Kritik hastalarda genellikle sepsis sirasinda ortaya cikan bu sendrom “sepsis iliskili ensefalopati” olarak da adlandirilir ve bu durum, mortalite, morbidite artisi ve uzun donem kognitif bozukluk gelismesi ile iliskili oldugu icin onemlidir. Kritik hastalarda beyin fonksiyonlarinin monitorizasyonu, ayrintili norolojik muayene ile baslamalidir ve degerlendirme sirasinda norolojik yanitlari degistirebilecek sedatif ilaclarin etkileri dikkate alinmalidir. Beyin goruntuleme yontemleri ve elektrofizyolojik incelemeler ise norolojik muayeneyi tamamlayan tani yontemleridir. Bilgisayarli tomografi, yapisal intraserebral lezyonlarin tanisini saglarken, manyetik rezonans goruntuleme, sepsis iliskili ensefalopatinin primer patolojik mekanizmalarina ve beyinde gelisen yapisal degisikliklere iliskin onemli bilgiler saglar. Elektroensefalografi kullanimi ile akut beyin disfonksiyonunun tanisi ve prognozu hakkinda kanitlar elde edilebilir. Typically observed during sepsis in critical patients, this syndrome is also named as “sepsis-associated encephalopathy” and this situation is of significance since it is related to mortality, increase of morbidity and longterm cognitive impairment. Monitorization of brain functions in critically ill patients should be commenced with detailed neurological examination and effects of sedative drugs, which can alter neurological responses during evaluation, should be taken into consideration. On the other hand, brain imaging methods and electrophysiological examinations are diagnostic procedures which complement neurological examination. While computed tomography enables diagnosis of structural intracerebral lesions, magnetic resonance imaging provides important information on primary pathological mechanisms of sepsisassociated encephalopathy and structural alterations developing in the brain. Evidence of diagnosis and prognosis of acute brain dysfunction can be acquired through use of electroencephalography for. Although it was believed that neurological biomarkers can be useful in determination of diagnosis and prognosis, further studies are needed in this subject.

E-pos ta: drsenaycanikli@yahoo.com Tel.: +90 533 543 54 22 ÖZ Amaç: Çalışmamızda çekal ligasyon ve perforasyon yöntemi kullanılarak akut akciğer hasarı gelişimini sağladığımız sıçanlarda Silimarin’in anti-inflamatuvar ve antioksidan özelliklerini değerlendirmeyi amaçladık. Gereç ve Yöntem: Toplam 46 sıçanın kullanıldığı çalışmamızda sham (n=14), kontrol (n=16) ve silimarin (n=16) grupları ve her bir grubun erken ve geç alt grupları vardı. Silimarin grubundaki sıçanlara silimarin, kontrol ve sham grubuna ise salin verildi. Kontrol ve silimarin grubundaki sıçanlarda çekal ligasyon ve perforasyon yöntemi ile sepsise bağlı akciğer hasarı gelişimi sağlandı. Erken gruptaki sıçanlar 12. saatte sonlandırılırken geç gruptakiler takibe alındı. Sıçanların serum ve bronkoalveolar lavaj sıvısı (BAL) tümör nekroz faktör alfa (TNF-α), interlökin (İL)-1, İL-6 seviyeleri, akciğer dokusunda malondialdehit (MDA) ve glutatyon sülfidril (GSH) seviyeleri, akciğer dokusunun histopatolojik incelemesi ve yaş/kuru ağırlık oranına bakıldı. Bulgular: Çalışmanın sonucunda 72. saatte silimarin grubunda sağkalım oranı %62,5, kontrol grubunda %12,5 bulundu (p=0,0506). Silimarin grubunda kontrol grubuna göre yaş/ kuru ağırlık oranı (p=0,019), serum ve BAL, TNF-α, İL-1, İL-6 seviyeleri (her biri için p<0,05) ve histopatolojik skorlama anlamlı olarak daha düşük bulundu (p=0,001). Doku MDA seviyesinin kontrol grubundan daha düşük (p=0,011) ve GSH seviyesinin daha yüksek (p=0,001) olduğu tespit edildi. Sonuç: Silimarin sepsise bağlı akut akciğer hasarı gelişimini önlemede yararlı olmuştur. Bu etkinin anti-inflamatuvar ve antioksidan özellikleri sayesinde olduğunu düşünmekteyiz.
