Effect of prematurely primary front teeth loss with anterior esthetic fixed appliance on fricative production in Arabic-speaking children 前牙美容固定矫治器对阿拉伯语儿童乳牙过早脱落产生摩擦的影响

Abstract Background Primary teeth affect the life quality regarding chewing, speech production, esthetics, and psychological well-being. Young children usually lose their primary maxillary incisors due to trauma or care and that leads to fault production of alveolar and dental fricatives. Fixed esthetic appliance replacing the lost primary front teeth were inserted and fricative sounds /s/,/ s /,/∫ /,/z/and / z / were tested before and after insertion of the fixed appliance through different time intervals. Results Improvement of fricative sound production occurred after fixed appliance insertion the /s/ sound, followed by / s / sound showed immediate improvement while the /∫ / sound was the least to be improved even after 1 month duration. Both sounds / z / and /z/ sound showed distortion immediately. Conclusion Premature loss of primary incisors in young children has a significant impact on speech production.

Endonasal endoscopic nasopharyngectomy for nasopharyngeal malignancies: a survival analysis 鼻内镜鼻咽切除术治疗鼻咽恶性肿瘤:生存分析

Abstract Introduction Nasopharyngeal cancer has a global incidence of less than 1.0 per 100,000 person-years. It originates from nasopharyngeal mucosa with the Fossa of Rosenmuller being the most common site. Radiation therapy with or without chemotherapy is an established first-line treatment modality given the sensitivity of the tumor. Objectives The aim of our study was to report survival outcomes amongst patients undergoing endonasal endoscopic nasopharyngectomy at a tertiary care referral center. Methods We conducted a retrospective longitudinal cohort study. We collected data on age, gender, prior treatment, histology, extent of surgery, post-operative adjuvant therapy, and recurrence and reported the survival along mean survival time using Kaplan–Meier curves and log-rank test. Results A total of six patients, three males and three females with a mean age of 43.7 years were included in our study. Of the six, three patients underwent a salvage procedure whereas three patients received upfront definitive surgical resection. Three patients developed recurrence with a mean survival time of 19.3 months. The disease-free survival at 1 and 2 years of surgery was 88.3% and 66% respectively. Conclusion Endonasal endoscopic nasopharyngectomy is an effective surgical procedure that can be undertaken for both salvage and primary cases. Our survival results are comparable to the literature.

Abstract Background Coronaviruses are large, encapsulated RNA viruses that can infect both humans and animals and cause minor respiratory illnesses. In December 2019, numerous cases of pneumonia of unknown origin were reported in Wuhan, China. Coronavirus Disease 2019 (COVID-19), the cause of these cases, was discovered on January 6, 2020. The new coronavirus was declared an epidemic by the WHO on March, 2020. Several studies on COVID-19 have found that auditory complaints and hearing impairment can be detected using various tests. Objectives Pure tone audiometry (PTA) and transient evoked otoacoustic emissions (TEOAEs) were used to assess hearing in recovered SARS-CoV-2 patients. Also, to compare the presence of patient's auditory complaints with the test findings. Methods A case- control study was conducted, with each case and control group consisting of 58 people who were age and sex matched and ranged in age from 18 to 50 years. PTA, Extended PTA and, TEOAEs were used to evaluate hearing in both groups. Results PTA revealed a statistically significant difference in right ear thresholds at 250 Hz, 500 Hz, 4 kHz, and 8 kHz and left ear thresholds at 250 Hz, 4 kHz, 8 kHz, and 12.5 kHz between patients and controls. Additionally, a statistically significant difference in TEOAEs' overall reproducibility and amplitude between patients and controls was discovered. Affection for PTA and TEOAEs were related to the patient's complaints of hearing loss and tinnitus, respectively. Conclusions Whether a patient is symptomatic or not, COVID-19 may have a negative impact on their hearing.

Vestibular evoked myogenic potentials and videonystagmography findings in type 2 diabetes mellitus patients with and without polyneuropathy 伴有和不伴有多发性神经病变的2型糖尿病患者的前庭诱发肌源电位和视震图表现

Abstract Background Vestibular impairment is a common pathology in patients with type 2 diabetes mellitus (DM) due to ischemia of the vestibular end organs and lysis of the myelin of the vestibular nerve. We aimed to evaluate function of the vestibular end organs and vestibular nerve in patients with type 2 DM with polyneuropathy and compare results to those of the patients without polyneuropathy. Method The participants consisted of three groups: thirty patients with type 2 diabetes mellitus without polyneuropathy (DM), thirty patients with type 2 diabetes mellitus with polyneuropathy (DPN), and thirty healthy non-diabetic individuals as the control group. Clinical examination, videonystagmography, cervical vestibular evoked myogenic potential(cVEMP), ocular vestibular evoked myogenic potential (oVEMP), Gans Sensory Organization Performance (SOP) test, and nerve conduction study were all performed on all groups. Results P1 and N1 cVEMP latencies and n1 and p1 oVEMP latencies were statistically significantly delayed in both the DM and DPN groups than the control. Also, amplitudes of P1-N1 and n1-p1 were statistically significantly lower in the DM and DPN groups than the control group. DPN patients had longer latencies and lower amplitude of the cVEMP and oVEMP response compared to diabetes patients without neuropathy. There was a statistically significant prevalence of BPPV in the DPN group compared to the control and DM groups. DPN included the following: 4 (13.3%) had vestibular pattern, 12 had polyneuropathic pattern, and 5 (16.7%) had vestibular and polyneuropathic pattern as regards the Gans Sensory Organization Performance (SOP) test. Conclusion VEMP is considered a promising objective tool in the assessment of the vestibular end organ disorders in patients with type 2 DM with and without polyneuropathy. Diabetics with prominent diabetic polyneuropathy showed higher vestibular impairment than diabetics without DPN, which may increase the risk of falling.

We aimed to evaluate function of the vestibular end organs and vestibular nerve in patients with type 2 DM with polyneuropathy and compare results to those of the patients without polyneuropathy. Method The participants consisted of three groups: thirty patients with type 2 diabetes mellitus without polyneuropathy (DM), thirty patients with type 2 diabetes mellitus with polyneuropathy (DPN), and thirty healthy non-diabetic individuals as the control group. Clinical examination, videonystagmography, cervical vestibular evoked myogenic potential(cVEMP), ocular vestibular evoked myogenic potential (oVEMP), Gans Sensory Organization Performance (SOP) test, and nerve conduction study were all performed on all groups. Results P1 and N1 cVEMP latencies and n1 and p1 oVEMP latencies were statistically significantly delayed in both the DM and DPN groups than the control. Also, amplitudes of P1-N1 and n1-p1 were statistically significantly lower in the DM and DPN groups than the control group. DPN patients had longer latencies and lower amplitude of the cVEMP and oVEMP response compared to diabetes patients without neuropathy. There was a statistically significant prevalence of BPPV in the DPN group compared to the control and DM groups. DPN included the following: 4 (13.3%) had vestibular pattern, 12 had polyneuropathic pattern, and 5 (16.7%) had vestibular and polyneuropathic pattern as regards the Gans Sensory Organization Performance (SOP) test. Conclusion VEMP is considered a promising objective tool in the assessment of the vestibular end organ disorders in patients with type 2 DM with and without polyneuropathy. A novel approach to spontaneous retropharyngeal hematoma—a case report 一种治疗自发性咽后血肿的新方法——一例报告

Abstract Background Retropharyngeal hematoma is a rare but life-threatening situation and may require emergency tracheostomy or surgical drainage if the airway is compromised. We present a case of a retropharyngeal hematoma with shown active submucosal bleeding and describe the novel approach taken for compression and control of the hemorrhage. Case presentation A 76-year-old man presented to our institution complaining of dysphagia and pharyngeal discomfort hematoma since that morning. The laryngoscopy showed a hematoma of the posterior wall of the hypopharynx. A computerized tomography scan with contrast was done, demonstrating a collection in the retropharyngeal space, and a small active vessel could be identified. The patient was intubated, and compression was performed in the location of the retropharyngeal hematoma using the esophageal balloon of a Blakemore tube, with successful control of the active bleed. Conclusions Retropharyngeal hematoma can be fatal, and we present a unique solution that can be used to successfully stop the bleeding while avoiding more invasive strategies, by performing compression with a Blakemore esophageal balloon to stop the bleeding.
