. It usually occurs between 12 to 72 hours after the insult. It has high mortality and morbidity. Diagnosis of this fatal syndrome is mainly based on clinical presentation. At present, management is predominantly supportive care. Here, we report a case of FES in patient with multiple fractures of left upper and lower limb. Given the absence of a gold standard diagnostic test or pathognomonic feature, he fulfilled the criteria by Lindeque, Schonfield and Gurd. The clinical course of the disease as well as the management in the intensive care unit are discussed.

The prevalence among surgical patients varies between 24-41% depending on the type of surgery and can exceed 70% in bariatric surgeries, being more frequent in males, associated mainly with obesity, diabetes, heart disease and advanced age. [1,3,11,14] Regarding the factors that affect due to anesthetic management, it is due to procedures under general anesthesia, which includes difficulty in ventilation, and difficult intubation, opioidbased analgesia and surgical position. Background: Osteoarthritis (OA) of knee is one of the most common debilitating conditions associated with pain and limitation in daily living activities which negatively affect quality-of-life. It is a disease of the entire joint, involving not only the joint lining but also cartilage, ligaments, and bone. It is characterized by breakdown of the cartilage, bony changes of the joints, deterioration of tendons and ligaments, and various degrees of inflammation of the synovium. Aim of the Work: To discuss the incidence and updates in Pathophysiology of Osteoarthritis (OA) of knee and demonstrate updates in management to solve this problem. Study Design: Narrative review article. Conclusion: The current review revealed that Osteoarthritis (OA) of knee joint has multi mechanism and increases the risk of complications bed redden as such, accounts for burden on economics budget.

Original Research Article To study the effect of alkalinization on the onset action of lignocaine in epidural anaesthesia. After the institutional ethical committee approval and informed consent from the patient, 100 patients posted for lower abdominal surgery were selected after inclusion and exclusion criteria and they were divided into two groups. Group S received Lignocaine with adrenaline added with Saline and Group A received alkalized Lignocaine (with Sodium bicarbonate) with adrenaline. It was concluded that alkalinization shortens the onset of action of Lignocaine.
