NUMEROUS procedures have been described for repair of congenital clefts of the lip. The older methods aimed at closure of the defect in primitive fashion, with insufficient regard for the cosmetic result. 1 From the time of Celsus until well into the 19th century, the operation consisted of an excision of the margins of the cleft and a crude approximation of the raw edges (Fig 1). Needless to say, failures were frequent. About 1830, Dieffenbach 2 introduced lateral undermining incisions to allow sliding of the flaps together with less tension. Mirault 3 is generally credited with having been the first, in 1844, to bring a flap from the lateral segment across the lower margin of the cleft, thereby adding improvement to the postoperative appearance of the lip (Fig 2). His method became the forerunner of various modern procedures. Numerous contributors have added their own modifications over the years. Collis improved

To the Editor .—The article "Audiologic and Otologic Screening of Disadvantaged Children" in the AprilArchives, (Fay et al: 91 :366-370) contains a statement that I feel needs amplification. A recommendation for T & A was made in 88 of 255 or a third of the children examined. In the first paragraph the authors state that these children are the bottom of the barrel and have had virtually no medical care and apparently little past history was available. The turnover was rapid and there was apparently no opportunity to follow these patients. In view of these facts, I do not see how a recommendation could be made that they have a major surgical procedure. There is much difference of opinion among physicians regarding T & A but I feel that almost everyone agrees a history and a period of observation is necessary to arrive at a reasonable approach to patients

The free amino acid content of horse perilymph, cerebrospinal fluid, and blood serum was comparatively studied. The results obtained show clear-cut differences between the three fluids, and suggest that perilymph may be considered as a biological fluid with its own specific characteristics, probably correlated to the metabolism of its secreting structure.

To the Editor .—In the article "Sodium and Potassium Changes in Inner Ear Fluids" by Nakashima et al ( 92 :1-6, 1970) the authors report large shifts in endolymph sodium and potassium ion concentrations following intense acoustical stimulation of the guinea pig's inner ear (140 dB). These changes failed to return to normal after the stimulation was stopped. These results appear to me to be caused by a rupture of the Reissner's membrane and not by an increased permeability of the Reissner's membrane as the authors suggest. In these experiments, histological study of serial sections of the guinea pig's inner ear would provide the necessary information to interpret these results. Histological or electron microscopic study is frequently necessary before interpreting experiments on the inner ear.

Five cases of unilateral vocal-cord paralysis after surgery unrelated to the neck occurred. The asymmetric inflation of an endotracheal tube cuff which lies just beneath the vocal cords is suspected as the etiologic agent. Surprisingly, this complication is almost unreported in the literature.
