Outcome of Total Surgical Debridement of Covid Associated Rhino-Orbito-Cerebral Mucormycosis Based on a New Surgical Staging System: A Cohort Study. 基于新手术分期系统的 Covid 相关性鼻-眼-脑粘液瘤病的全手术清创结果:一项队列研究

To propose a surgical staging system with management protocol for post-covid Rhino-orbito-cerebral mucormycosis (ROCM) with central skull base osteomyelitis. A prospective cohort study of a total of 193 post-covid ROCM patients was conducted between May 2021 and January 2022 at a tertiary care centre. Patients were assessed radiologically and staged from I to V. Follow up period was 16 months and the surgical outcome in terms of recurrent disease was assessed. A total of 193 patients (129 primary and 64 revision) were studied. Maxilla was found to be the epicenter of anterior disease (69.3%) and pterygoid wedge was noted to be the epicenter of posterior disease (85.6%). More than 65% of our patients, at the time of presentation, presented with involvement of the central skull base. Intracranial disease was noted in 13.9% of patients and the mortality rate was 6.2%. This staging system provides a systematic step-by-step protocol for the management of ROCM, with emphasis on meticulous disease clearance at the central skull base.

Invading the dynamics of economic growth and CO2 emission: panel data error correction model (ECM) approach. 入侵经济增长和二氧化碳排放的动态:面板数据误差修正模型(ECM)方法。

This paper examined the impact of green finance and financial technology on economic growth by utilizing statistics between 1990 to 2000 gathered from 50 US states and regions. A two-step generalized GMM was used to analyze the link between green finance, financial technology, and continued economic growth using panel regression analysis. We found that the US green finance sector had grown significantly and such growth was accompanied by an increase in the usage of non-combustible energy and an advancement in the green finance economic forum. This research concludes that green financing has a beneficial impact on all three components of continued financial development. There is a favorable influence of financial technology on green finance in environmental and economic dimensions, although the link between green banking and investment performance is only somewhat affected by technology. When total emissions increased, non-oil energy usage did not grow and initiatives for renewable energy initiatives were lacking. As a result, there was a decline in the growth of green finance. Green funding and total emissions had a significant impact on the US non-combustible energy usage as did explicit policy changes. The recommendation is also canvassed to strengthen the adoption of green financing policies, increase the use of non-combustible energy, and build an alternative energy economy. It also offers three policy recommendations for policymakers, namely, to improve the integration of banking technology with green finance, develop a corporate environmental approach to manage and control state authorities in increasing green finance productivity, and generate medium- and long-term favorable steps to support green finance in the financial market.

<p><strong>Background: </strong>The management of dental caries has traditionally involved removal of all soft demineralised dentine before a filling is placed. However, the benefits of complete caries removal have been questioned because of concerns about the possible adverse effects of removing all soft dentine from the tooth. Three groups of studies have also challenged the doctrine of complete caries removal by sealing caries into teeth using three different techniques. The first technique removes caries in stages over two visits some months apart, allowing the dental pulp time to lay down reparative dentine (the stepwise excavation technique). The second removes part of the dentinal caries and seals the residual caries into the tooth permanently (partial caries removal) and the third technique removes no dentinal caries prior to sealing or restoring (no dentinal caries removal). This is an update of a Cochrane review first published in 2006.</p><p><strong>Objectives: </strong>To assess the effects of stepwise, partial or no dentinal caries removal compared with complete caries removal for the management of dentinal caries in previously unrestored primary and permanent teeth.</p><p><strong>Search methods: </strong>The following electronic databases were searched: the Cochrane Oral Health Group's Trials Register (to 12 December 2012), the Cochrane Central Register of Controlled Trials (CENTRAL) (The Cochrane Library 2012, Issue 11), MEDLINE via OVID (1946 to 12 December 2012) and EMBASE via OVID (1980 to 12 December 2012). There were no restrictions regarding language or date of publication.</p><p><strong>Selection criteria: </strong>Parallel group and split-mouth randomised and quasi-randomised controlled trials comparing stepwise, partial or no dentinal caries removal with complete caries removal, in unrestored primary and permanent teeth were included.</p><p><strong>Data collection and analysis: </strong>Three review authors extracted data independently and in triplicate and assessed risk of bias. Trial authors were contacted where possible for information. We used standard methodological procedures exacted by The Cochrane Collaboration.</p><p><strong>Main results: </strong>In this updated review, four new trials were included bringing the total to eight trials with 934 participants and 1372 teeth. There were three comparisons: stepwise caries removal compared to complete one stage caries removal (four trials); partial caries removal compared to complete caries removal (three trials) and no dentinal caries removal compared to complete caries removal (two trials). (One three-arm trial compared complete caries removal to both stepwise and partial caries removal.) Four studies investigated primary teeth, three permanent teeth and one included both.
