Background: One in six adults have experienced four or more Adverse Childhood Experiences (ACEs). Increased exposure to ACEs is associated with increased risk for chronic health problems that manifest during adolescence or earlier. Because these ACEs are deep-rooted in an adolescent’s childhood, they take a multidisciplinary approach to address. Only a low number of pediatric residents and attendings are formally trained on how to screen for or recognize ACEs, however they are front-line providers who identify and interact with these health issues. Teachers are adults who spend a maximum amount …

Introduction: Shelter and food are two core determinants of health. Food insecurity has been linked to poor health outcomes. Since enactment of the National School Lunch Act in 1946, free and discounted school meals have been available for children and youth of low-income families. The goal of this research was to assess social factors that influence enrollment of housing insecure youths into these programs. Method: Youth Hope Foundation provides housing, food, clothing, and education to homeless, runaway, and at-risk youth9. Participants were surveyed by Youth Hope case managers between 2009 and 2019. Information obtained include demographics, social history, …

Background: In an election year with considerable consequences for child health policy, mobilizing patients and families as advocates for children is vital. Little is known, however, about whether and how to engage caregivers around health policy issues affecting families and children. Our cross-sectional, mixed-methods study aimed to assess caregiver interest, acceptance, and perspectives on health policy discussions within the pediatric primary care context. Methods: Twenty-six semi-structured, in-depth interviews were conducted with a consecutive sample of caregivers in three, urban pediatric primary care clinics. …

Background: Empowerment encompasses having the knowledge necessary to influence one’s own behavior and positively impact one’s quality of life. Empowering children with serious illness is important because such children experience lower levels of self-esteem and a lack of independence and resilience, compared to healthy peers. Double H Ranch (HH) is a camp in the Adirondack region of New York that provides children, who would otherwise be ineligible to attend standard camps, unique opportunities to socialize with peers who have similar health experiences. Background: Adverse Childhood Experiences (ACEs) are a recognized childhood epidemic, affecting approximately 34 million children nationally and increasing risk for negative health, behavioral, and developmental outcomes. Pediatricians are uniquely positioned to engage in conversations with families about childhood adversity and toxic stress, and universal ACEs screening can help eliminate selection bias in addressing this topic with families. Several barriers exist in implementing ACEs screening however, leading to questions on how to best incorporate the …

Background: Adverse social conditions in childhood contribute to an increased burden of acute and chronic disease in adult life. Childhood exposure to poverty, financial stress, food insecurity, housing instability, and poor housing quality is associated with stress, academic and cognitive deficits, socio-emotional difficulties, and poor health status. Screening for social determinants of health (SDH) in pediatric practices increases referrals to community-based resources, decreases social needs, and improves parent-reported child health. The American Academy of Pediatrics (AAP) therefore recommends screening for SDH at every health supervision visit from birth until age 21. However, …

Importance: Contemporary health care reform strives to fulfill the “triple aim” by, 1) reducing costs, 2) improving the care experience, and 3) improving population health. There is great need for such reform in caring for pediatric patients experiencing medical complexity and social vulnerability given that this subset of patients makes up a disproportionately large percentage of total health care utilization and expenditures. Novel Interventions in Children’s Healthcare (NICH), a novel clinical behavioral health program, was developed to address the triple aim for pediatric patients experiencing both medical and social complexity. This study describes initial NICH pilot …
