Cardiac troponin I – A prognostic marker in acute exacerbation of chronic obstructive pulmonary disease 心肌肌钙蛋白I -慢性阻塞性肺疾病急性加重的预后标志物

Aim: The aim of this study is to assess the use of cardiac troponin I as a prognostic marker in the acute exacerbation of COPD. Objectives: Primary Objective: Evaluate the prevalence of elevation of cTnI in patients admitted with acute exacerbation of COPD. Secondary Objective: Evaluate the association of longer duration of COPD, lower SpO2 and higher PaCO2 at admission, increased need for ventilatory support, increased duration of hospital stay and increased inhospital mortality in cTnI positive patients. Material And Methods: Study Site: Apollo Main Hospital, Chennai Study Population: Patients greater than 40 years with spirometry confirmed COPD according to GOLD guidelines admitted with AECOPD in the emergency or ward in the specified period. Study Design: Prospective, Observational Study Sample Size With Justification: 62 cases. Study Duration: From JANUARY 2018 to NOVEMBER 2019. Inclusion Criteria: Study to be conducted in 62 patients with Spirometry confirmed COPD according to GOLD guidelines, age greater than 40 years and admitted with AECOPD in the emergency or ward in the specified period. Exclusion Criteria: Patients with the below mentioned are all excluded from the study - Acute coronary syndrome, post CPR. post cardiac surgery, persistent hemodynamic instability, acute aortic dissection, pulmonary embolism, primary lung diseases other than COPD, patient already on NIV, marked renal failure, severe anaemia, sepsis ,stroke. Study Methodology: The study design is prospective, observational and non-interventional. Study to be conducted in patients with spirometry proven COPD according to GOLD guidelines admitted with AECOPD in the emergency, ward, HDU, MDCCU in the specified period. Detailed medical history along with smoking history, comorbidities and medication history noted, general and respiratory examination to be done for every patient at the time of admission.SpO2 under room air, O2 requirement, ABG values, CXR, Hemogram values, creatinine levels, ECG findings, ECHO findings at the time of admission to be noted. Blood sample to be taken to analyse serum cTnI levels along with routine investigations with informed consent from each patient at initial point of care. Cardiac Troponin I to be estimated by immunochromatographic qualitative assay, DIAQUICK Troponin I cassette. The DIAQUICK Troponin I Cassette is an immunochromatographic assay. The minimum detection level is 1.0 ng/ml with Sensitivity 95%, Specificity 97% and Accuracy 90%. Whether the patient was admitted in ward or ICU, whether the patient required any ventilatory support (NIV or invasive ventilation), duration of the stay in the hospital was all followed up. The end point of the follow up is discharge or death. Results: Out of the 62 patients, 7(11.3%) patients were tested to be Troponin I positive during exacerbation. These patients had longer duration of COPD, increased presence of Pulmonary hypertension, Lower Spo2 on room air and higher PaCo2 at presentation, increa

Acute respiratory distress syndrome following near drowning: A case report and review of literature 溺水后急性呼吸窘迫综合征:一例报告及文献复习

According to the World Health Organization, drowning is the third leading cause of unintentional injury death worldwide, accounting for 7% of all injury-related deaths. National Crime Records Bureau estimates an average of 83 deaths every day in India due to drowning. A 36-year-old male, who is a known case of posttraumatic seizure disorder, was admitted with an alleged history of drowning into the river. On admission, the patient had severe hypoxemia and chest X-ray revealed bilateral diffuse heterogeneous fluffy infiltrates suggestive of acute respiratory distress syndrome (ARDS). The patient was immediately intubated and mechanically ventilated. Subsequent computed tomography imaging of the patient showed mixed areas of consolidation and ground-glass opacities. Later in the course of stay in the hospital, the condition of the patient gradually improved and he was discharged from the hospital in a stable condition. The outcome of ARDS in drowning victims is better when compared to the ARDS of other etiologies.

The importance of knowledge about pulmonary aspergillosis has increased immensely in the era of COVID-19 as a significant number of post-COVID patients develop lung infection with aspergillosis, an entity called COVID-associated pulmonary aspergillosis producing increasing morbidity and mortality. In this case report, we describe an immunocompetent elderly man presenting with a peripheral lung mass, suspected to have lung malignancy which turned out later to be pleuropulmonary aspergillosis.

Utility of chest X-ray in health checkup program in detecting pleuroparenchymal lesion in a tertiary care hospital 胸部x线检查在三级医院胸膜实质病变诊断中的应用

Introduction: Asymptomatic individuals may have unnoticed pulmonary lesions (PL) in the form of infective, noninfective, and neoplastic etiology. Earlier studies detected pulmonary pathology in chest radiographs of health checkup patients and this helped in characterizing the PL and changing the course of the disease pattern and prognosis. Aims and Objectives: The aim is to detect the incidence of PL in asymptomatic individuals presenting to preventive health checkup. The objective is to study each chest PA view chest radiographs for any PL. Materials and Methods: Chest PA radiographs of 51,612 individuals are taken as per international standard protocol for health check individuals after prior informed consent. The radiographs are evaluated for PL by senior radiologists. Discussion and Results: The high prevalence of tuberculosis, chronic lung diseases, and occupational PL was noted in the Indian population by various studies. In the present study for PL, a total of 51612 asymptomatic individuals had undergone routine medical health checkup. Out of those screened, 27,508 (53.3%) were males and 24,104 (46.7%) were females, maximum patients were in the 36–55 years age group (46.3%) and minimum being >65 years (4.5%), smokers were 6050 (11.7%), and PL: 1577 (3.1%) which includes parenchymal opacities and pleural based. In addition, cardiomegaly was noted in 1774 (3.4%) of the patients. The maximum PL was seen in 46–55 years age group (59.4%) and a similar trend was observed in cardiomegaly (34.4%) also. Conclusion: The role of routine chest radiographs in health checkup was found to be extremely useful for early detection of the PL and further patients are recommended for higher imaging investigations, biopsy, and bronchoscopy.

A metachronous malignancy: An interesting case report 异时恶性肿瘤:一个有趣的病例报告

Guillian-Barré syndrome (GBS) is an acute immune-mediated polyradiculoneuropathy that is often related to a previous infectious exposure. GBS emerged as a potentially serious complication of covid 19 infection. Here we present a case of 64 years old female who presented to us with worsening respiratory failure and progressive flaccid symmetrical sensorimotor neuropathy following covid infection. She was diagnosed to have GBS as post covid complication . Hence started on Intravenous Immunoglobulin infusion following which she had improvement in respiratory failure and prevented from intubation and mechanical ventilation.

A review article on biochemical predictors of COVID-19 infection COVID-19感染生化预测指标综述

The coronavirus disease 2019 (COVID-19) pandemic is a scientific, medical, and social challenge. The complexity of the severe acute respiratory syndrome coronavirus 2 is centered on the unpredictable clinical course of the disease that can rapidly develop causing severe and deadly complications. The identification of effective laboratory biomarkers and being able to classify patients based on their risk is imperative in guaranteeing appropriate treatment. The analysis of recently published studies highlights the role of systemic vasculitis and cytokine-mediated coagulation disorders as the principal actors of multi-organ failure in patients with severe COVID-19 complications. In this review article, we will be discussing the role of biomarkers in the management of COVID-19 infection.

Radiological imaging and its utility in lung transplantation: Our experience and review of literature 放射成像及其在肺移植中的应用:我们的经验和文献回顾

Lung transplantation (LT) can be considered a definite treatment for end-stage pulmonary disease in appropriate patients. Depending upon various clinical factors, LT can be performed either as a single LT, bilateral LT, or heart LT. Although LT is a well-established procedure now, it is not without its own complications. In post-LT patients, several complications can have overlapping clinical and imaging features; therefore, early and accurate diagnosis can be challenging. Due to this difficulty, an interdisciplinary approach is imperative. Bronchoscopy, bronchoalveolar lavage, and transbronchial biopsy may be required in this approach, in addition to the routine investigations. The treating team should be aware of the possible complications after LT and the associated imaging features that may occur at varying time points following transplantation to provide prompt management. This article describes our experience and reviews the utility of radiological imaging in LT in relation to complications that may occur following LT.
