How many times have procedures in your operating rooms been delayed because the instruments needed were still in the sterilizer? As Perioperative nurses you are likely to be quite familiar with the constant pressure to ensure that scarce instrumentation is available when needed. In 2003, a Canadian company developed a unique sterilization process employing ozone as the sterilizing agent. This technology is a safe, rapid and economical alternative to other low temperature sterilization modalities and may relieve some of the pressure experienced when instruments in short supply are in high demand. This article will discuss the principles of the sterilizer and the cycle and will explore the advantages of using this sterilization technology.

Determining the impact of a surgical liaison nurse role in the paediatric operating room. 确定外科联络护士角色在儿科手术室的影响。

A two-group (N = 92) quasi-experimental pre-post test design was used to examine the effects of intra-operative communication by a surgical liaison nurse (SLN) on parental anxiety. Group I received in person progress reports from the SLN. Group II received routine perioperative care. The Speilberger's State-Trait Anxiety Inventory (STAI) Scale and investigator developed family rating scales were distributed to both groups and used as a quantitative as well as a qualitative approach to determine what was helpful for the family members during the operative period. Two hundred and eighty feedback forms were dispersed to various health care professionals including all anaesthetists, surgeons, nurses and other staff in the perioperative care team, including the operating room, day surgery unit and the post anaesthetic recovery room areas. Feedback forms were also circulated to staff from the intensive care unit and the surgical in-patient units. The results showed the anxiety levels were lower for the families who received in person progress reports but the difference by group was not statistically significant. Thematic written responses provided examples of improved care and effective time management behaviours on the part of the health care professionals. Written responses provided validation for the scale scores for both families and health care professionals and were an indication of the support for the role of the surgical liaison nurse. Qualitative findings implied that the surgical liaison nurse facilitated the transfer of necessary information between the perioperative care team and the family thus providing a support mechanism for families under stress.

The Greenlight laser--a high powered laser used for Benign Prostate Hyperplasia--was used for the first time in Canada in Newmarket, Ontario by Dr. Liquornik BSc MDCM FRCSC in 2003. The laser is proving to eliminate hospital stay, decrease the catheter indwelling time from one week to less than 28 hours, decrease pain and suffering and make repeat procedures something rare. The laser melts away the prostate and leaves a urethra that is unscathed by the effects of the laser because the beam concentrates on the overgrown prostatic tissue only. The patients are very satisfied and back to normal routines in one to two days.

The Vancouver Island Health Authority (VIHA) in liaison with the University of Victoria (UVIC) offers an introduction to Perioperative Nursing Program to 4th Year undergraduate nursing students. The aim of this program is to help recruit Registered Nurses to the Operating Room. It has been advantageous to the recruitment and retention of nurses graduating from UVIC. Its importance is increased by the fact that a significant quantity of Victoria's perioperative nurses will be retiring in the next few years. Due to the high cost of nursing education and the financial investment that has already been committed by nursing students, the Perioperative nursing program is free to the student as the program can be included, for the successful candidate, as part of the UVIC nursing course. The intention is to encourage participation by reducing the financial burden, stress, and anxiety for the new graduate who intends to specialize. In return, the student is required to work in the VIHA for a minimum of one year, thus supporting the retention efforts of the hospital. For eligible nursing students, this program provides access to extensive perioperative nursing experience. Over the course of 3 months they are exposed to extensive theory in a classroom setting as well as clinical practice through a preceptorship program. The mentoring relationships that develop between perioperative nurses and students lead to meaningful relationships and professional growth for staff. The perioperative focus of the program improves the knowledge and skill set of nursing students. The intent is to increase nursing student's interest in pursuing a career as a perioperative nurse and to help ensure continued growth of the perioperative nursing profession in Victoria.

Occasionally perioperative nurses are involved in the complex role of caring for medical-legal evidence while also advocating for the patient's right for privacy. Appropriate management of evidence may be the deciding factor in the outcome of a criminal investigation. Understanding the importance of evidence is the first step in learning how to prevent its unintentional destruction during the surgical procedure. Our first consideration with every patient is to provide safe and efficient care. In the OR, cooperation with the police is balanced with the patient's right to privacy and confidentiality of their health care information. This is not always an easy task. Perioperative nurses have a responsibility to their patients as well as to investigators to do what is appropriate, since our actions can have direct effects on the outcomes of criminal investigations. Understanding the importance of evidence is the first step in learning how to keep evidence from being destroyed. Hospital documentation is often accepted in a court of law without challenge.

Unlabelled: Surgical face masks were originally developed to contain and filter droplets of microorganisms expelled from the mouth and nasopharynx of healthcare workers during surgery, thereby providing protection for the patient. However, there are several ways in which surgical face masks could potentially contribute to contamination of the surgical wound. Surgical face masks have recently been advocated as a protective barrier between the surgical team and the patient, but the role of the surgical face mask as an effective measure in preventing surgical wound infections is questionable. The aim of the systematic review is to identify and review all randomised controlled trials evaluating disposable surgical face masks worn by the surgical team during clean surgery to prevent postoperative surgical wound infection. All relevant publications about disposable surgical face masks were sought through the Specialised Trials Register of the Cochrane Wounds Group (March 2001). Manufacturers and distributors of disposable surgical masks as well as professional organisations including the National Association of Theatre Nurses and the Association of Operating Room Nurses were contacted for details of unpublished and ongoing studies. Randomised controlled trials (RCTs) and quasi-randomised controlled trials comparing the use of disposable surgical masks with the use of no mask were included.

Main results: Two randomised controlled trials were included involving a total of 1453 patients. In a small trial there was a trend towards masks being associated with fewer infections, whereas in a large trial there was no difference in infection rates between the masked and unmasked group. Neither trial accounted for cluster randomisation in the analysis.

Reviewers' conclusions: From the limited results it is unclear whether wearing surgical face masks results in any harm or benefit to the patient undergoing clean surgery.
