Children with medical complexity (CMC) require specialized healthcare services, including habilitative and rehabilitative therapy services. Occupational, physical, and speech therapy aim to maximize function, reduce caregiver burden, and enhance the quality of life of both CMC and their families. Providing therapy services in the home reduces transportation barriers, mitigates risks posed by attending outpatient clinics, and may improve family stability. Barriers to accessing home care therapy services have not previously been described. Understanding barriers to home care therapy services are crucial for the development of strategies to improve access. Therefore, this study aimed to explore the prevalence and reasons for missed visits among CMC receiving home care therapy services.

Home care professionals have an ethical obligation to promote justice for their patients. Promotion of justice in the home care setting requires thoughtful consideration as patients are situated within a complex context where their health and quality of life are influenced by a myriad of factors. Like most people, individuals receiving home care are living their daily lives and are generally focused on priorities other than, though perhaps dependent on, their personal health. They should be able to live their best lives, and home care interventions and goals need to account for this. A resource-centered approach to justice fails to fully account for individuals' unique needs. Instead, I argue for use of a capabilities approach to justice. This approach offers a framework for developing and delivering care that promotes justice, accounts for disability considerations, and promotes equity. In the home care setting, a capabilities approach to justice can ensure person-centered care and advance justice.

Every 2 minutes, a mother who just gave birth dies. This paper reviews the most life-threatening postpartum complications: hemorrhage, depression, hypertensive crisis, clotting disorders, and sepsis. Home care nurses play a critical role in preventing such complications. Home visits offer the opportunity to recognize early warning signs, provide essential education, and promote urgent medical intervention. Strategies are provided for home health nurses to assess and educate mothers on effectively reducing these risks. Education on symptom recognition and when to seek help is vital as inadequate postpartum teaching is common. Hence, home care nurses can improve maternal health outcomes, reduce complications, and provide a safety net for new mothers. This paper emphasizes the importance of access to home care and using it as a powerful tool to combat preventable maternal mortality.
