Occupational therapy and end-of-life care: Development of high-density foam orthosis as comfort measure 职业治疗和临终关怀:高密度泡沫矫形器作为舒适措施的发展

An orthosis is a rehabilitation device applied to a body segment (with one or more joints), whose function is to stabilize, immobilize, prevent, or correct deformities, and maximize patient functionality.However, in end-of-life care and palliative rehabilitation, changes are needed for orthosis prescription, once the main objective is comfort and symptom management. The aim of this article is to present a specific model of high-density foam orthosis developed in a Brazilian hospice for terminally ill patients, and discuss about the roles and strategies in palliative rehabilitation. This orthosis model is proposed for patients with advanced disease and no prognosis of functional recovery. It is manually made with high-density foam, cotton fabric, board velcro, EVA (ethylene vinyl acetate), hot glue, folding or electric knife. The foam is cut with the folding knife according to the required orthosis model, then covered with fabric and EVA. Board velcro is added to place the device on the patient. Positive results can be described: adherence to the use of the orthosis, minimal discomfort reported, muscular relaxation/tonus regulation, pain management and reduction of edema. Rehabilitation is an integral part of Palliative Care, and in case of terminally ill patients, comfort is the main objective of the care plan. The intervention of Occupational Therapy and the use of foam orthosis is an important strategy to achieve this goal.

Intraosseous meningioma of the orbital roof 眶顶骨内脑膜瘤

Primary intraosseous meningioma is a rare variety of meningioma that represents about 1% of all meningiomas Intraosseous or intradiploic meningioma of the pediatric orbital roof is reported in case reports in very isolation.. The following article is carried out with the objective of describing a case of primary intraosseous meningioma of orbital roof in a pediatric patient as well as reviewing the literature in this regard. It was concluded that intraosseous or intradiploic meningioma of the orbital roof is a rare entity. They are more frequent in males and the common age of presentation is at the beginning of the second decade of life. They usually only present with ipsilateral proptosis, without impaired visual acuity or pain. Most are histologically of the psammomatous variety and the treatment of choice is total surgical excision.

Vipareeta Avipareeta Swapna Nidarshaneeyam (VASN) is the 29th chapter of Sutra Sthana of Sushruta Samhita. This chapter has 81 verses that deal with various Shakunas (omens), Doota (caregiver) and Swapna (dreams). The aim of the present work is to provide a comprehensive and critical analysis of the contents of VASN chapter with the help of contemporary prognostic and dream literature. Characteristic features of a caregiver and their influence on patient’s clinical outcome, various omens (both good and bad) and dreams (both auspicious and inauspicious) and their positive or negative prognostic consequences are documented in the VASN chapter. Shakunas of the VASN chapter denote the belief systems that were prevalent in ancient India and the prognostic significance needs to be evaluated further. Interpretation of dreams that are documented in VASN chapter seems to be congruent with the findings of contemporary dream research. Interdisciplinary studies among Ayurveda, Jyotishya Shastra (medical astrology) and modern prognostic science are required to authenticate the good and bad omens and their association with the positive or negative clinical outcomes.

Effect of Reiki as a stress management technique for parents of hospitalized neonates 灵气作为一种压力管理技术对住院新生儿父母的影响

Objective: To determine the effect of Reiki, as nursing care, on parental stress in mothers with hospitalized neonates. Materials and methods: Explanatory, pre-experimental study, carried out on mothers of hospitalized neonates. Sampling was by availability and a total of thirty-two mothers over 18 years of age participated voluntarily. Results: 9.4% of the participants had an excessive level of parental stress, with the highest level of stress occurring in the parental role. The pre- and post-intervention parental stress scores had a difference of 12 points (p<0.004). Conclusions: Because the levels of parental stress following Reiki Therapy decreased, with significant statistical difference, the research hypothesis is accepted. The results of the intervention offer an alternative to be considered as a nursing intervention within the NICU in the management of parental stress.

This is an experience report on the use of the Failure Mode Analysis Tool (FMEA) to identify the possible obstacles in the control and delivery of products by the Sterile Material Central of a private institution in Rio de Janeiro. Objectives: To present the experience of using the FMEA for the improvement of the production chain of sterile materials, to present the stages of implementation of the tool and to discuss the obtained results. It was concluded that it is possible to use the tool to control work processes, achieve better results with a view to patient safety, as well as stimulate financial control of the business unit within the hospital environment.

Palliative care and locked-in syndrome: Brazilian case report 姑息治疗和闭锁综合征:巴西病例报告

The Locked-in syndrome (LIS) is characterized by tetraplegia, aphasia and preservation of the level of consciousness, and it´s associated with a poor prognosis. The palliative approach to these patients is a challenge. This article aims to examine how palliative care could improve LIS patients’ quality of life. It is a case report of a male patient, 29 years, married, photographer, with LIS after traumatic brain injury (TBI), assisted at a hospice in São Paulo. Palliative Care for situations like this is still a relatively unexplored field; in Brazil, there are no published studies. The identification LIS is a clinical challenge and misdiagnosis can occur; the association of serial clinical examination, imaging tests and multimodal neurophysiological evaluation is necessary. The care provided by the Palliative Care team provided symptom control and emotional support to the patient and family. However, the integration with the Neurology team was insipient. It was observed that the access to the health services and health technologies were complicated factors. The lack of early diagnosis and rehabilitation also interfered negatively in the process, causing psychological and social suffering to the patient and his family.

An odd study case of primary amenorrhea: Swyer syndrome 原发性闭经的一个奇怪的研究案例:斯威耶综合征

Introduction: Usual sex determination and differentiation occurs progressively during the period of embryonic development, involving chromosomal differentiation, gonadal differentiation, and somatic or genital differentiation. Swyer syndrome is a disorder of sex chromosomes and sexual differentiation characterized by 46, XY pure gonadal dysgenesis in a phenotypically female person with primary amenorrhea, and a complete absence of functioning gonadal tissue. Gonadal malignancy occurs in 10-30% of cases, so gonadectomy is indicated at the time of diagnosis. The treatment includes, in addition to the gonadectomy, hormone replacement therapy, and psychological support. Objective: To present a case diagnosed and managed in our institution, in addition to reviewing the recent literature about this rare condition, but with a strong physical, psychological and social impact. Methods: Analysis of clinical history of a patient with Swyer's Syndrome, diagnostic and therapeutic management. Bibliographic review. Presentation of the clinical case: An 18-year-old patient attends our hospital presenting primary amenorrhea. Bilateral adnexectomy was performed, obtaining as a histological result well-differentiated left dysgerminoma SALL4 +, OCT3/4 +, negative Inhibin and CD 117 +, and right streak gonad. Conclusions: In the presence of a person with primary amenorrhea and absence or poor development of secondary sex characteristics, Swyer's syndrome should be suspected, and pertinent studies should be requested to arrive at the diagnosis. It is essential to perform gonadectomy at the time of diagnosis due to the risk of malignant transformation. The importance of a multidisciplinary approach and psychological accompaniment of patients and relatives is highlighted.

Coffee prevents damaged of wound bed cells 咖啡可以防止伤口细胞受损

Objectives: The superficial wound cells adhered to the dressing during replacement and may delay wound healing. It is different from using the coffee ground as a wound dressing. The coffee has an impressive antioxidant, anti-inflammatory, and antibacterial power. Some of it stayed on the wound to protect the proliferating cells. Are there any wound cells shed when dressing change using coffee ground? Methods: We conducted a prospective observational study in Wistar rats as an experimental randomized control trial using 27 rats of 250 grams divided into three groups. The wound size was 2cmx2cm, deep 2 mm at the back skin, and closed using gauze-saline (Group-1), foam dressing (Group-2), and coffee powder (Group-3). On the 7th day, the dressing was fixed using Liqui-PREP solution to make histologic slides to counting the epithelial cells using a microscope with 200x magnification. The wound biopsied for histologic assessment. Scoring of attached cells in wound dressing: < 10/HPF= score 3, 10-20/HPF= score 2, >20/HPF = score 1. The healing of collagen tissue density 0 to 50% = score 1, 50-75% =score 2, 75-100%=score 3. The healing of epithelialization density 0 to 50% = score 1, 50-75%=score 2, 75-100%=score 3. The healing of new vascular density, none = score 1, 1-2 new vascularization = score 2, more than 2 of new vascularization = score 3. Results: The scoring of attached epithelium in Group-1: 1.55±726, Group-2: 2.55±0.527, Group-3: 3 (p=0.0001). The coffee powder group was the significant least attached cells. Wound healing rate, Group-1: 5.77±0.44; Group-2: 6.55±0.88; Group-3: 7.22±1.20 (p=0.012). Collagenization in Group-1: 1,33±0,50, Group-2: 1.78±0.67, Group-3: 2.67±0.50 (p=0.001). Epithelialization in Group-1: 1,77±0,66, Group-2: 1.88±0.60, Group-3: 2.11±0.60 (p=0.004). Vascularization, in Group-1: 2.33±0,50; Group-2: 2.10±0.60; Group-3: 2.78±0.44 (p=0.041). Conclusions: The coffee powder has minimum detached epithelial cells.

Cutaneous metastases from an upper urinary tract carcinoma: A unusual entity and a poor prognosis 皮肤转移的上尿路癌:一个不寻常的实体和预后差

We report the case of a 58-year-old male with a history of diabetes who presented with a macroscopic hematuria. On physical examination, he was afebrile. Investigations revealed an anemia. A CT scan showed a tumor of the bladder and upper excretory system with multiple lung, spleen and liver metastases. The patient received blood transfusions and a trans-urethral resection of the tumor was performed. The patient’s condition later deteriorated and skin lesions appeared on the back and the scalp. A biopsy of the lesions confirmed their metastatic nature. The patient’s condition deteriorated and he died 4 weeks later.
