Three Years Follow-Up of Single-Stage Correction with Modified Chula Technique for Frontoethmoidal Encephalomeningocele: The Advantage of The Teamwork Approach (A Case Report) 改良Chula技术单期矫正额筛脑膜膨出三年随访:团队合作的优势(附1例报告)

Backgrounds: The main objective of Frontoethmoidal encephalomeningocele (FE) treatment are neural morbidities defect correction and aesthetically pleasing looks. Staged procedures are used to be performed in Indonesia. This article aimed to reveal the result of FE correction through the single-stage modified Chula technique (ST-MCT) procedure in collaboration with the neurosurgery team.Case Reports: A rare case of 5 years old girl diagnosed with FE was reported in this study. The FE was slowly increased in size, causing apparent facial deformity and the appearance of telecanthus. An ST-MCT procedure in collaboration with the neurosurgery team was conducted to correct the defects. The IOD values, IPD values, postoperative complications, and anesthetic improvements were evaluated in this study.Result: The patient was well after the surgery, with no complications and short length of stay. There were noted improvement of ICD, IOD and IPD postoperative follow up. At three years after the surgery ICD= 29mm; IOD= 26mm; IPD= 52 mm (normal range).Summary: ST-MCT procedure conducted in collaboration with neurosurgery team had shown excellent correction of ICD, IOD, and IPD values, no complication, shorter length of stay, and minimal scars. It considers as the most proper technique to reach a good result of correction and aesthetically pleasing looks in FE cases.

Epidemiology and Characteristics of Plastic Surgery Cases in Simpang Lima Gumul General Hospital, Kediri Regency During Covid-19 Pandemic 新冠肺炎大流行期间Kediri县Simpang Lima Gumul总医院整形手术病例的流行病学及特点

Backgrounds: Chinese Government found the first case of coronavirus disease 2019 (COVID-19) in Wuhan, China, in December 2019. The pandemic impacts the number of visits to plastic surgery clinics in many countries such as Italy and the United States following surgical limitations. This study was aimed to determine the impact of the pandemic on the field of Reconstructive and Aesthetic Plastic Surgery in Indonesia, particularly in Kediri Regency, East Java.Methods: The study design was an analytical cross-sectional study, using data obtained from the medical records of plastic surgery patients at SLG General Hospital in Kediri Regency. The data analysis included descriptive analysis and dummy regression analysis.Results: This study included a total of 29 observations, 14 before the pandemic era and 15 during the pandemic, with a total of 460 individuals from various diagnostic categories. Based on the findings, the number of patients diagnosed with Congenital Disorders, Skin, and Soft Tissue, and Aesthetic Surgery dropped by 7%, 2%, and 6%, respectively. Meanwhile, there was an increased number of patients with Maxillofacial, Burn and Wound Care, and Hand and Microsurgery by 5%, 4%, and 1%, respectively. Conclusions: The average number of patient visits for the Skin and Soft Tissue, Burn and Wound Care, and Hand and Microsurgery categories did not vary much during this pandemic.

Nutrition Therapy in Type 2 Diabetic Burn Patient 2型糖尿病烧伤患者的营养治疗

Summary: Severe burn patients experience pronounced metabolic changes that caused hyperglycemia. Other existing metabolic conditions such as diabetes mellitus may worsen this condition. Early, adequate, and personalized nutrition therapy may result in better glycemic control and prognosis.A 44-year-old male with severe burn injury involving 27,5% total body surface area (TBSA) and type 2 diabetes mellitus (T2DM) was given early and diabetes-specific nutrition therapy to meet the recommended energy and protein needs. Lower carbohydrate contents and higher mono-unsaturated fatty acids (MUFA) were components of diabetes-specific nutrition therapy. Desirable blood glucose levels, a positive trend of albumin levels, and reduced inflammatory markers were achieved while being given this nutrition therapy. Sepsis was not diagnosed in this patient. The patient was discharged from the hospital after an improvement in clinical condition. Hyperglycemia commonly occurs in critically ill patients, especially with pre-existing T2DM. The provision of prompt and personalized nutrition therapy will improve clinical outcomes.

A Rapid Growth of Verrucous Carcinoma After Biopsy: Treatment and Reconstruction 活检后快速生长的疣状癌:治疗和重建

Summary: An increasing amount of evidence from clinical and experimental studies suggests that biopsies, thought to assist with definitive diagnoses, may aggravate tumor progression. We present a case of fast-growing giant verrucous squamous cell carcinoma, observed two weeks following biopsy. A 38-year-old man presented with a nose tumor that recurred two weeks after a biopsy and debridement to remove a lesion that was thought to be an abscess. Wide excision was performed on the tumor, frozen sectioned; the defect was reconstructed with a two-step forehead flap. The rapid tumor growth observed in this case raises the question of whether biopsies or other interventions may accelerate the growth of malignant tumors. Verrucous squamous cell carcinoma is slow-growing with a predilection for oral and nasal mucosa and rarely metastasizes. The patient presented here is an unusual case, with a tumor rapidly growing within fifteen days. A two-staged paramedian forehead flap is the workhorse of nasal reconstruction.Even though the benefit of doing biopsies overall outweighs the potential side effects, further research is required to determine whether biopsies affect the progression of the tumor. Early differential diagnosis is also crucial, as it may affect the patient's plan of treatment and prognosis. Giant tumors on the facial area may cause a functional and cosmetic disturbance. A two-step forehead flap produces a satisfactory cosmetic appearance for the patient one-year post-procedure.

Application of Buccal Fat Pad as Adjunct Flap for Lateral Defect Closure in Palatoplasty 颊脂肪垫辅助皮瓣在腭裂修复术中外侧缺损的应用

Summary: Cleft lip and palate are the most widely recognized craniofacial congenital malformation. The buccal fat pad (BFP) can be utilized to close lateral fistulas at the time of palatoplasty due to its rich blood supply and simple accessibility. The aim is to report on the use of BFP as a pedicled flap in cleft palate surgery and to discuss promising results for this reconstructive concept. The case subject was a five-year-old male born with cleft lip and palate; the cleft lip was operated on 4 months old. The patient was operated on using Bardach's two flap palatoplasty techniques, and for the closure of the lateral lining defect, the BFP was used as a pedicled flap.After the surgery, no abnormality was found, including infection, graft loss, palatal fistulas, or cheek depression. All wounds were fully epithelialized. In palatoplasty, BFP is a safe and reliable method with a fast healing benefit.

Microsurgical Reconstruction During Covid-19 Pandemic Setting: A Systematic Review and Clinical Applications 2019冠状病毒病大流行背景下显微外科重建:系统综述及临床应用

Introduction: The outbreak of the novel coronavirus disease 2019 (COVID-19) has taken almost all countries worldwide. This situation has compromised the safety and health of all medical workers, including microsurgeons. Given that there have been various articles published about the recommendations in microsurgical reconstruction during the COVID-19 pandemic, We felt compelled to compile all relevant studies about the microsurgical reconstruction in the COVID-19 pandemic setting, describe the various actions from different perspectives, as well as providing suggestions based on evidence-based medicine.Method: We did a systematic literature search using electronic databases such as PubMed, Embase, Medline, Wiley, and Cochrane using keywords reconstruction, Microsurgery, coronavirus, and COVID-19. All included studies were critically appraised and reviewed.Results: Nine relevant articles were included for the final review, four of them were qualitative studies, and 5 were guidelines. Studies look at various surgeon's experiences from different healthcare systems during the COVID-19 pandemic. The focuses of the included studies are also diverse from head and neck surgery, breast surgery, limb surgery, and general Reconstructive Microsurgery.Conclusion: Microsurgical reconstruction is not suggested to perform during the peak of the COVID-19 pandemic. It is indicated only if necessary after a rigorous review by the multi-disciplinary team. A full PPE, including a Powered Air Purifying Respirators (PAPR) system, fit-tested N95 respirator masks, face shield, and tight-fitting eye protection goggles, are recommended in performing the surgeries.
导语:2019年新型冠状病毒病(COVID-19)的爆发几乎席卷了全球所有国家。这种情况损害了包括显微外科医生在内的所有医务工作者的安全和健康。鉴于新冠肺炎大流行背景下的显微外科手术重建建议已经发表了很多文章,我们觉得有必要整理所有关于新冠肺炎大流行背景下显微外科手术重建的相关研究,从不同角度描述各种行动,并基于循证医学提出建议。方法:利用PubMed、Embase、Medline、Wiley、Cochrane等电子数据库进行系统文献检索,检索关键词为重建、显微外科、冠状病毒、COVID-19。所有纳入的研究都经过严格的评价和审查。结果:最终纳入9篇相关文献,其中4篇为质性研究,5篇为指南。研究着眼于在COVID-19大流行期间来自不同医疗保健系统的各种外科医生的经验。纳入研究的重点也从头颈外科、乳房外科、肢体外科和一般重建显微外科等多种多样。结论:2019冠状病毒病疫情高发期不建议进行显微外科重建。只有在必要时才会由多学科团队进行严格的审查。建议佩戴全套个人防护装备,包括动力式空气净化呼吸器(PAPR)、经过测试的N95口罩、面罩和贴身护目镜。

Given that there have been various articles published about the recommendations in microsurgical reconstruction during the COVID-19 pandemic, We felt compelled to compile all relevant studies about the microsurgical reconstruction in the COVID-19 pandemic setting, describe the various actions from different perspectives, as well as providing suggestions based on evidence-based medicine.\u0000Method: We did a systematic literature search using electronic databases such as PubMed, Embase, Medline, Wiley, and Cochrane using keywords reconstruction, Microsurgery, coronavirus, and COVID-19. All included studies were critically appraised and reviewed.\u0000Results: Nine relevant articles were included for the final review, four of them were qualitative studies, and 5 were guidelines. Studies look at various surgeon's experiences from different healthcare systems during the COVID-19 pandemic. The focuses of the included studies are also diverse from head and neck surgery, breast surgery, limb surgery, and general Reconstructive Microsurgery.\u0000Conclusion: Microsurgical reconstruction is not suggested to perform during the peak of the COVID-19 pandemic. It is indicated only if necessary after a rigorous review by the multi-disciplinary team. Long Term Evaluation Of Maxillary Growth After ‘The Non Denuded Palatoplasty’ Technique “非剥脱腭成形术”后上颌生长的长期评价

Background : Conventional Two Flap Palatoplasty technique will produce lateral defects without any periosteal coverage. These denuded lateral defects are prone to contamination and infection. These will result in wound contraction, scar formation and maxillary growth impairment. In 2011, we studied “The Non Denuded Palatoplasty” technique. This technique precipitated the epithelialization process of the lateral defects. Faster epithelialization is expected to decrease wound contraction and good maxillary growth. Method : This is a case control study to compare the maxillary growth of 2 groups consists of unilateral cleft lip and palate patients repaired with “The Non Denuded Palatoplasty” technique and Conventional Two Flap Palatoplasty. The outcome will be evaluated from cephalometry and the dental cast for each patient is evaluated using GOSLON YARDSTICK method. Data will be analyzed using SPSS version 20. Result : A total of 4 patients in The Non Denuded Palatoplasty group and 10 in the Conventional Two Flap Palatoplasty. The cephalometric SNA, SNB and ANB point showed Class III skeletal jaw relationship or deficient maxilla. Meanwhile the GOSLON yardstick type III are the most common GOSLON on both group with good inter-ratter reliability (p=0.839) based on Mann Whitney test. In these study, there was no correlation between cephalometric variables with GOSLON score. Conclusion: Our results showed that modification (The Non Denuded Palatoplasty) technique made no statistically significant difference to the maxillary growth. However this study has several limitations, one of which being the small sample size due to family, social and other factors that are beyond the control of the investigating team. Also the evaluation was conducted in patients aged 7-9 years, hence the result of this study is not the final outcome. Reconstruction Of Noe Fracture With Immediate Diced Costal Cartilage Grafting: A Case Series 即刻肋软骨切块移植重建鼻骨折一例

Background : Naso-orbital-ethmoid (NOE) fracture is one of the common injury and the management remains difficult and controversial due to the anatomic complexity. Severe facial deformity and dysfunction are the results from the untreated injury. Moreover, the results from many procedures are not aesthetically satisfying. Meanwhile diced cartilage graft popularity is increasing in order to correct nasal deformity whether post trauma, revision (secondary) rhinoplasty or cleft nose. The procedure is less time consuming, easy to perform and also highly malleable. Methods : This study presents our experience in managing four patients with NOE fracture using costal cartilage graft technique immediately after injury. A columellar V-shaped incision was made, the finely diced cartilage harvested from the eight rib was injected to the glabella, dorsum and nasal tip using one cc syringe needle with cut tip. Demographic data were obtained from the patients’ medical record. Post-operative results were observed.  Result: The four surgeries were done, with average 5.75 (range, 2-9) days after injury. During the observation, nasal tip misalignment was observed in only one patient. Diced cartilage were not visible through the skin, although it could be palpable. No cartilage extrusion were occurred. After averange follow up 10.25 mo (range 8-12 mo) All patients were satisfied with their facial appearance. Conclusion: The fracture of NOE can be manage with immediate diced costal cartilage graft, since this treatment can achieve a normal facial function and appearance. The complications are low and manageable, making it as serious contender technique of choice in managing NOE fracture to other techniques. Association Between Serum Albumin And The Success Of Fluid Resuscitation In Children Hospitalizen In Cipto Mangunkusumo Hospital Burn Center 血清白蛋白与Cipto Mangunkusumo医院烧伤中心住院儿童液体复苏成功的关系

Background : Burn injury has a great impact on mortality and morbidity in children. Significant loss of albumin (hypoalbuminemia) in burn patient often leads to serious complications. However, it is still unclear whether serum albumin has a role in the success of fluid resuscitation in children with burn injury. Method : This is a retrospective cohort study based on medical record of children hospitalized with burn injury at Cipto Mangunkusumo Hospital Burn Centre from January 2012-March 2018. The subjects collected with the total sampling method. Result : Most burn injury happen because of scalds, and have grade 2 burn injury with PELOD score<10. Almost all subjects was succesfully resuscitated in the first 24 hour (95,1%). No association was found between the success of fluid resuscitation with either serum albumin [RR 1,175(95%CI 0,3-4,4) p=0,812], or with ureum, creatinin, lactate level, weight and the degree/extent of the burn injury. Conclusion: The success rate of fluid resuscitation in pediatric burn injury was quite high in Cipto Mangunkusumo Hospital Burn Centre. No association was found between serum albumin and the success of fluid resuscitation during the first 24 hour period. Keywords: albumin, burn, pediatric, resuscitation

Maxillary Growth Evaluation Of Patients With Unilateral Complete Cleft Lip And Palate After Two Flap Palatoplasty With Honey Oral Drops 单侧完全性唇腭裂患者双瓣腭裂术后上颌生长评价

Background : Honey given as oral drops significantly precipitate epithelialization of the lateral palatal defects post two-flap palatoplasty by 2.1 times. Honey is believed to reduce wound contraction, scar formation, and would contribute as an important factor that will result in a satisfactory maxillary growth. The aim of this study is to evaluate maxillary growth as the long-term effect of rapid epithelialization of the palates treated by honey oral drops. Method : This is a case control study consisting of 2 groups; comparing maxillary growth of the UCCLP patients that were and were not given honey as oral drops following their two-flap palatoplasty in 2011-2012. The cephalometric measurements were recorded and the dental cast are evaluated using GOSLON Yardstick method.Result : This study included a total of 20 patients. Goslon Yardstick type IV are the most frequent GOSLON on both groups (40%) with moderate inter-rater reliability between examiner 1-2 and 2-3 (kappa; 0.583 and 0.512) and substantial between examiner 1-3 (kappa 0.716). Forty-percent of SNA angle in the honey group were considered as normal, while only 20% normal SNA angle were found in the control group.Conclusion: Honey oral drops following two-flap palatoplasty resulted in satisfactory SNA angle. As the completion of maxillary growth occurs at the age of 20, the results of this study would only serve as a preliminary report. Other measures to support maxillary growth should also be taken into account. Further studies are warranted to discover innovations in surgical technique that may be a major contributing factor in maxillary growth. Keywords: Maxillary growth, Two Flap Palatoplasty, honey

The aim of this study is to evaluate maxillary growth as the long-term effect of rapid epithelialization of the palates treated by honey oral drops. \u0000Method : This is a case control study consisting of 2 groups; comparing maxillary growth of the UCCLP patients that were and were not given honey as oral drops following their two-flap palatoplasty in 2011-2012. The cephalometric measurements were recorded and the dental cast are evaluated using GOSLON Yardstick method.Result : This study included a total of 20 patients. Goslon Yardstick type IV are the most frequent GOSLON on both groups (40%) with moderate inter-rater reliability between examiner 1-2 and 2-3 (kappa; 0.583 and 0.512) and substantial between examiner 1-3 (kappa 0.716). Forty-percent of SNA angle in the honey group were considered as normal, while only 20% normal SNA angle were found in the control group.Conclusion: Honey oral drops following two-flap palatoplasty resulted in satisfactory SNA angle. As the completion of maxillary growth occurs at the age of 20, the results of this study would only serve as a preliminary report. Other measures to support maxillary growth should also be taken into account.
