Adaptation of a health game for adults over 50 and examination of its acceptability, feasibility, and preliminary impact when implemented with a nutrition education intervention 针对50岁以上成年人的健康游戏改编,并检查其可接受性、可行性,以及在营养教育干预下实施的初步影响

Background: Diabetes impacts greater than 37 million people in the United States. Adults over 45 years, racial-ethnic minorities, and those who are overweight or obese are at increased risk for type 2 diabetes mellitus (T2DM). Digital health games offer one innovative opportunity to engage, educate, and motivate adults at risk for T2DM about healthy lifestyle changes. Objective : Describe the adaptation of an educational exergame for adults over 50 and examine feasibility, acceptability, and implementation within a health education intervention in African American adults at risk for T2DM. Method : Mixed methods were used: exploratory adaptation phase involving expert inter-views and constituent focus groups; a randomized two-group (nutrition workshop alone vs workshop plus game) pilot exploring adherence and preliminary outcomes (i.e., eating habits, physical activity level, body mass index); and a post-gameplay explanatory component examining acceptability and feasibility. Results : Exploratory phase findings identified areas for game adaptation, such as modifying graphics and adjusting movement to user preferences and abilities. The pilot sample included 24 African American women with T2DM risk factors (Mean age = 63.1). Preliminary outcomes suggest pre-post intervention changes in the direction of improvement for both groups; however, there were no statistically significant differences between groups. Intervention adherence was slightly higher for the game-plus workshop group. Accept-ability and feasibility feedback of the game was generally positive. Conclusion: Constituent and expert input helped inform game adaptation. Preliminary outcomes and acceptability/feasibility feedback of game implementation within a health education intervention showed promise in engaging adults over 50 with T2DM risk factors about topics of healthy eating and physical activity. Continued research is suggested to develop/adapt health games to promote healthy lifestyle change in diverse older adults (e.g., adults over 65; racial-ethnic minority groups;) and to conduct large-scale studies to evaluate the impact of health games in diverse groups and settings.

Adapting natural language processing and sentiment analysis methods for an intervention in older adults: Positive perceptions of health and technology. “采用自然语言处理和情绪分析方法对老年人进行干预:对健康和技术的积极看法”

Background: Older adults frequently participate in behavior change studies, yet it is not clear how to quantify a potential relationship between their perception of the intervention and its efficacy.

Research aim: We assessed the relationship between participant sentiment toward the intervention from follow-up interviews with physical activity and questionnaires for the perception of health.

Methods: Sentiment was calculated using the transcripts of exit interviews through a bag of words approach defined as the sum of positive and negative words in 28 older adults with obesity (body mass index ≥30kg/m2).

Results: Mean age was 73 years (82% female), and 54% lost ≥5% weight loss. Through linear regression we describe a significant association between positive sentiment about the intervention and weight loss; positive sentiment on technology and change in PROMIS-10 physical health and reduced physical activity time, while controlling for sex and age.

Conclusions: This analysis demonstrates that sentiment analysis and natural language processing in program review identified an association between perception and topics with clinical outcomes.

Preliminary results from a technology-integrated health and wellness intervention for seniors in an urban low-income community in Korea 韩国城市低收入社区老年人技术综合健康和保健干预的初步结果

Purpose Health inequities based on regional disparities can be addressed by regionally tailored services. However, health management strategies for the socioeconomically vulnerable elderly in the city are insufficient high levels of health inequalities based on socioeconomic status. On the other hand, the COVID-19 pandemic has increased the need for older adults to stay connected via technology-based interventions amidst periods of social distancing and closure of public centers. The purpose of this study is to describe the preliminary results of a pilot trial of the ‘Health and Wellness Program for Seniors(HWePS)' project, a local community-based, technology-enhanced, multilevel health equity intervention conducted in an urban low-income community in South Korea. Method The study area was selected based on vulnerability in various health and socioeconomic indicators. The sample includes 28 seniors who participated in the first cluster of the project, which operated as a pilot trial. Participants of the pilot trial were purposively sampled for those with relatively higher levels of education and smartphone usage rates to assess feasibility of the technology-based components. The HWePS is an evidence-based, integrated health visit service model comprised of: 1) multichannel health information delivery(instant messaging services or postal mail);2) technology enhanced care-management with tailored nutrition, exercise, emotional, and social intervention programs, 3) region-level intervention including capacity building and creating healthy living environments. Participants utilized a smartphone application linked with a web-based management system, developed by the research team, to participate in the intervention. The application was used to collect initial data on seniors' functional status and needs, to design and deliver case management through task assignment and direct seniors to appropriate local resources. Community health leaders were assigned to seniors to encourage seniors to engage in health behaviors both in person and remotely using the application. To verify the contents of the intervention program and to understand its preliminary effects, health indicators and satisfaction levels were collected through questionnaires before and after the 6-week intervention period. Results and Discussion Pre-post analysis was performed on 23 seniors who had completed the surveys. All 23 participants had utilized the smartphone application during the intervention while levels of usage varied. Nineteen (83%) received weekly health information via IMS, while remaining 17% received it through postal mail. The average age of the sample was 73 with 55% women. 63% had elementary or middle school education and 41% reported a monthly household income of less than 1 million won (≈1,000 USD).
