Introduction: Tram (transverse abdominal rectum flap) has remained the most used autologous breast reconstruction method over the last 30 years. First described by Holmström, the flap allows breast reconstruction with analogous tissue, providing natural appearance and consistency and lasting results. Reconstruction of the abdominal wall of the defect installed after flap transposition is a major challenge, and there is no consensus on the form for elevation or closure of the abdominal wall flap. The article aims to present a practical method for abdominal wall reconstructions to reduce morbidity in patients undergoing breast reconstruction with TRAM. Methods: This is a descriptive work of a systematic abdominal wall reconstruction technique using propylene mesh. Result: Once the technique is applied, we have an abdominal wall covered with polypropylene fabric, fully fixed and well adapted. The systematization of abdominal wall reconstruction after reconstruction with TRAM was performed, which is characterized by being easy to reproduce and applicable. Conclusion: The technique is a good alternative in abdominal wall reconstructions for the surgeon, systematizing polypropylene mesh adaptation. ■ ABSTRACT

Introduction: Homologous skin grafting may represent the difference between the life and death of large burned patients. Its use consists of a precious treatment when there is no availability of autologous graft. The tissue banks were created to perform the processing and storage of the allogenous skin. The objective is to analyze the epidemiological profile of skin donors and recipients from the skin bank of the Hospital Universitário Evangélico Mackenzie (HUEM) and its productivity, from its inauguration in 2013 to 2019. Methods: Consultation of annual reports for the seven years. Results: The skin of 187 donors was captured, of which 61% were men and 39% were women. The mean age was 41.07 years. The number of donors averaged 21.3 per year. A total of 201,000cm2 of viable tissue were collected, which resulted in 3,770 skin slides. Since 2013, 325 allogenous grafts have been performed at HUEM that have benefited 194 people. The mean age of patients receiving the skin was 34.67 years. Most of the skin captured, processed, and stored by the HUEM bank (about 91%) was used in grafting carried out in the institution itself. Conclusion: The HUEM skin bank provided allografts that benefited 194 people in 7 years of operation. Most donors and recipients were male and approximately 40 years old. The number of captures performed by this skin bank was compatible with that of other institutions in Brazil. ■ ABSTRACT

Perfect understanding of the differences between masculinizing and feminizing attributes on a face should be encouraged in order to achieve better aesthetic results and the prevention of treatment stigmas. Introduction: Lipografting is an alternative with important applicability for breast reconstruction and/or corrections of asymmetries resulting from cancer treatment. This technique consists of autologous fat transfer, whose stroma contains stem cells derived from adipose tissue that can differentiate itself throughout the mesodermal lineage. For adipose tissue preparation, Coleman-based centrifugation of syringe-aspirated material at 3000 revolutions per minute (rpm) for 3 minutes. However, studies question whether lower centrifugation speeds could be less harmful to cell viability. Methods: An experimental study was conducted to evaluate the adipose cells of six patients; from 60mL of liposuction of each one. The sample collected was fractionated into four tubes and submitted to different protocols, decanting and centrifugation at speeds 500, 1000, and 3000rpm for 3 minutes. Afterward, the samples were processed with collagenase IA for 30 min, submitted to cell culture for 24 hours, and a cell viability analysis. The results were tabulated and analyzed by the ANOVA test using the Graphpad Prism 6.0® and SAS®. Results: Cell viability was higher in the cell sample centrifuged at 3000rpm and lower in the decanted sample. Giemsa staining indicated maintenance of cell morphology on the samples. Conclusion: Centrifuged cells at a speed of 3000rpm showed higher cell viability. Centrifugation was effective in compacting tissue and eliminating unwanted waste (blood and residual oil). ■ ABSTRACT

Introduction: Although treatment of head and neck cancer has evolved in recent years, it is still a challenge to preserve and restore facial symmetry afterward; this fact significantly compromises the quality of life of patients. In reconstructive surgery, autologous fat grafting has been used to treat volume and contour defects. The objective is to review the effects of fat grafting in patients with radiodermatitis due to head and neck cancer radiotherapy treatment. Methods: Literature review in PubMed and SciELO databases, looking for studies published in the last 20 years in Portuguese, English, or Spanish. The descriptors “adipose tissue”, “transplantation”, “neoplasms”, “head and neck neoplasms” and “radiotherapy” and the non-descriptive terms “fat grafting”, “fat transplantation” and “fat graft” were used. Results: Of 212 articles found, only seven complied with the eligibility criteria. Conclusion: The review suggests that fat grafting in head and neck cancer reconstruction is a safe treatment and optimizes aesthetic and functional results. Prospective studies are lacking for a better definition of techniques and results. ■ ABSTRACT

Trata-se de um relato de caso de reconstrução microcirúrgica de mandíbula com retalho osteocutâneo de fíbula em um paciente de 16 anos, vítima de agressão com projétil de arma de fogo e trauma contuso em face, acarretando em fratura cominutiva de mandíbula, com perda de elementos dentários e lesões extensas de partes moles. Após tratamento da fase aguda, optou-se pela reconstrução microcirúrgica com retalho livre de fíbula, devido à extensão do defeito mandibular e possibilidade de reabilitação dentária. O uso de modelos tridimensionais da mandíbula e fíbula foram excepcionalmente úteis para tornar possível o melhor planejamento cirúrgico das áreas de osteotomia, com diminuição do tempo de isquemia do retalho da cirurgia. ■ RESUMO

Introduction: Kaposiform cutaneous hemangioendothelioma (HEK) is a rare locally aggressive vascular tumor, seen mainly in newborns and children. It has a prevalence of 0.91 cases per 100,000 children, being most common in the extremities. The treatment of choice is total resection; however, it is often not possible due to the lesion’s extent and association with the Kasabach-Merritt phenomenon. Objectives: To describe the evolution of a rare tumor in the plantar region of a child, correlating the radiological, surgical, and histopathological findings. Methods: The authors report the case of a boy admitted at the age of five with a recurrent painful plantar skin lesion. In the magnetic resonance examination (NMR), he presented a lesion in the posterior plantar region measuring 3cmx2cm, superficial to the plantar fascia. In the biopsy examination, he revealed kaposiform hemangioendothelioma without association with the Kasabach-Merritt phenomenon. He underwent a surgical procedure for excision and presented recurrence after six months. A new broad resection, reconstruction with a plantar flap, and partial skin graft were performed, obtaining free margins, with no recurrence in the 15-year follow-up. Conclusion: Clinical findings suggested plantar fibromatosis, NMR helped in delimiting the tumor, and histopathological examination with immunohistochemistry confirmed the diagnosis of kaposiform cutaneous hemangioendothelioma. Resection was performed up to the fascia with recurrence, requiring re-approach and resection to the periosteum with reconstruction using a plantar flap and skin graft, without recurrence after 15 years. The authors call attention to the wide resection of deep and lateral margins to control tumor growth. ■ ABSTRACT

Introduction: The increasing incidence of skin cancer leads to a high number of surgical procedures worldwide. The surgical treatment of skin cancer’s main objective is its complete excision, preserving the function and the best aesthetic result. Incomplete initial resection can result in recurrences and major damage. The objective is to analyze the risk factors for positive margins in the follow-up of cutaneous lesions surgically removed, for one year, in the plastic surgery department of the Royal Perth Hospital. Methods: A survey of histopathological samples from 947 operated patients was analyzed. All patients with confirmed incomplete excision (IE) underwent a second surgery or even a third time. Results: In total, 947 lesions were found, 6.6% of surgeries had compromised margins, with a histopathological distribution of 75% of basal cell carcinoma, 21.4% of squamous cell carcinoma, and 3.6% of other lesions. The relation of the presence of compromised surgical margins between the SCC, compared to BCC, leads to a relative risk of 2.8 and a p-value of 0.041, which suggests that the SCC is a risk factor for the presence of compromised surgical margins. For staging, the need for a second surgical approach was present in 61.29% of the patients, 20.9% were under observation, 3.2% were absent from the service, 8% went directly to chemotherapy or radiotherapy, and 6.4% rescheduled the surgery. Conclusion: Knowledge of risk factors for positive margins is necessary for the surgeon to understand the prognosis and monitoring of each patient. ■ ABSTRACT

Tromboses e tromboflebites superficiais de membros superiores são doenças frequentes e muitas vezes pouco valorizadas. Relatamos três pacientes no pós-operatório de cirurgias plásticas mamárias estéticas que apresentaram tromboflebite em membros superiores e que evoluíram com tromboembolismo pulmonar. Todas as pacientes apresentaram quadro clínico típico, com comprovação pelo Doppler ultrassonografia de trombose/flebite superficial de membros superiores e ausência de lesões em membros inferiores, bem como aumento de dímero-D e comprovação da embolia pulmonar por tomografia computadorizada ou cintilografia pulmonar. Os três casos evoluíram com melhora após anticoagulação e sem sequelas. ■ RESUMO
