This case report outlines the management of a 16-year-old male with a rare combination of injuries : an open distal femur physeal fracture, ipsilateral open diaphyseal tibia fracture, and vascular compromise due to bone displacement following a crush injury by a car. Immediate interventions included pain management, wound care, and emergency reduction of fractures. Vascular exploration, along with orthopedic interventions, were crucial for the patient's recovery. Successful surgical interventions led to gradual improvement, with restored distal pulses and neurovascular function. The patient was discharged for rehabilitation, highlighting the importance of prompt multidisciplinary care in achieving favorable outcomes for complex trauma. This case contributes to the understanding of orthopedic trauma management, presenting the first documented instance of such a combination of injuries.
