1. Small items sent via the post present a much greater hidden hazard and a higher potential for catching people unawares. 2. Large items accompanied by a Decontamination Certificate need to be treated with caution because only those surfaces which are accessible will have been cleaned; as soon as engineers start dismantling equipment, areas which are still soiled become exposed. 3. Total co-operation between hospital staff and service engineers is necessary to resolve these problems-neither can work safely without the assistance of the other. 4. Many products in use in hospitals may be up to 20 years old and clearly were not designed to facilitate decontamination and therefore present far greater risk to all concerned. 5. For the future there a clearly a responsibility with manufacturers to give more thought to the design of their equipment so that decontamination can be achieved more effectively.

The development of medical equipment that cannot adequately be sterilised or disinfected has caused growing concern in recent years. Areas of concern are equipment that is likely to be damaged by normal autoclaving temperatures, and is therefore heat labile, or is so complicated in structure that adequate penetration of chemical disinfectants is unlikely. The need has led to the search for a method that would give adequate penetration of a bacteriocide without the use of excessive temperatures. The penetrative power of gaseous sterilant/disinfectants appears to be the area most likely to fulfill these considerations. Ethylene oxide (Phillips 1949) and Low Temperature Steam, with or without Formaldehyde, have been investigated by various workers.

Hospitals and manufacturers of medical supplies use ethylene oxide (EO) gas almost exclusively to sterilize devices that are heat sensitive. However, the evidence relating the mutagenicity and cytogeneticity shows the desirability of reducing personal exposure to a practical minimum.
