Background: COVID-19 made it mandatory for Namibian education institutions to transition from traditional face-to-face classroom learning to online learning. Minimal time was available to prepare nursing students to adopt this model of learning, which subsequently influenced their learning experiences.

Aim: The aim of the study was to explore and describe nursing students' experiences regarding their preparedness to migrate to online learning during the COVID-19 lockdown at a public university in Namibia.

Setting: Semi-structured interviews were conducted in English at the public university in Kavango East, Namibia.

Methods: A qualitative approach utilising an exploratory and descriptive design was used. Convenience sampling and a semi-structured interview guide was used to assess the experiences of undergraduate nursing students. Data saturation was achieved after 15 interviews. ATLAS.ti 8 software assisted with management of data that was analysed inductively following the six steps of thematic analysis.

Results: The following themes emerged from analysis of the data: (1) students' readiness to migrate to online learning; (2) challenges faced by nursing students during the migration to online learning; and (3) strategies to support the transition from face-to-face to online learning.

Conclusion: The study's findings show that the student nurses were unprepared for online learning due to lack of skills and the ability to use technology to navigate online learning platforms. Access to online learning was also hampered by poor Internet connectivity and unreliable electronic devices.

Contribution: These findings may be used to develop targeted interventions and strategies to mitigate challenges faced during transition from face-to-face to online learning.

Patients' tendency to recommend plastic surgery clinic shaped by appearance consciousness.
IF 0.9 Q4 HEALTH CARE SCIENCES & SERVICES Pub Date : 2023-11-16 eCollection Date: 2023-01-01 DOI: 10.4102/hsag.v28i0.2320

Background: Plastic surgery services need adjustment from a patient experience perspective. However, its association with outcome quality was rarely studied. Postoperative appearance can play the role in predicting the clinic recommendations likelihood.

Aim: This study is aimed to analyse recommendation likelihood (LRC) to the clinic, incorporating postoperative patient consciousness of appearance (PCA) as a mediator.

Setting: Postoperative patient from two plastic surgery clinics in Jakarta, the Capital of Indonesia.

Methods: Quantitative study with postoperative patient self-reported survey and cross-sectional data from 97 respondents were taken purposively. Respondent data were analysed through partial least squares structural equation modelling (PLS-SEM).

Results: Five elements of patient experience had positive association with LRC (p < 0.05) mediated by PCA, while PCA demonstrated a large effect on LRC (β: 0.403; p = 0.004; confidence interval [CI] = 0.173-0.671). Thus, PCA can mediate the relationship of patient experience to encourage clinic recommendations. Furthermore, healthcare by plastic surgeons showed predominant relationships followed by staff service and accessibility, suggesting those should be of utmost concern to plastic surgery patients.

Conclusion: Patient experience elements have positive association with LRC mediated by PCA. Therefore, the importance of PCA as a quality outcome must be considered in quality care delivery at plastic surgery clinics. Clinic management should prioritise optimising factors contributing to PCA.

Contribution: This study showed evidence that postoperative consciousness of appearance as outcome quality occurred as a mediator in patient experience relationship towards behavioural intention.

Background Energy-dense, nutrient-poor snacks are associated with an increased risk of non-communicable diseases (NCDs) and the double burden of malnutrition, especially among poor communities. Aim To determine and compare the snacking preferences and consumption frequency of children and adults from a low SES community in South Africa. Setting A preschool, primary school, and an elderly centre in Gauteng. Methods A cross-sectional study with a convenience sample of 90 children (3–8 years) and 100 adults (≥ 18 years) to assess snack consumption frequency and preferences of children and adults by means of a snack survey. Mann-Whitney U test was used to determine the differences in preferences and snack consumption frequency of children and adults. Results The most preferred and consumed snacks included fruits, potato chips, corn chips, sweets, and cookies. Children frequently ate more potato chips (p < 0.001), corn chips (p < 0.001), cheese curls (p < 0.001), and muffins (p = 0.024) than adults. In contrast, adults frequently consumed more peanuts or nuts (p = 0.024), savoury biscuits (p = 0.048) and biltong (p < 0.001) than children. Conclusion Apart from fruits, the most preferred and frequently consumed snacks by the sample were highly processed snacks, which are low in fibre and high in added sugars, saturated fat, and sodium. Contributions Findings from this study highlight current snack trends and can guide future nutrition education interventions on healthy snacking and in developing nutritious snacks for the South African community

Background Women in South Africa are battling the scourge of intimate partner violence (IPV), and men are usually responsible. Despite this, no studies have been carried out to specifically explore and describe Christian women’s IPV first-hand experiences in the Limpopo and Gauteng provinces. Aim This study explored and described lived experiences with regard to IPV among women attending Pentecostal churches in Limpopo and Gauteng provinces. Setting The study was conducted in the Makhado Local Municipality, City of Tshwane and Johannesburg metropolitan municipalities. Methods The study utilised a qualitative research method, using interpretative phenomenological analysis. Sampling was purposeful. Semi-structured individual interviews were conducted to collect data among women attending Pentecostal churches in Limpopo and Gauteng provinces. Data saturation was achieved after 10 survivors were interviewed. Results Two themes surfaced from the analysis: the experience of various forms of IPV by survivors and the hardships survivors encountered when seeking help after IPV. Conclusion In spite of the salient role Christian belief played in survivors’ lives, the expected support from family, pastors and law enforcement officers after disclosing IPV was non-existent. The survivors were stranded, frustrated and displayed symptoms of depression and anxiety. Contribution This study revealed survivors’ need for comprehensive, multidisciplinary, and transdisciplinary collaborative support by strengthening partnerships with church leaders to protect Christian women against IPV.

Background: Successful implementation of patient safety plans in a hospital necessitates, among other things, the leadership capacity of nurse managers. Patient care delivery errors and adverse events continue to occur for a variety of reasons, including a failure to follow recommended patient safety strategies. Certain leadership styles foster interactions with healthcare staff, resulting in work environments that promote positive patient outcomes. It is unclear what nurse managers believe about the type of leadership style that drives patient safety.

Aim: The goal was to explore the nurse managers' perceptions of leadership styles as an impetus to patient safety.

Setting: The study was conducted at an academic hospital in the Tshwane District of Gauteng province.

Methods: A qualitative exploratory and descriptive contextual design was used. Semi-structured face-to-face interviews were conducted with 20 purposefully selected nurse managers. A thematic data analysis method was used to analyse data.

Results: Common leadership styles among nurse managers and challenges affecting the efficiency of nurse managers' leadership styles emerged as themes.

Conclusion: Nurse managers must have appropriate leadership styles to improve patient safety. Human and material resource shortages, as well as a lack of collaborative practice among healthcare professionals, jeopardise nurse managers' ability to maximise patient safety.

Contribution: The article provides insight into nurse managers' perceptions of leadership styles as critical to improving patient safety. Recommendations included the need for a structured ongoing leadership training programme to develop and strengthen the skills of newly appointed and existing nurse managers.

Background: Migraines are highly prevalent among the female population and have a significant burden on one's quality of life and physical functioning.

Aim: The study explored the physical impact and contributory factors of migraines on women and their experience of chiropractic treatment for migraine pain management.

Setting: The study was conducted in the eThekwini region of KwaZulu-Natal, South Africa.

Methods: The study used a qualitative descriptive design and adopted purposive sampling. The data were collected through 12 semi-structured interviews, between March and September 2021, and analysed using thematic analysis.

Results: The first theme that emerged focused on the physical effects of migraines. The second theme related to the factors that contributed to migraines. The third theme that emerged focused on chiropractic treatment for migraines.

Conclusion: The majority of the participants experienced chronic migraines and migraines without aura. The participants felt incapacitated and experienced debilitating physical effects with their migraines. The study highlighted that chiropractic treatment was favourable among the female population in improving the quality of life and reducing the severity, disability, duration of suffering and frequency of migraines. It was revealed that chiropractic was the preferred non-pharmacological approach for migraine treatment, as it proved to be a beneficial and effective treatment for migraine pain management.

Contribution: The findings contribute to a greater awareness of chiropractic as an effective evidence-based treatment approach for migraine pain management, which may be beneficial to migraineurs and healthcare practitioners.

Development of higher-order thinking skills in nursing students through online problem-based assessment.
IF 0.9 Q4 HEALTH CARE SCIENCES & SERVICES Pub Date : 2023-10-24 eCollection Date: 2023-01-01 DOI: 10.4102/hsag.v28i0.2423

Background: The development of higher-order thinking skills (HOTS) in problem-based learning (PBL) is not confined to teaching and learning but extends to authentic assessment methods, similar to real-life situations. The assessments aligned to PBL attempt to eliminate the students' tendency towards memorisation. Rather, it instils and encourages their ability to analyse, interpret, synthesise, and evaluate knowledge and its sources.

Aim: The study had two primary aims: (1) to describe undergraduate nursing students' experiences of an online problem-based assessment (PBA), and (2) to explore how online PBA assessment contributed to the development of undergraduate student nurses' HOTS.

Setting: An urban-based South African higher education institution (HEI) in KwaZulu-Natal, Durban, South Africa.

Methods: A descriptive, exploratory qualitative approach was used. The target population was 4th-year psychiatric nursing students (N = 39) studying for the degree of Bachelor of Nursing at the preselected university, utilising two focus groups (n = 5, n = 7). Data were analysed through content analysis using the clinical reasoning model as a framework.

Results: Three categories (trigger problem, re-visioning the cues, treatment direction) and seven sub-categories (trigger problem posing, early cue identification, cue interpretation and clustering, focussed cue investigation, information processing and interpretation, reprioritise hypotheses, and diverse intervention[s]) emerged.

Conclusion: Psychiatry, Nursing and Midwifery practices require a practitioner skilled in HOTS to provide quality, efficient and cost-effective patient care.

Contribution: The findings in this study can benefit nursing education, particularly learning interruptions in HEIs.

'It breaks my heart': Healthcare practitioners' caring for families with epidermolysis bullosa.
IF 0.9 Q4 HEALTH CARE SCIENCES & SERVICES Pub Date : 2023-10-24 eCollection Date: 2023-01-01 DOI: 10.4102/hsag.v28i0.2355

Background: Epidermolysis bullosa (EB) is a painful genodermatosis presenting with skin fragility and blisters. There is no cure; the prognosis is guarded and depends on the subtype of the disease. Managing these patients can be emotionally challenging for healthcare practitioners.

Aim: To determine the perceptions, impact, and needs of healthcare practitioners (HCP) caring for patients and their families with EB.

Setting: Nelson Mandela School of Medicine, Durban and Grey's Hospital, Pietermaritzburg, KwaZulu-Natal.

Methods: The study was guided by interpretative phenomenological analysis. Individual in-depth interviews were conducted with 10 healthcare practitioners. Guba's trustworthiness framework was used to ensure rigour.

Results: Six global themes were identified, each related primarily to the perceptions, impact, and needs of healthcare practitioners. The experiences and perceptions of healthcare practitioners were that caring for patients with an incurable disease such as EB could negatively impact healthcare practitioners. There were divergent views among the disciplines of HCPs regarding the extent of care in a resource-limited environment. This resulted in negative emotions, ethical concerns, and a need for continued medical education and the application of coping strategies. Healthcare practitioners observed that patients and their families were vulnerable, requiring comprehensive biopsychosocial care.

Conclusion: Healthcare practitioners should be aware of their emotional challenges, seek support where necessary, and use effective coping strategies and self-care.

Contribution: The concerns and needs of healthcare practitioners are highlighted and interventional strategies to assist healthcare practitioners are suggested which will ultimately improve patient care.
