The Effects of Cooling Therapies on Fatigue, Physical Activity, and Quality of Life in Multiple Sclerosis: A Meta-Analysis. 冷却疗法对多发性硬化症患者疲劳、体力活动和生活质量的影响:一项荟萃分析。

Purpose: This meta-analysis examined the effects of the cooling therapies on fatigue, physical activity, and quality of life (QoL) in patients with multiple sclerosis (MS).

Methods: Articles published between 2000 and 2020 were searched in six databases. The standardized mean differences were determined by the upper and lower limits of 95% confidence intervals. Publication bias was assessed by conducting the Egger test, which uses linear regression. Publication bias was examined visually using a funnel plot.

Results: Nine studies were included in this meta-analysis. The types of cooling therapies included cooling garment ( n = 4), cooling device ( n = 2), cooling room ( n = 1), precooling ( n = 1), and cold water ingestion ( n = 1). The results indicated a significant decrease in fatigue and an increase in physical activity following cooling therapy. Cooling therapies improved the QoL of patients with MS. The Egger test indicated no significant publication bias. However, the funnel plot presented a slight asymmetry among studies.

Conclusions and clinical relevance: Cooling therapies have a beneficial effect on fatigue, physical activity, and the QoL of patients with MS. Healthcare professionals can use cooling methods to manage thermosensitive symptoms in patients with MS.

Quality Measure Concepts for Inpatient Rehabilitation That Are Best Understood From the Patient's Perspective. 最好从病人的角度理解住院康复的质量测量概念。

Purpose: The aim of this study was to identify inpatient rehabilitation quality-of-care concepts that are best understood from the patient perspective.

Design: We conducted 12 focus groups with 95 former patients, caregivers, and rehabilitation clinicians and asked them to describe high-quality inpatient rehabilitation care.

Methods: We independently reviewed the focus group transcripts and then used an iterative process to identify the quality measure concepts identified by participants.

Results: Based on participants' comments, we identified 18 quality measure concepts: respect and dignity, clinician communication with patient, clinician communication with family, organizational culture, clinician engagement with patient, clinician engagement with family, rehabilitation goals, staff expertise, responsiveness, patient safety, physical environment, care coordination, discharge planning, patient and family education, peer support, symptom management (pain, anxiety, fatigue, sadness), sleep, and functioning.

Clinical relevance to the practice of rehabilitation nursing: Rehabilitation nurses should be aware of the quality-of-care issues that are important to patients and their caregivers.

Conclusion: Important patient-reported domains of quality of care include interpersonal relationships, patient and family engagement, care planning and delivery, access to support, and quality of life.

Purpose: Poststroke rehabilitation is an inevitable element of the treatment for stroke survivors. This study aimed to investigate the effect of balance training with mirror therapy in older adults with poststroke balance impairment.

Design/methods: The study adopted a two-arm randomized clinical trial and included 38 older adults with poststroke balance impairment. The intervention group received balance exercises with mirror therapy, whereas the control group received the same balance exercises without mirror therapy (a nonreflective plate was used instead). The patient outcome, the balance score, was measured using the Berg Balance Scale. Analysis of covariance was used for statistical analysis.

Results: Results showed that balance exercises combined with mirror therapy were significantly more effective than balance exercises without mirror therapy in improving balance in the stroke survivors ( p < .001).

Conclusion: Mirror therapy combined with regular balance exercises is an effective and practical method for enhancing balance in older adults suffering from balance impairment.

Clinical relevance: Balance training combined with mirror therapy may be included in the rehabilitation programs of older adults with poststroke balance impairment.

Looking Beyond Physical Disability: Cognitive Impairment in Persons With Multiple Sclerosis. 超越身体残疾:多发性硬化症患者的认知障碍。

Purpose: Cognitive impairment (CI) is an underestimated part of disability in persons with multiple sclerosis (PwMS). The purpose of this evidence-based project was to influence clinicians treating PwMS by education of the current guidelines on cognition and multiple sclerosis and to screen CI with a valid neuropsychological tool.

Methods: Staff were educated on the 2018 National Multiple Sclerosis Society guidelines about CI in PwMS. The Processing Speed Test (PST) measured CI.

Results: Of 50 eligible patients in a 3-month period, 21 (42%) PwMS were cognitively screened. Of the 21 enrolled patients, nine (42.9%) patients were cognitively impaired. Employed patients' PST scores tended to be higher than unemployed patients. As age increased, PST raw scores decreased.

Conclusions: In this project, less than half of eligible patients received cognitive screening after an extensive education program. Thus, education alone was not sufficient to change behavior. Other strategies to influence change may be more effective.

Health Literacy-Based Self-Management Intervention for Patient-Family Caregiver Dyads Undergoing Hemodialysis. 基于健康素养的自我管理干预对血液透析患者-家庭照顾者的影响。

Purpose: The aim of this study was to determine the effect of a health literacy-based self-management intervention for patient-family caregiver dyads undergoing hemodialysis.

Design: A quasi-experimental design was used.

Methods: The intervention group ( n = 20) received a health literacy-based self-management intervention for patient-family caregiver dyads, whereas the control group ( n = 23) received standard informational messages through a short messaging service for 8 weeks. All participants were assessed for hemodialysis knowledge, self-efficacy, family support, self-management, the ratio of interdialytic weight gain to dry weight, and serum phosphorus and potassium levels at pretest and posttest.

Results: Participants in the intervention group exhibited higher scores on hemodialysis knowledge, self-efficacy, family support, and self-management than those in the control group. In addition, the ratio of interdialytic weight gain to dry weight decreased significantly in the intervention group.

Conclusions: The health literacy-based self-management intervention for patient-family caregiver dyads undergoing hemodialysis was effective in improving hemodialysis knowledge, self-efficacy, family support, self-management, and the ratio of interdialytic weight gain to dry weight.

Clinical relevance: Because health literacy is an important factor influencing self-management, interventions for patients undergoing hemodialysis should consider the health literacy level of patients and their family caregivers.
