A 51-year-old female was admitted to the gastroenterology clinic in October 2020 because of a 2-year history of epigastric pain unrelated to food intake. No diarrhea was noted. Her documented medical history was notable for well-controlled hypertension. Her physical examination and Helicobacter pylori antigen detection showed no remarkable abnormalities. A computed tomography scan of the abdomen revealed a 0.5 x 0.5 cm well-defined hyperattenuating structure in the wall of the descending duodenum, which was confirmed by esophagogastroduodenoscopy demonstrating a 0.5 cm nodule in the descending duodenum (Fig. A). The lesion was located within the deep mucosa and submucosa without invasion of adjacent structures detected by endoscopic ultrasound (Fig. B). Endoscopic submucosal dissection was performed to remove the nodule despite the high risk for postoperative complications. Microscopic examination of the specimen by hematoxylin and eosin staining (Fig. C) and the lesion staining strongly immunoreactive for synaptophysin (Fig. D) and chromogranin (Fig. E) by immunohistochemistry were consistent with a well-differentiated grade 2 neuroendocrine neoplasm, which was further defined by a low proliferation index Ki67 around 3% in neoplastic cells (Fig. F). No etiology for patient’s epigastric pain was discovered on esophagogastroduodenoscopic examination. Duodenal neuroendocrine tumors consist of 2% of all gastrointestinal neuroendocrine tumors and 1% of all duodenal tumors, which are very rare (Baliss et al., 2021). The rarity of duodenal neuroendocrine tumors coupled with the absence of neuroendocrine clinical syndromes creates a noteworthy diagnostic challenge (Delle et al., 2016). Patients with nonfunctional and nonmetastatic duodenal neuroendocrine tumors should be considered for resection regardless of tumor size (Sato et al., 2016).

Background: Although nerve conduction study (NCS) represents the commonly performed test to functionally confirm the presence of carpal tunnel syndrome (CTS) in the daily clinical practice, the value of ultrasonography (U/S) is well established as a diagnostic tool for structural evaluation of CTS. Τhe purpose of this study was to compare U/S and nerve conduction velocity in patients with clinically diagnosed CTS, for assessing the usefulness of U/S in determining CTS severity. Material and Methods: A cross-sectional study with prospective data collection was carried out. 71 patients (100 hands) with clinically diagnosed idiopathic CTS were included in this study. Τhe protocol was comprised of a NCS and an U/S where the cross sectional area (CSA) and the flattening ratio (FR) of the median nerve at the carpal tunnel inlet (level of the pisiform) were measured. Results: In 94 hands (65 patients) out of 100 hands (71 patients), CTS was electrophysiologically confirmed. The CSA increase of the median nerve was proportional to the electrophysiological severity of the CTS. ROC analysis demonstrated good diagnostic value in both CSA and FR (CSA: AUC = 0.974, p<0.001, FR: AUC = 0.928, p <0.001). The CSA at the tunnel inlet with a threshold of 10.5 mm2 had the best diagnostic accuracy with a sensitivity of 94.7% and a specificity of 92.5%, as well as the FR with a threshold of 3.1 had the best diagnostic accuracy with a sensitivity and a specificity of 78.7% and 98.8%, respectively. Furthermore, ROC curve analysis for CTS electrophysiological severity showed a superior performance of the CSA (AUC=0.911, p<0.001) compared to the FR (AUC=0.647, p=0.023). The optimal cut-off point of 15.5 mm2 for CSA at the carpal tunnel inlet was defined to determine severe CTS, which yielded a sensitivity of 82.8% and a specificity of 92.3%. Conclusions: The U/S of the median nerve combined with electrodiagnostic study, increases significantly the sensitivity and reliability of the patient's diagnostic approach suffering from CTS. It can also be used for the assessment of CTS severity, replacing NCS, as its benefits such as low cost, non-invasive procedure, less time and patient’s convenience are remarkable.

The occurrence of regressive autism is closely related to the disturbance of immunity and intestinal microbiota. The intestinal microbiota plays an important role in maintaining the balance of the human immune system, and also regulated central nervous system activity. Soil is a micro-ecological environment that contains the majority of all known microbial species. The number of species found in human feces is around 1/10 times of that found in soil. Therefore, soil can be considered as the microbial “seed bank”. Our previous research indicated that everyday living environments are too clean, which, combined with the lack of contact with soil, results in the loss of a large number of microorganisms, and eating soil increased the gut microbial diversity and immune function in mice. As a special soil, Fulonggan (Terra Flava Usta) is a safe traditional Chinese medicine that has been used in the clinic for 1500 years. Here we report a case of 4.5-year-old male regressive autism with comorbidities of food allergy, allergic rhinitis, and constipation. Fulonggan was used to treat for 12 months from the age of 52 months to 63 months. For the first month of taking Fulonggan, he was also receiving the allergy medicine Singulair (montelukast). The constipation disappeared 5 days after taking Fulonggan, the allergic rhinitis disappeared 4 months later, and the food allergy disappeared after the treatment was stopped. Furthermore, the language skills, social skills, and mood were obviously better according to the Gesell Developmental Scale–Chinese Revised Version and Autism Behavior Checklist. At present, the child has returned to kindergarten. This report proposes a new method for the treatments of regressive autism and comorbidities including food allergy, allergic rhinitis, and constipation. The therapeutic method is effective and safe, and worth to be an alternative for regressive autism or allergy diseases.

Drug abuse in young people is a public health problem worldwide. In Mexico it is also a social problem. Methodology: Descriptive, cross-sectional study, the sample was random. A survey and Excel base analysis were used. Objective: Identify knowledge and consumption of drugs in adolescents of basic secondary education. Discussion: Schoolchildren in Mexico have extensive knowledge about drugs, as in other countries in the world. Marijuana is mostly consumed, followed by cocaine, the main factors that led to consumption were problems, friends, curiosity, depression and combinations of these factors. Men are the biggest consumers, starting at 13 years of age. Consumers report that it is not a major problem, but not in all schools. More and more scientific papers are warning about the progressive normalization of the consumption of illicit substances among young people.

years after resection without recognition of IgG4-RD. As the primary skin lesion prior to the systemic symptoms was rare and mostly appeared in head and neck (Charrow et al ., 2016). Once again at this recurrence, cutaneous dermatofibroma wasn’t initially identified as the primary symptom of IgG4 -RD. Therefore, IgG4-RD should be fully considered and evaluated in clinical practices to avoid unnecessary and potentially harmful operations.

Air pollution is currently a major concern for human health and the environment of the world. Rapid growth of metropolitan cities has led to an increase of vehicles and industries which emits toxic substances. India is experiencing a major growing risk factor for Human Health. Due to high concentrations of particulate and gaseous material, toxic metals and aldehydes, with a significant increase in acute cardiopulmonary mortality, particularly in certain high-risk population subgroups, and with an acute increase in hospitalizations for cardiovascular and pulmonary diseases.
