The impacts of nurses' attitudes towards dementia, critical reflection competency, and nursing work environment on person-centered nursing in acute care hospitals: A descriptive study 护士对痴呆症的态度、批判性反思能力和护理工作环境对急诊医院以人为本护理的影响:描述性研究

Purpose: The purpose of the study was to examine the attitudes of acute care nurses towards dementia, their critical reflection competency, and the level of nursing work environment and to identify the factors relating to person-centered nursing. Methods: The study was conducted for 149 nurses with at least 1 year of experience working in acute care hospitals and providing nursing care to hospitalized elderly patients. The measurements used were the Dementia Attitude Scale Korean version, the Critical Reflection Competency Scale, the Nursing Work Environment Scale, and the Person-Centered Nursing Assessment Tool. The collected data were analyzed with frequency analysis, descriptive statistics, independent t-test, ANOVA test, Pearson's correlation, and multiple regression using IBM SPSS 29.0 software. Results: The factors influencing person-centered nursing performance were critical reflection competency (β=.49, p<.001), attitudes towards dementia (β=.29, p<.001), and clinical career (β=-.26, p=.024), having the explanation power of 59.6%. Conclusion: The results of this study indicate that critical reflection competency, attitudes towards dementia, and clinical career have an impact on person-centered nursing performance. Specifically, higher levels of critical reflection competency and positive attitudes towards dementia were predictors with higher levels of person-centered nursing performance. Therefore, to enhance person-centered nursing performance among acute care nurses, it is important to prioritize critical reflection training programs for experienced nurses to enhance their critical reflection competency.

This study investigated the effects of excretion care using a smart automatic defecation treatment system on skin humidity, pressure ulcer (PU) occurrence, and severity of Incontinence-Associated Dermatitis (IAD) among patients with incontinence residing in long-term care facilities. Each patient in the experimental group was fitted with a smart automatic defecation treatment system. The system detected the release of urine/stool via a built-in sensor, followed by suction, cleaning with a water jet, and drying with warm air. Incontinence management for the control group involved the use of wet wipes for cleaning and changing diapers. The nursing interventions lasted for 2 weeks, and data were collected at baseline, week 1, and week 2. Descriptive statistics, t-tests, and the repeated-measures analysis of variance were used for data analysis. The analysis revealed statistically significant differences in skin humidity and IAD between the experimental and control groups. This study is pertinent because it demonstrated the positive effects of excretion care using a smart automatic defecation treatment system on skin humidity, PU risk, and IAD among patients with incontinence residing in long-term care facilities. Further research is necessary to investigate additional factors such as pressure and physiological traits.

Dual caregiving burden for older parents and grandchildren in middle-aged and older adults: A concept analysis 中老年人对年长父母和孙辈的双重照料负担:概念分析

Purpose: This study aimed to clearly identify and define the nature of the concept of dual caregiving burden for grandchildren and older parents of middle-aged and older adults. Methods: By employing Walker and Avant's concept analysis procedure, this study clarified dual caregiving attributes in the middle–old-aged group. Literature reviews were conducted through April 2023 using several databases. Out of an initial 55 articles, Seven met the criteria focusing on dual caregiving for the target age group. Results: Based on the literature review, the dual caregiving burden for the middle–old-aged was characterized by (1) an overconcentration of caregiving in the middle–old-aged generations, (2) absence of alternatives for caregiving outside of the middle generation, (3) ambiguity within the scope of caregiving roles, and (4) insufficient compensation for caregiving for two generations. Conclusion: This study paves the way for the development of tools to appraise dual caregiving burden and nursing interventions to mitigate it accordingly. Future research will enhance support for middle–old-aged generations vis-à-vis their caregiving roles.

Purpose: This study aimed to develop and examine the effectiveness of the stepwise handoff protocol based on the situation, background, assessment, recommendation (SBAR) application for long-term-care hospital nurses. Methods: A nonequivalent control group pre-posttest design was used for this study. There were 23 subjects in the experimental group and 23 subjects in the control group. Using SPSS Statistics 21.0, data were analyzed by descriptive statistics, independent t-test, and repeated measure ANOVA. Results: The effectiveness of the stepwise handoff protocol based on the SBAR for long-term-care hospital nurses showed a difference in the communication self-efficacy by group (F=8.81, p=.005), by time (F=11.43, p<.001), the interaction effect of group point and time (F=9.67, p<.001), the communication clarity by group (F=10.05, p=.003), by time (F=5.74, p=.005), interaction effect between group point and time (F=3.37, p=.039), and the quality of handoff by group (F=12.48, p=.001), by time (F=7.03, p=.010), and interaction effect between group point and time (F=4.29, p=.042). Conclusion: The stepwise handoff protocol based on the SBAR is expected to improve the work by systematically implementing the transfer of long-term-care hospital nursing to increase nurses’ job efficiency.

Mediating effect of social support between COVID-19 stress on the mental health of older adults using customized care services: A cross-sectional study COVID-19 压力之间的社会支持对使用定制护理服务的老年人心理健康的中介效应:横断面研究

Purpose: This study examined the mediating effects of social support on the relationship between COVID-19 stress and the mental health of older adults using customized care services. Methods: A cross-sectional study was conducted with 179 older adults receiving services at two senior welfare centers in Korea. Data were collected from April 1 to 30, 2022, using self-report questionnaires. Data were analyzed using descriptive statistics, and correlations, multiple regression using Baron and Kenny steps for mediation, with the SPSS 22.0 program. Results: The COVID-19 stress of the study participants was significantly correlated with social support (r=-.16, p<.001) and mental health (r=.30, p<.001). Social support showed partial mediating effects in the relationship between COVID-19 stress and mental health (Z=-3.43, p<.001). Conclusion: These findings indicated that mental health programs could improve community social support resources for older adults in vulnerable groups after the COVID-19 pandemic. In the nursing practice, community nurses suggest the development of programs that can be operated in the community by proposing governmental policies for the mental health management of vulnerable older adults in the blind spot.

Factors related to depression in primary caregivers of older adults with dementia in the COVID-19 pandemic era: A cross-sectional descriptive study COVID-19 大流行时期老年痴呆症患者主要照顾者抑郁的相关因素:横断面描述性研究

Purpose: The purpose of this study was to identify the factors related to depression in the primary caregivers of older adults with dementia at home during the COVID-19 pandemic era. Methods: The participants were 114 primary caregivers of older adults with dementia living in the community. Data were collected from August 2020 to October 2020 using questionnaires on activities of daily living, behavioral and psychological symptoms of dementia in older adults with dementia, and social participation, mindfulness, and depression in primary caregivers. Data were analyzed using descriptive statistics, independent t-test, one-way ANOVA, Pearson's correlation coefficient, and hierarchical multiple regression. Results: The average age of the subjects was 64.6±13.9 years, females accounted for 54.4%, and the average caregiving duration was 4.55±2.73 years. After controlling for the factors of the older adults with dementia and primary caregivers, the strongest factor of depression was mindfulness. Total explanatory power was 55% by adding 18.0% explanatory power of mindfulness. The related factors of depression were mindfulness (β=-.46, p<.001), social participation (β=-.28, p<.001), monthly income (β=-.23, p=.009), and behavioral and psychological symptoms of dementia (β=.20, p=.018). Conclusion: During the COVID-19 pandemic, the importance of mindfulness was confirmed in the depression of primary caregivers of older adults with dementia at home. Therefore, it is necessary to provide interventions to improve the changeable level of mindfulness from the early stage of dementia diagnosis to prevent depression in primary caregivers.

Results: The average age of the subjects was 64.6±13.9 years, females accounted for 54.4%, and the average caregiving duration was 4.55±2.73 years. After controlling for the factors of the older adults with dementia and primary caregivers, the strongest factor of depression was mindfulness. Total explanatory power was 55% by adding 18.0% explanatory power of mindfulness. The related factors of depression were mindfulness (β=-.46, p<.001), social participation (β=-.28, p<.001), monthly income (β=-.23, p=.009), and behavioral and psychological symptoms of dementia (β=.20, p=.018). Conclusion: During the COVID-19 pandemic, the importance of mindfulness was confirmed in the depression of primary caregivers of older adults with dementia at home. Purpose: The objective of this study is to explore nurses’ perception of their roles in shared decision-making about caring for older adults with chronic disease. Methods: This study was a qualitative descriptive study. Ten nurses participated in the focus group interviews. The collected data were analyzed using qualitative content analysis to explore nurses’ roles in shared decision-making about care for older adults with chronic disease. Results: The shared decision-making experiences about caring for older adults with chronic disease of nurses yielded four main themes and ten categories including ‘facilitating involvement in decision-making’, ‘providing information for decision-making’, ‘respecting patient values and preferences’, ‘evaluating the outcome of decision-making’. Conclusion: This study explored the roles of nurses in shared decision-making about caring of older adults with chronic disease. Nurses can enhance the shared decision-making process by identifying the decision needs and by facilitating decision-making.

Purpose: This study aims to evaluate the implementation of a Spirit program, which consists of physical and mental training for older adults who can perform independent activities as nursing home residents. Methods: In July 2022, the researchers interviewed seven older adults who had attended the Spirit program and two caregivers at a nursing home in Surakarta, Indonesia. The researchers applied the Reach, Effectiveness, Adoption, Implementation, and Maintenance framework during interviews. We measured participant engagement with the Spirit program, effectiveness to assess perceived benefits, adoption to gauge nursing homes’ willingness to adopt the Spirit program, implementation to evaluate consistent execution, and maintenance to assess future program sustainability. Results: Older adults who could carry out independent activities participated in the Spirit program. The Spirit program adds new activities that they can perform, which have various benefits, including increased happiness, stamina, and sleep quality. The participants expressed receiving instructions regarding the Spirit program from caregivers without any pressure to participate, with instructors guiding the exercises and regular schedule management. Older adults move well; however, some experience obstacles while performing balance exercises. None of the participants experienced injuries while participating in the program. The continuity of a program can be supported by the availability of tools that are easily accessible to older adults. Conclusion: The program improved the quality of life of older adults who can move independently as nursing home residents.

Factors influencing fall in the community-dwelling elderly: Data from the 2020 national survey of older people: A secondary analysis study 影响社区居住老年人跌倒的因素:来自2020年全国老年人调查的数据:一项二次分析研究

Purpose: This study was performed to identify the factors influencing fall in the community-dwelling elderly using the raw data of the 2020 national older Koreans data. Methods: The study included 9,920 community-dwelling seniors aged 65 years and older. The data were analyzed by using a complex sampling design univariate logistic regression analysis. Results: Although the subjects of this study were 9,920 people, it can be generalized to 7,617,710 people because it is a probability sample using the stratified systematic sampling with cluster plots. As a result of this study, the fall rate of community-dwelling elderly was 6.4% and the weighted percentage was 7.1%. Satisfaction with economic status, number of chronic diseases, use of medical facilities, smoking, limitation of lower muscle strength, decreased visual acuity, limitation of daily living function, and housing type were identified as factors influencing fall in older people dwelling in the community. Conclusion: Based on the variables identified in this study, it is necessary for nurses to select high-risk groups for fall and to actively develop and implement nursing intervention to prevent fall.

Development and effectiveness of an integrated medication management program centered on senior welfare centers for older adults with hypertension: A cross-sectional study 以老年福利中心为中心的高血压老年人综合药物管理项目的发展和有效性:一项横断面研究

Purpose: Purposes of this study were to develop an integrated online and offline medication management program and to examine effects of the program for older adults with hypertension residing in the community. Methods: A nonequivalent control group pretest-posttest design was used. Participants were 57 older adults with hypertension (Intervention group: 29, control group: 28). The experimental group participated in the medication management program, which included the following, verbal and video education, individual counseling and using medication note over three weeks. Collected data were analyzed using the SPSS/WIN 22.0 program. The data were analyzed by Generalized Estimation Equation. Results: Statistically significant differences were found between the experimental and control groups in terms of their knowledge of hypertension and self-efficacy for appropriate medication. Conclusion: The integrated medication management program was effective as indicated in knowledge of hypertension and self-efficacy for appropriate medication. Future studies are required to explore the medication management program’s effects on older adults with hypertension using more rigorous research methods.
