The Effectiveness of Using Breathing Exercise on Sleep Quality Among Hospitalized Patients 呼吸运动对住院患者睡眠质量的影响

Sleep disturbance among hospitalized patients has many factors especially at critical care units. Daily breathing exercises as pursed lip breathing (PLB) and diaphragmatic breathing improves sleep quality and reduces the occurrence of delirium. Aim of the study Evaluate the effectiveness of using breathing exercise on sleep quality among hospitalized patients. Design quasi experimental design. Sample A purposive sample including 152 patients were divided into four equal groups two from the Medical ward (study and control) 38 patients each group and two form the Medical Intensive Care Unit (MICU) (study and control) 38 each group at Minia university hospital. Results majority of the groups were male; all patients were in the middle age. (39% and 44.7%) of the medical ward groups (study and control) and (52.6% and 44.7%) respectively of the MICU groups had sever sleep problem. But after applying breathing exercise for 5 continuous days, the study groups (44.7%) of the medical ward and (50%) of the MICU had great sleep quality. Majority of study group’s patients at the medical ward and MICU had no Confusion,” with percentage (60.5% and 52.6%) respectively which mean no presence of delirium. Conclusion breathing exercise had significance effect on improving sleeping quality and reduce the occurrence of delirium among the study groups than the control groups.

Background: High tibial osteotomy is preferential treatment for patients with knee osteoarthritis. Strengthening exercise program helps post (HTO) patients achieve greater functioning during sport, recreation, and activities of daily living after surgery. Objective: To examine the effect of postoperative exercise program on knee physical function among high tibial osteotomy patients. Method: Design: quasi experimental research design. Setting: Department of Orthopedic Surgery and orthopedic Outpatient Clinic at Assiut University Hospital. Sample: A purposive sample of 60 adult patients undergoing high tibial osteotomy were included in our study. Method: Knee Outcome Survey Activities of Daily Living Scale and Lysholm knee scoring scale were used. Results: A highly statistically significant improvement of knee Function score and Lysholm knee score (<0.001**) among patients with high tibial osteotomy. Conclusion: Application of exercise program had a statistically significant effect on improvement physical function of knee among the study group than among the control group. Recommendations: Successful postoperative exercises should be practiced for each patient undergoing high tibial osteotomy in order to improving physical function of the knee.

Nursing Care of an Extremely Preterm Infant with Sodium Taurocholate Cotransporting Polypeptide Deficiency: A Case Report 牛磺酸胆酸钠共转运多肽缺乏症极早产儿的护理1例

Sodium taurocholate cotransporting polypeptide (NTCP) is a protein encoded by the solute carrier family 10 member 1 gene and expressed in the basolateral membrane of the hepatocyte to uptake conjugated bile acids from the plasma. This study reported an extremely premature infant (EPI) with NTCP deficiency (NTCPD). The patient presented with jaundice, persistently elevated total bile acids, 25-(OH)-VitD deficiency, and cleft palate. Nursing care mainly focused on disease surveillance, jaundice care, nutrition, kangaroo mother care, and psychological care. In response to 5 months of the nursing care, the infant’s weight reached 3.324 kg, the jaundice was alleviated, and the infant gained the ability to suck milk. However, his TBA levels were abnormally elevated, and 25-(OH)-VitD was still deficient when the patient was discharged from hospital. The nurses taught the patient’s parents the importance of monitoring liver function and trace elements during a routine outpatient visit. Telephone follow-up 1 year later showed that the patient was in good health with no obvious clinical manifestations. This article reports our experiences in caring for an EPI with NTCPD. To the best of our knowledge, this is the first case report worldwide in NTCPD nursing care.

Nursing Experience of CT Examination in a Case of Critical Elderly Male Patients with Neck Injury: A Case Report 老年男性颈部损伤危重患者CT检查1例的护理体会

We present a 63-years-old male who are accidentally fell down while riding a bicycle and was inserted into his neck by a 4cm thick bamboo pole on the morning of August 30, 2019. In rescue process, patient’s mental status is bad, that the patient's state of mind is in tension, fear, anxiety and restlessness. the patient takes out stitches on September 9, 10 days rehabilitation hospital discharge. As the patient had pale complexion, slightly shortness of breath, temporarily stable vital signs, and multiple blood scabs formed throughout the body, he underwent neck CT plain scan, enhancement, 3D reconstruction, chest CT plain scan, enhancement and 3D reconstruction. After that, the patient was sent to operating room, he received suitable treatment. Furthermore, the sharing of patient information was a great help in this rescue. The local hospital provides enough patient information to our CT examine. the difficulty of this rescue lies in whether the patient can complete CT examination smoothly. In nursing aspect, we not only fixed the patient's body position and keep the patient's breathing open to prevent secondary injury but also provide psychological counseling and comfort to patients so that we ensure that the mental health of the patient is good.

Application of CSHA Frailty Index and Clinical Frailty Scale in Geriatric Assessment of Elderly Males in China CSHA衰弱指数与临床衰弱量表在中国老年男性老年评估中的应用

People at an advanced age often live with multiple chronic diseases as well as disabilities. To access the health condition of elderly persons, various models have been used in developed countries in the West. However, it is not known how these models apply to the elderly in Asian developing countries. This study investigated the application of the Clinical Frailty Scale-09, a widely used method in patient assessment in Western countries, to older adults living in mainland China. Two hundred and ten elderly males were assessed for their health conditions using a list of variables by the Canadian Study of Health and Aging to construct the 70-item CSHA Frailty Index. The obtained Frailty Indices were compared with the scores of The Clinical Frailty Scale-09 for the same sample group. The assessment revealed the changing pattern in the health condition of the sampled population. Compared to the group aged 65-74 years old, the Frailty Index and the Clinical Frailty Scale increased in the groups aged 75-84 and 85-89 years old. The greatest increase was in the group aged ≥90 years old. The scores of the Frailty Index and the Clinical Frailty Scale-09 correlate with each other. These findings suggest that the Frailty Index and Clinical Frailty Scale-09 provide reliable assessment of the health condition of elderly Chinese males.

Effect of Mindfulness-decompression on Stresspsychology and Sleep of Heterogeneous Hematopoietic Stem Cell Transplantation Donors 正念减压对异种造血干细胞移植供者应激心理和睡眠的影响

Objective: To investigate the effect of mindfulness decompression on psychological status and sleep quality of allogeneic hematopoietic stem cell transplantation donors. Methods: From January 2017 to June 2019, 86 donors of allogeneic hematopoietic stem cell transplantation in our hospital were selected and divided into the control group and observation group with 43 cases in each group according to random numbertable method. The control group received routine nursing during the perioperative period, while the observation group received psychological intervention of mindfulminant decompression on the basis of routine nursing. Before and after the intervention, the emotional state and sleep quality of the donors were evaluated by the self-rating anxiety scale (SAS), self-rating depression Scale (SDS) and Pittsburgh Sleep Quality Index (PSQI). Results: The preoperative SAS, SDS and PSQI scores were not significantly different. (P>0.05). however, the post treamtment score of SAS, SDS and PSQI in observation group were significantly lower than the control group (P<0.05). Conclusion: Mindfulness decompression can significantly improve preoperative anxiety and depression of allogeneic hematopoietic stem cell transplantation donors, and improve sleep quality, which is worth popularizing in clinical practice.
目的:探讨正念减压对异基因造血干细胞移植供者心理状态和睡眠质量的影响。方法:选取2017年1月~ 2019年6月我院同种异体造血干细胞移植供体86例,按随机数字法分为对照组和观察组,每组43例。对照组围手术期给予常规护理,观察组在常规护理的基础上给予正念减压的心理干预。干预前后采用焦虑自评量表(SAS)、抑郁自评量表(SDS)和匹兹堡睡眠质量指数(PSQI)评价供者的情绪状态和睡眠质量。结果:术前SAS、SDS、PSQI评分差异无统计学意义。(P > 0.05)。观察组患者治疗后SAS、SDS、PSQI评分均显著低于对照组(P<0.05)。结论:正念减压能显著改善同种异体造血干细胞移植供者术前焦虑、抑郁,改善睡眠质量,值得在临床推广。

Methods: From January 2017 to June 2019, 86 donors of allogeneic hematopoietic stem cell transplantation in our hospital were selected and divided into the control group and observation group with 43 cases in each group according to random numbertable method. The control group received routine nursing during the perioperative period, while the observation group received psychological intervention of mindfulminant decompression on the basis of routine nursing. Before and after the intervention, the emotional state and sleep quality of the donors were evaluated by the self-rating anxiety scale (SAS), self-rating depression Scale (SDS) and Pittsburgh Sleep Quality Index (PSQI). Results: The preoperative SAS, SDS and PSQI scores were not significantly different. (P>0.05). however, the post treamtment score of SAS, SDS and PSQI in observation group were significantly lower than the control group (P<0.05). Analysis on Related Factors and Nursing Interventions of Vasovagal Syncope During Ultrasound-guided PICC 超声引导PICC血管迷走神经性晕厥相关因素分析及护理措施

Purpose: To investigate the causes and the nursing interventions of vasovagal syncope (VVS) during ultrasound-guided Modified Seldinger Technique (MST) PICC catheterization. Methods: Clinical data (medical records, demographic data, diagnosis, local vascular compression situation, B-mode ultrasound-recorded catheter/vessel ratio and the occurrence time, symptoms and treatment of syncope) of 14 patients diagnosed as vasovagal syncope during ultrasound-guided PICC catheterization at three hospitals in Guangdong province from September 2018 to September 2019 were collected for this retrospective analysis. Results: There were 3091 cases of ultrasound-guided MST PICC catheterization in the venous catheter outpatient clinics of the 3 hospitals from September 2018 to October 2019 with 14 cases (0.45%) of intraoperative vasovagal syncope, all cancer patients (5 males, 9 females) with a mean age 45± 3.8. They all received PICC for the first time and were under mental stress during the operation. Among the 14 patients (2 mild, 7 moderate, 5 severe), 8 had multiple lymphadenectasis in the neck and clavicular areas of the catheterization side, which resulted in different degree of venous compression and difficulty in insertion; 5 had catheter/vessel ratio over 45%. 2 cases of VVS occurred when the patients were asked to turn their heads against shoulders on to the puncture side; 10 occurred when using B-mode ultrasound probe to examine the jugular vein; 2 occurred when pulling out the catheter sheath. Conclusion: The related factors of vasovagal syncope during ultrasound-guided PICC catheterization include overcompression and avulsion on blood vessels and mental stress. Therefore, it is recommended to implement psychological interventions before surgery, reduce compression and avulsion of the vascular access during catheterization, and improve practitioners’ abilities in vessel selection (vessels with a catheter/vessel ratio less than 45%) and catheterization skills to eliminate the incidence of vasovagal syncope during PICC catheterization.
目的:探讨超声引导下改良Seldinger技术(MST) PICC置管过程中血管迷走神经性晕厥(VVS)的原因及护理措施。方法:收集广东省3家医院2018年9月至2019年9月超声引导PICC置管术中诊断为血管迷走神经性晕厥的14例患者的临床资料(病历、人口学资料、诊断、局部血管压迫情况、b超记录导管/血管比例、晕厥发生时间、症状及治疗)进行回顾性分析。结果:2018年9月至2019年10月,3家医院静脉导管门诊超声引导下MST PICC置管3091例,术中血管迷走神经性晕厥14例(0.45%),均为肿瘤患者(男5例,女9例),平均年龄45±3.8岁。他们都是第一次接受人保,在手术过程中精神压力很大。14例患者(轻度2例、中度7例、重度5例)中,8例患者置管侧颈部及锁骨区多发淋巴结肿大,造成不同程度的静脉压迫,置管困难;5例导管/血管比大于45%。2例VVS发生在患者被要求将他们的头靠肩转向穿刺侧;b超探头检查颈静脉时出现10例;2例发生于拔出导管套时。结论:超声引导PICC置管过程中血管迷走神经性晕厥的发生与血管的过度压迫、撕脱和精神压力有关。因此,建议术前实施心理干预,减少置管过程中血管通路的压迫和撕脱,提高从业人员的血管选择能力(导管/血管比小于45%)和置管技能,以消除PICC置管过程中血管迷走神经性晕厥的发生。

The Application of Online Consultant Mode in Postpartum Maternity and Infant at Home in COVID-19 Pandemic 新型冠状病毒肺炎大流行期间母婴在线咨询模式的应用

Objective: To explore the effects of providing guidance postpartum maternity and infant at home by online consultation mode under the situation of COVID-19 pandemic. Methods: The International Board Certified Lactation Consultants (IBCLC) developed the procedure of online consultation for the maternity and infant during the COVID-19 pandemic, used WeChat mini program to provide health education and guidance for postpartum maternity and their families, and monitored their conditions through the follow-up visits. Results: During the pandemic period of COVID-19 from January 20th to April 30th, 2020, Transitional Care Department of the authors' hospital used the WeChat mini program to provide health education and guidance for a total of 105 cases of patients, of which 8 cases were advised to consult relevant specialties online and 8 cases were recommended to visit hospital. The remaining 89 cases of patients had their medical problems solved effectively through a total of 177 times of online counseling. The satisfaction of maternity and their families on the online consulting service reached 98.63%. Conclusion: Under the situation of the COVID-19 pandemic, the application of the online consultant mode meets the medical needs of the maternity and infant at home; avoids the risk of cross-infection caused by face-to-face communication; and ensure the health of the patients and their families.

A Study on the Knowledge of SBAR of Clinical Nurses in Hospital Blood Glucose Management 医院血糖管理中临床护士SBAR知识的研究

Objective: To investigate the clinical nurses' knowledge of SBAR's (Situation, Background, Assessment, Recommendation) blood glucose management in a large hospital of Guangzhou and to provide a basis for the training and promotion of SBAR's blood glucose management knowledge in the hospital. Methods: 100 clinical nurses from a large hospital of Guangzhou were selected as the research objects. Data were collected by means of questionnaire survey to investigate and analyze the clinical nurses' knowledge of SBAR's management of blood glucose. Results: The awareness rate of SBAR in clinical nurses was 42.55%. The awareness rate of SBAR in the management of blood glucose in hospital was 23.40%, and the clinical significance of SBAR in the management of high and low blood glucose was 39.36%. With the improvement of educational background, professional title and working years, the scores of nurses on SBAR related knowledge gradually increased, and the higher education background had a significant impact on SBAR related knowledge (P < 0.05). The scores of nurses who received SBAR training were higher than those who did not received (P < 0.01). The knowledge most desired by clinical nurses in the management of in-hospital blood glucose was analyzed by SBAR (61.43%), and the clinical significance of SBAR in the management of non-endocrine blood glucose (50%). Conclusion: clinical nurses' knowledge of SBAR management of blood glucose in the hospital is relatively inadequate, and unified norms and standards should be formulated in terms of SBAR model analysis of high and low blood glucose and timely treatment.

Application Strategy of Staged Nursing Intervention After the Surgery of Huge Rotator Cuff Tears Under the Arthroscopy 关节镜下肩袖巨大撕裂术后分期护理干预的应用策略

Objective: To explore the application of staged nursing intervention after the surgery of huge rotator cuff tear under the arthroscopy, and to evaluate the impact on patients' postoperative rehabilitation. Methods: A total of 25 patients who underwent arthroscopic surgery for large rotator cuff tears in our hospital from January 2018 to December 2019 were selected. The intervention group used a staged nursing program to intervene, and Selected 25 patients who only received routine nursing intervention during the same period as the control group. The constant-murley shoulder function score, UCLA score, SF-36 quality of life score, and nursing satisfaction score were compared between the two groups after 6 months. Results: There was no statistically significant in the difference in the scores of pain, functional activity, shoulder joint mobility, and muscle strength of the constant-murley shoulder function score between the observation group and the control group before surgery (P > 0.05). The pain, functional activities, shoulder joint mobility and muscle strength scores of the observation group were significantly higher than those of the control group 6 months after surgery (P < 0.05), and the UCLA score and SF-36 score of the observation group 6 months after surgery were significantly higher than the control group (P < 0.05), and the nursing satisfaction score of the observation group was significantly higher than that of the control group (P < 0.05). Conclusion: Staged nursing intervention can significantly improve the Shoulder joint functional rehabilitation process and quality of life of patients who post operation of huge rotator cuff tears, and improve nursing satisfaction.
目的:探讨关节镜下巨大肩袖撕裂术后分期护理干预的应用,并评价其对患者术后康复的影响。方法:选择2018年1月至2019年12月在我院行关节镜手术治疗大肌腱套撕裂的患者25例。干预组采用分阶段护理方案进行干预,选取同期仅接受常规护理干预的患者25例作为对照组。比较两组患者6个月后的constant-murley肩功能评分、UCLA评分、SF-36生活质量评分和护理满意度评分。结果:观察组患者术前疼痛评分、功能活动度评分、肩关节活动度评分、肌肉力量评分与对照组比较,差异均无统计学意义(P > 0.05)。观察组患者术后6个月疼痛、功能活动、肩关节活动度、肌力评分均显著高于对照组(P < 0.05),且观察组患者术后6个月UCLA评分、SF-36评分均显著高于对照组(P < 0.05),观察组患者护理满意度评分显著高于对照组(P < 0.05)。结论:分阶段护理干预可显著改善巨大肩袖撕裂术后患者肩关节功能康复过程和生活质量,提高护理满意度。
