Eosinophilic myocarditis (EM) is a rare form of myocarditis with a very heterogeneous etiology. It is characterized by the presence of eosinophilic infiltrate in the myocardium. The course can vary from an asymptomatic clinical picture to life-threatening conditions. We describe a case of a 29-year-old patient with a history of bronchial asthma hospitalized for chestpain and manifesting heart failure. A moderate decrease of ejection fraction of the undilated left ventricle chamber was found. High levels of troponin, NTproBNP and eosinophilia were present in the laboratory findings. By endomyocardial biopsy we confirmed the presence of an eosinophilic infiltrate with a histological pattern of myocarditis. We did not confirm clonal proliferation of eosinophils. Corticoid therapy was initiated. After a week we observed normalization of eosinophils and a decrease in cardiac specific enzymes. Different types of ventricular tachycardias, which required multiple external defibrilation, occured during treatment. An ICD was implanted in the secondary prevention of sudden cardiac death. Ventricular arrhythmias have been stabilized by a combination of pharmacotherapy and individualized programming of ICD functions. Control biopsy was without histological finding of myocarditis. Left ventricular ejection fraction improved by 10%: the patient has been asymptomatic. Eosinophilic myocarditis is a rare disease but with poor prognosis in the acute phase. With the presence of eosinophilia in peripheral blood, this potential cause of acute heart failure should also be considered. Clear diagnosis must be confirmed by endomyocardial biopsy. Objective : To monitor possible changes in the incidence and severity of erectile dysfunction (ED) in hypertensive patients in relation to the targeted intervention by a nurse aimed at adopting the correct principles of lifestyle of a hypertensive patient. Methods: In a group of randomly enrolled 200 patients aged 40 to 70 years, the questionnaire gathered data on the incidence and severity of erectile dysfunction as well as on the adherence to the correct principles of lifestyle and quality of life. Data collection was repeated after a year, while in half of the patients a nurse performed a periodic targeted intervention during the year. Results: In hypertensive patients with organ complications (previous myocardial infarction, confirmed coronary artery disease, stroke), there was a higher incidence of moderate and severe erectile dysfunction compared to hypertensive patients without organ complications. In the groups without a nurse-intervention, we observed an 18% increase in the number of patients with erectile dysfunction in the group without organ complications and 37% in the group with organ complications, after one year. Conclusion: The results confirmed the assumption of a positive effect of targeted nursing interventions aimed at the adherence to correct lifestyle on the quality of life and within it the manifestations of erectile dysfunction. Transcatheter mitral valve replacement with Tendyne valve: The first two cases in Slovakia Cardiology Lett. 2021;30(1):53–56 Transcatheter mitral valve replacement (TMVR) with the Tendyne mitral valve has emerged as a novel potential therapy for patients with severe mitral valve disease who are unsuitable candidates for conventional surgery and transcatheter mitral repair with MitraClip. In this communication we present the first two cases that were performed in Slovakia and the V4 countries. Two surgically high risk female patients 78 and 79 years old respectively (EuroScore II 7.8% and 7.4% respectively) also unsuitable candidates for transcatheter mitral repair with MitraClip, underwent transapical TMVR with the Tendyne valve. Both procedures were successful: one of the patients two months after the procedure is doing well, the other patient had a complicated postoperative course and died on postoperative day 4. Moreover we present a brief description of the valve, and of the deployment technique, and an overview of the major clinical studies.
