Restorative Considerations, Prosthodontic Materials, Milling and Fabrication of Full Arch Final Restorations.

New digital dental technologies have and will continue to become available to dentists. To evaluate them properly, we must understand that for technology to be a benefit, it must provide measurable value for the patient. Beyond this, it is critical to realize that most improvement in a process comes from eliminating waste from the process. In full-arch implant treatment, utilizing and coordinating the right digital technologies can eliminate waste from all phases of the treatment process. Selecting the right face-scanning, intraoral scanning, photogrammetry, 3-dimensional printing, milling, and finishing equipment and techniques can dramatically improve the patient experience.

Post-operative care involves verifying prosthesis occlusion and clear patient instructions, focusing on mitigating complications through thorough planning. Future enhancements in implant planning and guide design are expected with the integration of artificial intelligence into digital workflows.

Complete arch oral implant treatment using photogrammetry was studied in 77 patients with 111 arches focusing on digitization of clinical records and optimization of the provisional. The aim of the study was to test the hypothesis that patient satisfaction with the provisional prosthesis during the first 4 months of care determined workflow efficiency and success. Digitization of 8 clinical records was done including centric relation, vertical dimension, esthetics, occlusion, inter-arch space, abutment selection, abutment capture, and soft tissue scan. These records were then used to manufacture the provisional prostheses. All 111 prostheses seated passively indicating accurate photogrammetry registration.

The enormous emphasis on digital full arch implant workflow has spawned vast innovation, opening the doors for practitioners to treat the terminally dentate and edentulous patients with more efficiency, precision, and satisfaction. As every patient presents with different conditions, protocols must be developed to facilitate an accurate virtual patient. These protocols must provide a direct match of the digital datasets to the patient.

The authors have questioned the accuracy of intraoral scanning when restoring the full arch implant patient. Yet clinicians use intraoral scanning (IOS) for this purpose due to the many efficiencies that the IOS workflow offers. This article describes a workflow that is used to enhance the accuracy of the intraoral scan without the need to use photogrammetry. It uses a design file for the provisional prototype restoration to create both a one piece prototype prosthesis and a segmented verification prosthesis. This verified workflow has the efficiency and comfort of the IOS workflow and the accuracy of the traditional workflow.
