Liraglutide Mortality Effect on Atrial Fibrillation Patients 利拉鲁肽对房颤患者死亡率的影响

Introduction: Liraglutide, a glucagon-like peptide 1 receptor agonist(GLP-1) utilized for management of type 2 diabetes mellitus, has beenassociated with reduced risk of cardiovascular events. However, it isalso associated with increased heart rate and reduced heart ratevariability. In this study, we investigate the effect of liraglutide inpatients with atrial fibrillation (AF). Methods: TriNetX global researchnetwork provided aggregate data for this retrospective cohort study ofAF patients on liraglutide that were matched to AF patients not onliraglutide from January 1, 2016, through November 13, 2021. Primaryoutcomes were all-cause mortality, ischemic stroke, hemorrhagic stroke,acute heart failure episode, and acute coronary syndrome episode.Results: 16,214 AF patients on liraglutide were propensity score matchedto AF patients not on liraglutide. They were matched for demographics,cardiovascular procedures, cardiovascular medications, hypertension,diabetes, heart failure, ischemic heart disease, and diabeticmedications. AF patients on liraglutide were found to have asignificantly lower risk of all-cause mortality (HR 0.67, 95% CI 0.631– 0.711, p < 0.001). There was a tendency toward lower riskof stroke, acute heart failure, and acute coronary syndrome but was notstatistically significant. Conclusion: Liraglutide is associated withlower risk of all-cause mortality in AF patients. These findings arelimited due to the retrospective nature of the study. Furtherexamination is needed of liraglutide effect on mortality in AF patients.

Risk Factors for Atrial Fibrillation following a Cardiac Surgery 心脏手术后心房颤动的危险因素

Background: Atrial fibrillation (AF) following cardiac surgery is commonand has clinical impact on morbidity. The preoperative andintraoperative risk factors are still not well defined. The objective ofthe study was to examine preoperative and intraoperative risk factorsfor AF following cardiac surgery. Methods: A retrospective analysis of adatabase of cardiac surgeries was performed during 2017-2019 at PoriyaMedical Center. Preoperative factors and intraoperative were recorded.Results: 208 patients were included in this analysis. Overall AFfollowing cardiac surgery was detected in 50 (24%) patients. Of 175patients who did not have history of AF prior to surgery, 27 (15.5%)had post-operative AF. In the 33 patients with previous AF, AF followingsurgery was detected in 23 (70%). Patients with AF following surgerywho were older (66.2±8.0 vs. 60.7± 11.4 years, p=0.002), were treatedmore with anti-arrhythmic drugs (18.9% vs 4.5, p<0.001), andhad higher rates of pre-operative AF (46% vs 6.3%, p=0.0001), priorcerebral vascular accidents (14% vs 4.4%, p=0.019), and prior valvereplacement (10% vs 1.9%, p=0.009) compared to patients without AFfollowing surgery. In multivariate Cox regression analysis, age (HR1.04, CI 1.01-1.07, P=0.006) and history of preoperative AF (HR 6.01, CI3.42-10.57, P<0.001) were predictors of AF following cardiacsurgery. The probability of being free of postsurgical AF was 80% amongpatients without history of AF compared to 30% in patients withprevious AF history (p<0.001). Conclusion: Preoperative AF andage were predictors of AF following cardiac surgery
