Background and Objective: Enhanced recovery after surgery (ERAS) has a large evidence base from colorectal surgery and is associated with reduced length of stay (LOS), lower cost, fewer complications and improved outcomes. Evidence for the benefit of ERAS across all disciplines within head and neck surgery is now emerging in the literature. This article aims to give an overview of current evidence in the literature supporting the use of ERAS based interventions in head and neck surgery.

Background and Objective: Extubation has been demonstrated to be a high-risk phase of anaesthesia, despite it frequently remains an afterthought and it is the subject of relatively little research. The aim of this review article was to look at the available current evidence, guidelines and expert opinions on extubation to help provide a summary of risk stratification of patients, optimisation strategies available and approaches to extubation to help improve safety and outcomes.

Background and Objective: Awake tracheal intubation (ATI) is considered the gold standard for managing the anticipated difficult airway. However, ATI occurs in only 0.2% of all anaesthetics and evidence shows this is declining further, eroding skill retention and putting patients at risk of harm. The Difficult Airway Society (DAS) recently published ATI guidelines but focused largely on two techniques: awake video laryngoscopy and awake flexible bronchoscopy. This narrative review aims to summarise all techniques described for ATI, discuss controversies in methods of optimizing intubating conditions as well as patient experience and clinician training.

Background and Objective: Sedation in dentistry is practiced worldwide as it alleviates patient anxiety and facilitates treatment. Japan has a dental anesthesiologist system for dentists, and intravenous sedation (IVS) is often administered by dental anesthesiologists. The first objective of this narrative review is to summarize dental IVS in Japan based on the two sedation guidelines. The second objective is to identify trends and perspectives by summarizing the latest literature on IVS in Japan over the past 5 years.

Background: Difficult Airway Society (DAS) guidelines for the management of unanticipated difficult intubation recommends the scalpel-bougie-tube (SBT) technique for emergency front of neck access (eFONA). Additionally, they recommend an 8–10 cm midline vertical incision in the obese patient. A novel cricothyroidotomy introducer, the Cric-Guide® has been developed to facilitate eFONA. We sought to compare the time required to secure eFONA using the Cric-Guide® compared to the SBT in a tissue model simulating the obese bleeding neck.
