ENDOSCOPIC PLANTAR FASCIA RELEASE IN CASES OF CHRONIC RESISTANT PLANTAR FASCIOPATHY 慢性难治性足底筋膜病的内镜下足底筋膜释放

Purpose : The intention of this study was to evaluate the efficacy of endoscopic plantar fascia release in the therapy of resistive plantar fasciopathy. Material and Methods: 30 individuals with plantar fasciopathy who had been recalcitrant to at least two forms of conservative therapy for at least six months were selected to participate in a prospective research.. Most of our patients had already been clinically diagnosed. Results: At 6 months after operation, the average AOFAS total score was significantly increased from 35 (8-62) up to 85.5 (range, 71-100), (P < 0.001). The median VAS score was significantly dropped from 10 (interquartile range, 9.25-10) up to 0 (interquartile range, 0-2.75) (P < 0.0001). The percentage of patients who scored good or exceptional on the Roles and Maudsley scale had dramatically risen after 4 weeks, rising from 73% to 93% (28/30). There were no serious adverse events. Conclusion: Endoscopic plantar fascia release is an efficient medication for people with relentless resistant plantar fasciopathy.

Background: Despite recent advances, the difficulties involved in management of bone infections continue to plague the practice of orthopedics, this study reviews the clinical results using biodegradable antibiotics impregnated calcium sulfate (AICS) beads in 24 patients who suffered from osteomyelitis. Aims of the work: evaluation the role of antibiotics impregnated calcium sulfate in eradication of infection in case of osteomyelitis. all patient prepared for surgical debridement and calcium sulfate Patient and methods impregnated with vancomycin and gentamycin beads were inserted into the bone cavity and surrounding soft tissue. Results : this study include 24 patients, 17 (70.83%) patients were males while 7 (29.17%) patients were females, The mean follow-up was 9 months (range from 6-12 months), the mean age of the patients was 33.2 ± 19.5& tibia is the most common affected bone & Staphylococcus aureus was the organism that had been reported in 45.83% of all swabs, In the final results 21(87.5%) patients were completely healed while only3 (12.5%) patients still had infection. Conclusion : antibiotics impregnated calcium sulfate is safe and effective method in treatment of osteomyelitis and the results show high rate of eradication of infection

COMPARATIVE STUDY BETWEEN EXTERNAL FIXATION WITH MONO-LATERAL EXTERNAL FIXATOR AND INTERNAL FIXATION OF CORRECTIVE OSTEOTOMY OF KNEE DEFORMITY IN CHILDREN 单外侧外固定架外固定与儿童膝关节畸形矫正截骨内固定的比较研究

Background: knee deformity is one of the most common presentations in pediatric orthopaedic clinic. It could be pathological or normal as a result of skeletal maturation. The gold standard for treating knee deformity is osteotomy. Normalization of the mechanical axis helps distribution of stress of body weight on all knee compartments and ligaments so prevent early development of knee osteoarthritis. Patients: It is a prospective study of 56 patients with knee deformity .genu varus having metaphyseal-diaphyseal angle greater than 11, genu valgus with a greater than 15-degree tibio-femoral angle, full extension range of motion and flexion greater than 100, exclusion of genu recuravatum, and limb length discrepancy. The patients' ages range from 10 to 18 years. Methods: Each patient assessed clinically for knee range of motion and pain as well as their radiological deformity correction and union. A nine-month follow-up period. Results: The results of patients operated by external fixation by monolateral external fixator were as follows: The deformity in all the 28 patients had been corrected. Mild pain was experienced in 16 cases (57.14%), moderate pain was experienced in 8 cases (28.57%) and severe pain was experienced in 4 cases (14.28%). The duration of union (in months) had a mean value of 2.23. Pin tract infection occurred in 14 patients (50%), 2 patient had superficial infection at the osteotomy site (8.33%) and no infection was encountered in 12 patients (41.67%). The results of patients operated by internal fixation were as follows: The duration of union in all the 28 patients was about 3.5 months. The pain score after union was mild in 14 cases (50%) and moderate in 14 cases (50%) patients. ROM of knee was >90 degree in 14 cases (50%) and >100 degree in 14 cases (50%). Wound infection occurred in 4 cases (14.29%) and no infection occurred in 24 cases (85.71%).

OPEN REDUCTION AND INTERNAL FIXATION OF ACUTE DISPLACED LATERAL CLAVICLE FRACTURES BY INTRAMEDULLARY SCREW MID-TERM RESULTS AND A BRIEF LITERATURE OVERVIEW 髓内螺钉切开复位内固定治疗急性移位的锁骨外侧骨折中期结果及简要文献综述

Aim: To investigate the use of intramedullary screw for fixation of lateral third clavicle fractures including indications, preoperative planning, operative technique, and complications during the study period. Setting and design: Prospectively we used intramedullary screw for fixation of lateral third clavicle fractures either non-displaced, minimally displaced guided by fluoroscopy. Patients and methods: The study included 20 patients after obtaining informed consent who underwent mid-shaft clavicle fractures using intramedullary screw fixation between February 2020 and January 2022. Statistical analysis used: The software for statistical package for social sciences for Windows 10 was used for all statistical calculations. Each variable was tested for its normal value using the Mann-Whitney Test and exact P values were calculated. Significance was set at the P value of less than 0.05. Results: All cases had “good to excellent” functional results according to the Constant and Murley shoulder score. Conclusion: Our results show that intramedullary screw fixation of lateral third clavicle fractures inserted from the lateral to the medial fragment is a simple, reliable and effective method of treatment with overall satisfactory functional and cosmetic results and low morbidity.

THE CLINICAL AND RADIOLOGICAL OUTCOMES OF THE USE OF ELASTIC INTRA-MEDULLARY NAILS IN THE FIXATION OF FRESH NON-COMMINUTED MID-SHAFT CLAVICLE FRACTURES IN ADULTS. 弹性髓内钉固定成人新发非粉碎性锁骨中段骨折的临床和影像学结果。

Aim: Clavicular fractures make up 5% to 10% of all fractures and 35% to 45% of injuries to the shoulder girdle. About 80% of all clavicular fractures are mid-shaft fractures, with lateral third fractures coming in second (12%-15%) and medial third fractures coming in third (5%–6%). Conservative treatment, such as a figure-of-eight bandage, is typically used to treat non-displaced mid-clavicular fractures. Absolute indications for surgical treatment include the floating shoulder, impending or existing neurovascular impairment, and imminent skin perforation. Relative indications for surgical fixing include non-unions and severe displacement of fracture fragments. The most common surgical procedure is plate osteosynthesis, but this method has been linked to increased rates of complications. This study's objective is to assess the clinical and radiological results of employing elastic intra-medullary nails to treat adults with fresh, non-comminuted mid-shaft Clavicle fractures. Patients and Methods : A prospective study was conducted on 20 adult patients treated with an elastic intra-medullary nail for non-comminuted mid-shaft clavicle fractures between August 2021 and August 2022. Results : According to the Constant and Murley shoulder score, the mean score at the conclusion of the follow-up period was 93, with a range of 58 to 100. 13 of the patients (or 65%) had great results, 4 (20%) had good results, 2 (10%) had fair results, and 1 (or 5%) had subpar results. Conclusion : Using an intramedullary elastic nail to fixe non-comminuted mid-shaft clavicular fractures is a successful course of treatment in some circumstances. All of the cases included in the study experienced complete union over the follow-up period, and most of them had good range of motion, strength and cosmetically accepted scar.

IS POSTERIOR-ONLY RIGID FIXATION AN EFFICIENT OPTION FOR THE TREATMENT OF UNSTABLE LATERAL COMPRESSION PEDIATRIC PELVIC FRACTURES? 单纯后路固定是治疗不稳定侧压型儿童骨盆骨折的有效选择吗?

Background: Pediatric pelvic fractures (PPF) are rare injuries and usually resulted from high-energy mechanisms. Unstable lateral compression fractures may leave permanent deformity with pelvic asymmetry . The purpose of this study was to evaluate the safety and efficacy of rigid posterior fixation for treatment of unstable lateral compression pelvic fractures in children. Methods: A retrospective case series of 13 patients with unstable lateral compression pelvic fractures. All cases were treated in a university hospital between January 2013 and May 2020. Inclusion criteria were: all patients under 16 years old, hemodynamically stable and who were treated posteriorly using rigid posterior fixation with a minimum follow-up of 12 months. Assessment was done clinically utilizing Majeed score and radiologically utilizing Matta score. Results: There were 8 boys and 5 girls with a mean age of 9.5 ± 4 years. There were 8 children with an immature pelvis. The most common causes of injury were motor car accident (61.5%). The interval from injury to surgery was 2-11 days with a mean of 6 ± 2 days. The mean intraoperative time was 81.9 ± 18.3 minutes with a mean operative blood loss of 113.9 ± 48 ml. According to the Matta and Tornetta score; the reduction was rated excellent in 10 patients and good in 3 patients. According to the Majeed score, the outcome was rated excellent in 11 patients and good in 2 patients. Conclusion: Posterior-only rigid fixation by interfragmentary intrailiac screws and plates is a safe and effective method for treatment of unstable lateral compression fractures in children.

Inclusion criteria were: all patients under 16 years old, hemodynamically stable and who were treated posteriorly using rigid posterior fixation with a minimum follow-up of 12 months. Assessment was done clinically utilizing Majeed score and radiologically utilizing Matta score. Results: There were 8 boys and 5 girls with a mean age of 9.5 ± 4 years. There were 8 children with an immature pelvis. The most common causes of injury were motor car accident (61.5%). The interval from injury to surgery was 2-11 days with a mean of 6 ± 2 days. The mean intraoperative time was 81.9 ± 18.3 minutes with a mean operative blood loss of 113.9 ± 48 ml. According to the Matta and Tornetta score; the reduction was rated excellent in 10 patients and good in 3 patients. According to the Majeed score, the outcome was rated excellent in 11 patients and good in 2 patients.
