Background Coronavirus disease 2019 (COVID-19) pandemic affected the world especially those with chronic diseases. Diabetes mellitus (DM) is considered one of the most prevalent noncommunicable diseases. Type 1 diabetes (T1D) is one of the most common endocrine metabolic disorders around the world. This study aimed to assess the perceived fear of COVID-19 and its relationship to HRQoL among children and adolescents with T1D during the pandemic in Ismailia City. Methods This is a retrospective study carried out on T1D children and adolescents during pandemic in Ismailia City and conducted at Suez Canal University endocrinology outpatient clinic, Ismailia City, Egypt from June 30, 2021, to December 31, 2021. All participants were interviewed using a semi-structured demographic and clinical checklist, fear of COVID-19 Scale, and the short form 36 (SF-36) health survey. Blood samples were taken to analyze HbA1c for all patients. Results This study analyzed data from 68 children and adolescents with T1D with age ranged from 10 to 14 years, 58.8% were females and 55.9% had preparatory school education. 38% had BMI more than 30, 38.2% had DM for more than 5 years. HbA1C was more than 8 among 41.2%. 59% had low FCV-19 and 41% had high FCV-19. Mean PCS was 39.1±9.6 and 82% had poor PCS. MCS was ranging from 19 to 36 with a median of 28. Older age, preparatory educated, and larger BMI children had higher fears of COVID-19. Poor DM control, and poor adherence to glucose and diet showed higher FCV-19 scores. Higher PCS was associated with higher FCV-19 score. Using univariate logistic regression, age, HbA1C, type of insulin regimen, family history of DM, and FCV-19 score are predictors for PCS. While age, grade, BMI, HbA1C, adherence to glucose testing, type of insulin regimen, adherence to diet control, and PCS are predictors for FCV-19. Conclusion This study suggested that children and adolescents with T1D showed above-average levels of fear of COVID-19, and it may be associated with lower quality of life, especially physical aspects.

Wanning rate of maternal-derived antibody against respiratory syncytial virus in children 小儿呼吸道合胞病毒母源性抗体检出率

Dear Editor: One of the most dangerous pathogenic diseases in children, respiratory syncytial virus (RSV) causes severe morbidity and mortality [1]. Despite extensive studies in infectious immunobiology, clinical signs, diagnostic techniques, and animal models. The information from the sero-epidemiology study for RSV among the pediatric population is helpful in creating a public health strategy to correspond to the virus. In a nutshell, the mother’s antibodies may be transferred to the newborn through the placenta. There will be a waning of transmitted antibodies and a potential for new infection. It’s interesting to see how the seroprevalence of the antibody against RSV changes dynamically with age. In babies, RSV antibodies rapidly decline, and RSV infection subsequently increases anti-RSV IgG titers, according to a recent study. Children should be urged to get their RSV shots before they turn seven months old [2]. For the seroprevalence research for RSV, however, the diagnostic power of the laboratory instrument is a key concern in laboratory medicine. ELISA kits frequently still have limited sensitivity and a high percentage of false negatives, despite the fact that they are normally used widely [3]. In order to evaluate the results of a seropeidemiology inquiry, it is essential to take the influence of false negative into consideration. This article’s authors address the altered sero-epidemiology pattern of RSV antibodies among children in a country in Southeast Asia throughout a range of age groups. The EUROMIN ELISA test offers 96.2% sensitivity and 100% specificity based on the product data available, which implies 3.8% false negatives. 95.2% of people had IgG antibodies to RSV at birth [2]. The antibody starts to decline after birth and reaches its lowest point at 7 months of age, or 8.1%. The crude wanning rate will therefore be 87.1% every seven months or 12.4% per month. The adjusted wanning rate will be 11.9% per month following the erroneous negative adjustment. Diagnostic and prognostic role of troponin I in neonates with critical duct-dependent congenital heart diseases 肌钙蛋白I在新生儿导管依赖性先天性心脏病中的诊断和预后作用

Background Cardiac troponins (cTn) are markers of myocardial injury with well established diagnostic value in adults. However, the benefits of using troponins in neonates with critical duct-dependent congenital cardiac diseases (cDD-CHDs) is questionable. Aim The study aimed to investigate the diagnostic role of cTnI in cDD-CHDs as a marker of myocardial injury and to explore its role in patient’s outcome. Methods A case-control study, conducted over 80 neonates; Group I: Forty neonates with cDD-CHDs and Group II: Forty healthy neonates. CBC, CRP, electrolytes, liver and kidney functions, cTnI and echocardiography were done for both groups in first week of life. Then serum level of cTnI were re-evaluated after 3 weeks from initial sampling. Results The mean value of serum cTnI was significantly higher among cases when compared to controls (0.91±1.88 ng/ml versus 0.003±0.006 ng/ml, P<0.001*). The diagnostic performance of cTn I for cardiac injury was at cut-off 0.03 ng/ml with sensitivity 65% and specificity 97.5%. However, the prognostic role of cTnI in predicting death was at serum level of 0.7 ng/ml with 61% sensitivity and 81% specificity. cTn I in cDD-CHD was positively correlated to the severity of heart failure (HF) based on Modified Ross Score. The mean value of survival of the cDD-CHDs neonates was 28.11 days when cTnI is >0.6 ng/ml. Conclusion Initial serum levels of cTnI of cDD-CHD neonates in first week of life might be a useful marker in predicting the 40-days-survival and outcome.

However, the benefits of using troponins in neonates with critical duct-dependent congenital cardiac diseases (cDD-CHDs) is questionable. Aim The study aimed to investigate the diagnostic role of cTnI in cDD-CHDs as a marker of myocardial injury and to explore its role in patient’s outcome. Methods A case-control study, conducted over 80 neonates; Group I: Forty neonates with cDD-CHDs and Group II: Forty healthy neonates. CBC, CRP, electrolytes, liver and kidney functions, cTnI and echocardiography were done for both groups in first week of life. Then serum level of cTnI were re-evaluated after 3 weeks from initial sampling. Results The mean value of serum cTnI was significantly higher among cases when compared to controls (0.91±1.88 ng/ml versus 0.003±0.006 ng/ml, P<0.001*). The diagnostic performance of cTn I for cardiac injury was at cut-off 0.03 ng/ml with sensitivity 65% and specificity 97.5%. However, the prognostic role of cTnI in predicting death was at serum level of 0.7 ng/ml with 61% sensitivity and 81% specificity. cTn I in cDD-CHD was positively correlated to the severity of heart failure (HF) based on Modified Ross Score. The mean value of survival of the cDD-CHDs neonates was 28.11 days when cTnI is >0.6 ng/ml.
