Summary Background. The COVID-19 pandemic has imposed a psychological burden on a wide range of people in the community, including COVID-19 patients and their family caregivers. Objectives. This study aimed to evaluate the effect of online psychoeducational support on the caring burden in family caregivers of individuals with COVID-19. Material and methods. This study was conducted on 70 family caregivers of patients with COVID-19 referred to Imam Reza Hospital in Mashhad, Iran in 2021 by using a convenience sampling method. The participants were randomly assigned to intervention and control groups. The intervention group received one week of online psychoeducational support in six online group sessions of 35 to 45 min-utes. Data were collected using the Zarit Burden questionnaire at the beginning and one week after the intervention. The data were analyzed using descriptive and inferential statistics (Independent t-Test, Chi-squared test, Fisher's exact test, Mann–Whitney U test, Wilcoxon, and Paired t-Test). Results. At the baseline, the level of caring burden in the two groups was moderate and no significant difference was observed between the groups (p = 0.28). The total mean scores of caring burden before and after the intervention in the support group were 50.2 ± 10.5 and 46.0 ± 9.2, respectively, which significantly decreased after the intervention (p < 0.001). Consequently, the decrease in the burden mean score was significantly greater in the support group than in the control group (p = 0.04). Conclusions. It is recommended to implement online psychoeducational support for the caregivers of patients with COVID-19 to reduce their caring burden.

The family physician (FP) is vital for the continuity of care (CoC) and is also a reliable guide in choosing the preferred treatment pathways, which lead to a reduction in the utilization of unnecessary services. Objectives. To investigate the utilization of outpatient services in the Fars province as a pilot center for the urban family physician program (UFPP) and East Azerbaijan. East Azerbaijan is a province which has been selected for comparison (control) through clustering. Material and methods. This population-based, comparative and cross-sectional study was conducted in Iran. Retrospective data on specialists/subspecialists visits (SSV), prescribed medicine (PM), laboratory services (LS) and medical imaging services (MIS) was obtained from administrative claims data of the Iranian Health Insurance Organization (IHIO) for a random sample of 4,800 people between September 2017 and September 2018. Results. Overall, 50.2% of the participants were female. In the Fars province, the mean SSV, PM, LS and MIS utilization during the study period was 1.21, 3.05, 0.49, and 0.29, respectively. These amounts were 1.14, 1.71, 0.36 and 0.21, respectively, in East Azerbaijan. Multivariate analysis showed significant differences in the utilization of SSV, PM, LS and MIS between provinces ( p < 0.05). The largest difference was observed in the utilization of PM by a mean difference of 1.33 (1.17–1.50, p < 0.001). Females were associated with more PM utilization ( p = 0.003), and older people utilized more outpatient services ( p < 0.05). Conclusions. Unexpectedly, after five years of the implementation of UFPP, utilization of services was higher in Fras than East Azerbaijan. Improved access by removing barriers, especially for women and the poor, as well as program execution problems could be the reason for these results. Further research is urgently needed to solve this mystery.

Anxiety is more common during pregnancy than in any other periods. Thus, medical problems such as diabe tes can increase the anxiety of pregnant women. Objectives. The main objective of this study was to investigate the effectiveness of mindfulness-based training on anxiety in pregnant women with gestational diabetes. Materials and methods. This study was a randomized controlled trial study with a pre-test, post-test and control group. Among 24–28 week pregnant women with diabetes who referred to health centers in Kerman, 88 women were selected. Participants were randomly allocated into intervention and control groups. Mindfulness-based training was used for the intervention group over 8 weeks, and situ ational and trait anxiety was measured through a Spielberg anxiety questionnaire. The collected data was analyzed using SPSS version 22 software. Results. There was no significant difference between the mean scores of situational and trait anxiety before intervention in both the intervention and control groups ( p = 0.159, p = 0.21). However, mean situational and trait anxiety decreased in the intervention group and increased in the control group after intervention/treatment. There was significant increase over time. The difference between the two groups was also significant during the intervention and post-intervention periods ( p -value < 0.0001). Conclusions. Mindfulness-based counseling conducted by a midwife decreased the anxiety of pregnant women with gestational dia -betes.

Background. Mortality rates connected with cancer are increasing nowadays due to demographic changes in the worldwide population and the increasing number of health risks leading to cancer. Primary cancer prevention is based on avoiding exposure to identified cancer risk factors. therefore, awareness of basic health hazards is necessary. Objectives. the purpose of this study was to assess the knowledge of the role of vaccinations, physical activity, second-hand smoke exposure and other factors in cancer prevention in Polish outpatients. Material and methods. this was a survey involving Polish primary care patients. the study was based on an original questionnaire designed for this purpose and was distributed among primary care patients in either hardcopy (among primary care patients in the urban settings of the city of lodz) or over the internet. Results. 612 respondents took part in the study. among the respondents, 28.4% claimed that vaccinations do not play any role in cancer prevention. Physical activity may prevent cancer according to 58.6% of the surveyed population. tobacco smoking is a well-known cancer risk factor, which was admitted to by 97.2% of the respondents. second-hand smoke exposure in the workplace was reported by 18.3% of the respondents, more often by those with secondary education than with a university degree. second-hand smoke exposure was strongly connected with the age of the respondents. Conclusions. there is little knowledge about vaccines and physical activity as cancer prevention methods. tobacco smoking is a well-known cancer risk factor. second-hand smoke exposure remains a serious problem in Poland despite a total ban on smoking in the workplace and public places.

Background. As the availability and quality of imaging techniques improve, more and more patients are identified with no history of transient ischaemic attack or stroke in whom imaging shows brain infarcts. clinical silent vascular cerebral lesions (csVcl) include asymptomatic brain infarction, white matter hyperintensities and brain atrophy. the impact of csVcl as cerebrovascular risk factors on brain injury increases the likelihood of later life dementia. cerebrovascular risk factors of stroke, such as hypertension, heart disease, diabetes, eating habits, obesity, smoking and alcoholism, are obvious in medical practise. csVcl also increases the risk of stroke. the blood’s properties are responsible for proper tissue perfusion of the brain. Objectives. Analysis of the different biochemical parameters of blood in women and men with csVcl. Material and methods. 45 patients were evaluated. csVcl was diagnosed by means of magnetic resonance imaging. All patients had blood count, blood sugar, urea, creatine serum, thyroid tests, liver tests, lipid profile, fibrinogen, igM, igG, igA and albumin/globulin ratio (A/G) performed. the control group (39 people) had no such symptoms in the central nervous system. Biochemical tests of blood were perform according to routine procedures – automatic analysis in a hospital laboratory. Results. there were observed lower values of haematocrit and igA, and higher of ers and igM were observed more in the group of women than men, as well as there being a higher value of igA and a lower albumin/globulin indicator in women with csVcl. Conclusions. the analysis of the results collected for women and men indicates the existence of different mechanisms of csVcl oc -currence.

Background. when the pain felt by the patient loses its alarming function, it becomes an element significantly reducing the quality of life. Due to the frequency of occurrence of pain in the population and its impact on various aspects of patients’ lives, this is a problem that doctors often encounter in primary care clinics. Objectives. The aim of this study was to show the relationship between the involvement of the primary care physician in the therapeutic process of pain treatment and the level of subjective and objective quality of life of patients. efforts were also made to review the situation regarding the treatment of pain with pharmacological and non-pharmacological methods used by both patients and doctors. Material and methods. The study included 191 patients and 37 doctors. separate surveys were prepared for both groups. additionally, among patients, an eQ-5D-5l questionnaire was carried out. The collected data was subjected to statistical analysis using the sPss 25 program. Results. The involvement of a doctor in the treatment of pain moderately correlates with a higher level of quality of life, both subjective and objective. There was no correlation between satisfaction with the physician’s involvement in the therapeutic process and the level of experienced pain. Conclusions. although the primary care doctor has a limited ability to control the therapeutic process of his patients’ pain, it is important to be involved in the treatment process, because it is associated with an increased quality of life, patient’s satisfaction with the primary care doctor and a reduced level of pain.

Background. Family physicians cooperate in the treatment process, not only with patients, but also with other medical professionals. there is no doubt that their psychological competency, such as communication skills and emotional intelligence, are just as important as their clinical experience. Objectives. the aim of the present study was to identify the difficulties that family physicians encountered in working with patients, as well as with their work-related emotions. Material and methods. a total of 36 physicians in the final year of specialisation in family medicine were engaged in the research. Results. Family physicians reveal that dealing with aggressive patients, setting boundaries with patients and motivating them to treatment were the hardest for them in their work. there are specific psychological difficulties experienced by family physicians (e.g. helplessness of self-limitation, disharmony between experienced feelings and beliefs about the duty of a doctor, contradictory attitudes towards patient autonomy). the way to prepare for the professional role of a physician leads to the reinforcement of strict injunctions and prohibitions on the performance. consequently, this leads to the development of ineffective strategies to build relationships with the patients and also deal with physicians’ own emotions. Conclusions. the present study may be helpful for the development of a set of psychological tools useful for solving problems raised by the family physicians in the intraand interpersonal area and during specialisation training.
