The Process from Decision Making Regarding Surgical Procedure to Perceiving the Results for First-time Breast Cancer Patients who Underwent Immediate Transverse Rectus Abdominis Musculocutaneous Flap Breast Reconstruction 乳腺癌患者首次行即刻腹直肌皮瓣乳房再造术从手术决策到效果感知的过程

本研究の目的は,横軸型腹直筋皮弁による一次乳房再建術を受けた初発乳がん患者の手術施行に関 する意思決定から決定した手術の結果を認識していくまでのプロセスを明らかにすることである.研 究参加者は,手術前に医師より乳房再建術の説明を受け,手術後1年以内の初発乳がん患者 12名で あった.データ収集は半構成的面接法にて行い,質的帰納的に分析した. 分析の結果,このプロセスは,まず手術の施行を決定していく局面は,【乳房全摘術施行の必要性 の認知】,【乳房全摘による社会生活の否定的変化の予測】と【乳房喪失による精神的ダメージの予 測】,【再建への期待】,【手術施行の決定への後押し】による【手術施行の自己決定】,【自家組織移植 の検討】と【シリコン挿入の検討】,【自家組織移植の選択への踏ん切り】からなった.次に自分で決 定した手術の結果を認識していく局面では2つのパターンを見出した.決定した結果に対する肯定 的な認識のパターンは,【徐々に元通りに近づきつつある身体】と【再建によって維持される社会生 活】,【再建への満足】,否定的な認識のパターンは,【手術によって変容してしまった身体】,【選択し た再建方法への後悔】からなった.

Significance of Bedside Photographs for ICU Nurses 床边照片对ICU护士的意义

This study examines what significance bedside photographs have for ICU nurses in order to obtain suggestive findings concerning using such photographs more effectively in nursing intervention. Nurses with experience caring for critically ill patients who have photographs at their bedsides were interviewed with semi-structured interviews. For analyzing their responses, the modified Grounded Theory Approach was used as a reference. From the results, five aspects of significance that such photographs have for nurses were extracted. The aspects were views about photographs affected by individual backgrounds and hospital ward culture, views shifting towards paying more attention to the experience itself of patients whose lives are being threatened, thoughts for considering and respecting complicated effects, photographs acting as a catalyst for nursing actions that respect patients as living persons, and mixed feelings caused by photographs. For ICU nurses, bedside photographs change their views on patients, helping them to achieve patient-centered care through establishing partnerships with patient families. Bedside photographs can be double-edged swords; they have both benefits and disadvantages. The results suggest that being aware that photograph significance differs depending on family crisis levels and family systems and that using photographs carefully are important.

Attitudes of Nurses toward Caring for Dying Patients in Intensive Care Unit 重症监护病房护士对临终病人护理的态度

The purpose of this study was to investigate actual conditions and factors that influenced the attitudes of nurses in intensive care unit (ICU) toward the management of terminal care. A self-report questionnaire was administered to nurses with minimum 1 year of clinical experience in ICU. The self-report questionnaire consisted of personal attributes and views on life and death using the Japanese version of the Frommelt attitudes toward care of the dying scale (FATCOD-B-J). Responses were obtained from 414 nurses and analyzed. The average score of the FATCOD-B-J of nurses in ICU was 113.3±11.1 points. The terminal care attitude of nurses in the ICU had positivism equal to that of nurses in the general ward with similar age and clinical experience. The views on life and death, which does not avoid thinking about death, finding significance in life and destination mission, often thinking about death, and experience in departments other than ICU influenced the terminal care attitude of nurses. Therefore, I speculate that qualitative factors, such as their age and years of experience, but not quantitative factors, influence the terminal care attitude of nurses in ICU.
