This article describes the results of a research about role conceptions and role expectations of physicians, nurses and patients in two general hospitals. The research reveals discrepancies between the role conceptions and role behaviour of nurses and also between the role expectations of nurses and role behaviour of physicians. In contrast, patients and physicians, for the most part, were satisfied with the role behaviour of the nurses. These discrepancies can be a barrier to collaboration between physicians and nurses and to quality of care. Discussing specific case studies is a possible strategy which can be used to influence the role conceptions and role expectations of physicians and nurses from a hospital ward.

The number of patients with heart failure is growing. To optimize care for these patients in hospital and at home, a supportive-educative intervention is developed. The intervention is guided by a standard nursing care plan which is developed from literature, existing standard nursing care plans, and interviews with nurses. A case study is used to explain and illustrate the intervention and to review the importance of the standard nursing care plan.

Seclusion is one of several possible interventions with which patients and health care professionals can be confronted on units for admission and crisis intervention in mental health care institutions. The way in which psychiatric nurses shape their practice with regard to seclusion and how they experience this intervention is unclear. The goal of this study is to describe this practice. The results render insight in the motives and reasons of nurses for use of seclusion, the content of the term emergency situation, the way in which decisions to seclude are taken and which interventions can be labelled as preventive. Finally conclusions are given and the questions raised in this study are discussed.

A controlled cross-over experiment was carried out to investigate the effect of snoezelen in the snoezelenroom on the well-being of elderly people with dementia. Seventeen elderly residents from the Bernardus Nursing Home in Amsterdam participated. All were in a very advanced stage of dementia requiring a high level of care and nursing. In this study, the presence of behavioural problems was used as a basis for the measurement of well-being. The behaviour was registered by video cameras and analysed by means of certain subscales of a Dutch behavioural observation scale for intramural psychogeriatrics, the Gedragsobservatieschaal voor Intramurale Psychogeriatrie (GIP). The results showed that less behavioural problems were seen with the experimental intervention. This indicates a higher degree of well-being during the snoezelen activity in the snoezelen room than when the residents remained in the living room. The results of this study indicate the need for a large-scale study in which additional outcome parameters are measured.

In this article, the results are discussed of a literature study on nursing care for heroin addicts in out-patient methadone maintenance clinics in the Netherlands and the United States. In the Netherlands, the most important aim of methadone maintenance is to limit the harm caused by heroin use. Community based methadone maintenance is the subject in several Dutch evaluation studies. Care is described in these studies, however the medical role is not distinguished from the nursing role and functions. Research variables are basic care assumptions and client satisfaction. In the USA, heroin misuse is considered a disease more than in the Netherlands. Nurses in the American methadone maintenance clinics have a more independent function compared with the nurses in the Netherlands. The main variables in the American studies on nursing care in the out-patient methadone maintenance clinics are the nursing process as well as the development and testing of nursing interventions. Further nursing research concerning the methadone maintenance clinics in the Netherlands is needed because the nursing contribution in maintaining and improving the basic health status of the heroin misuser is of vital importance. The American nursing research and the associated outcomes could be used as a guideline.

This article describes the development and psychometric evaluation of self-report measurement instruments in (nursing) research. The aim of this study is to gain more insight into, and understanding of the use of such instruments. To be more specific, this paper deals with: (1) What is a self-report measurement instrument?; (2) How to develop such an instrument?; (3) What is its psychometric quality in terms of validity and reliability?; (4) How to analyze an instrument statistically?; (5) Which are the pros en cons of a self-report measurement instrument in general?; and (6) Where do we find examples of good measurement instruments? These six questions will be answered with the help of several practical research examples. The article concludes with a few suggestions for literature concerning existing measurement instruments and their psychometric qualities.
