Introduction: Mobile phone based remote monitoring systems for heart failure patients could become simple and affordable tools to improve home management. Objective: to identify utility and acceptance of ControlVit ® in heart failure patients. Method: descriptive observational pilot study in 20 outpatients with reduced ejection fraction, attending at hospital heart failure program. For six months, patients use the ControlVit ® application, which supplies patient information to improve self-ca-re and allows daily recording of specific medical variables. The data was recorded in the system log to verify frequency and data transmission. A technological acceptance questionnaire (TAM) was applied to all patients and professionals. A descriptive analysis with absolute and relative frequency distribution was also performed. Objective: To determine mortality markers in outpatients with a diagnosis of heart failure in the Ecuadorian Andean population. Methods: A cross-sectional and monocentric study was carried out in 230 mestizo, indigenous and Afro-descendant patients diagnosed with heart failure. The differences between the three ethnic groups were statistically analyzed by one-way ANOVA and Crammer’s V test. Additionally, a logistic regression analysis was performed. Results: The predominant ethnic group was mestizo (73.5%), followed by indigenous people (15.5%) and afro-descendants (10.5%). The logistic regression analysis showed that age (OR: 1.046; 95% CI: 1.014-1.078; p = 0.04), ethnicity (OR: 1.713; 95% CI: 1.053-2.78; p = 0.030), the presence of atrial fibrillation (OR: 2.711; 95% CI: 1.03-7.12; p = 0.042) and the number of hospitalizations (OR: 3.026; 95% CI: 1.85-4.94; p = 0.000), were markers of poor prognosis. On the other hand, mean arterial pressure (OR: 0.969; 95% CI: 0.94-0.99; p = 0.010), absence of ischemic cerebrovascular event (OR: 0.15; 95% CI: 0.48-0.527; p = 0.03) and total cholesterol levels (OR: 0.991; 95% CI: 0.987-0.996; p = 0.000) were protection markers. Conclusions: In this population, ethnicity, atrial fibrillation, mean arterial pressure, ischemic cerebrovascular event, total cholesterol levels and the number of hospitalizations were established as mortality markers in outpatients diagnosed with heart failure.

Several studies suggest that gene mutations from thrombophilia may constitute a significant risk factor for coronary artery disease, especially in young patients with normal coronaries and non-significant lesions. It is presented a case of a 42-year-old male patient who arrived at the emergency room due to an acute myocardial infarction. The electrocardiogram showed an ST elevation with upward convexity of 4 mm from V1 to V4 and of 1 mm at V5 and V6. Cardiac enzymes were positive (troponin I 8.4 ng/dl, normal range < 1 ng/dl). It was found septum and apical hypokinesia, with an ejection fraction of 44%. The culprit lesion of the infarction was treated with primary angioplasty, successfully dilating the left anterior descending coronary artery. However, spontaneous thrombi appeared at the middle third of the circumflex artery, an area without occlusive lesions previously. A full laboratory assessment for prothrombotic state diseases revealed positivity for gene mutations at A1298C from the methylene tetrahydropholate reductase (MTHFR) and plasminogen activator inhibitor-1 (PAI-1). Gene mutations at A1298C from the MTHFR and PAI-1 should be considered in young patients with acute coronary syndrome, mainly when thrombi are found in non-culprit arteries with no or non-significant coronary lesions.
