Experimental sleep deprivation in schizophrenic patients. 精神分裂症患者的实验性睡眠剥夺。

In this investigation the effects of 100 consecutive hours of wakefulness were studied in six chronic psychotic hospital patients. It was intended to observe, record, and measure physiological and psychological changes occurring during prolonged wakefulness and to study the possible influence of this specific stress on the course of the psychotic process. Although there are many references to experimental sleep deprivation in animals and in healthy volunteers, we could not find reports on similar experiments in psychotic subjects in the literature. Previous Observations Patrick and Gilbert1investigated the effects of sleep deprivation in 1896. Three subjects were kept awake for 90 hours. The authors observed a tendency to visual hallucinations in one subject. The study reported in this paper derives from an earlier investigation of the discrepancies in real, perceived, and ideal decision-making influence among the professional clinical personnel on five wards of a mental hospital. 2 The findings in that study clearly indicated that the manner in which staff members viewed their participation in the decisions made concerning patient care and treatment was a function of two distinct organizational features of the hospital considered. It was found that, although staff personnel tended to view their participation as that of members of professional groups rather than of private operatives, these perceptions were significantly modified by the situational context in which they worked (i.e., the ward). It was found, for example, that discrepancies between perceived and desired decision-making influence were inversely and significantly related to the officially prescribed authority hierarchy (e.g., psychiatrists indicated the least, while the occupational

Introduction Out of many problems which have held our attention in our studies of disorders in sexual identification in intersexed patients, one that has especially interested us is the question of how a person maneuvers in order to pass from one sex to the other. It is evident that this is an issue which must almost continuously preoccupy a person so driven to attempt to pass. Of the several “passing” patients whom we have been studying in detail, we wish to concentrate on one. She presents a rare combination of physical anomalies in a person who nevertheless has been making an active attempt to comply with socially structured requirements of a “normal life.” As with all our patients, she is of interest in the same way that a cultural stranger is of interest; the stranger must bring a great deal of awareness, skills, aptitudes, rehearsals,
