Amac: Nutrisyonel rikets, buyume cagindaki cocuklarda D vitamini ve kalsiyum eksikligine bagli olarak gelisen epifizyal kikirdagin defektif mineralizasyonudur. Nutrisyonel riketsin onlenmesi icin D vitamini profilaksisi ulkemizde sut cocuklugu doneminde rutin olarak uygulanmaktadir. Ancak halen semptomatik hipokalsemi ve D vitamini eksikligi nedeniyle hastane basvurularinin olmasi dikkat cekicidir. Calismamizda bu olgularin ozelliklerinin ortaya konulmasi amaclanmistir. Gerec ve Yontem: Ocak 2018-Mayis 2018 tarihleri arasinda nutrisyonel rikets tanisi ile yatirilarak izlenmis olan 28 hastanin (9 kiz, 19 erkek) verisi retrospektif olarak incelendi. Olgularin basvuru anindaki antropometrik ve laboratuvar ozellikleri kaydedildi. Bulgular: Olgularin ortanca basvuru yasi 8,8 (1-35) ay idi. Olgularin %67,9’u Suriye gocmeni idi. Hastalarin %57,1’i hic D vitamini profilaksisi almamis, %42,9’u ise duzensiz almisti. Basvuru yakinmasi olgularin %50’sinde konvulsiyon gecirme, %28,6’sinda rikets bulgulari iken %21,4’unde ise ates, enfeksiyon vb. nedenlerden dolayi hastaneye basvuran hastalarda rastlantisal olarak hipokalsemi tespit edilmis olmasi idi. Ilk basvuruda ortanca kalsiyum 6,6 mg/dL; iyonize kalsiyum 0,85 mmol/L; fosfor 3,5 mg/dL; alkalen fosfataz 787 U/L; parathormon 241 pg/mL ve 25-hidroksi vitamin D 3,4 ng/mL saptanmisti. Olgularin %60,7’sinde hipofosfatemi, %3,6’sinda hiperfosfatemi mevcuttu. Tum olgular hastanede yatirilarak tedavi edilmis ve D vitamini eksikligine bagli nutrisyonel rikets tanisi almisti. Sonuc: Ulkemizde D vitamini profilaksisine ragmen halen nutrisyonel rikets gorulebilmekte ve kis-ilkbahar aylarinda hipokalsemik konvulsiyon nedeniyle hastane basvurularina yol acabilmektedir. Ozellikle gocmen Suriyeli sut cocuklarinda D vitamini destek programina uyumun artirilmasi nutrisyonel riketsin onlenmesi acisindan onemlidir.

Acute ischemic stroke is rare in children, but it is one of the important causes of neurological morbidity. The etiology of AIS in childhood includes cardiac pathologies, cerebral arteriopathies, hypercoagulation, chemicals, drugs, inflammatory and autoimmune causes. In this article, we present an infant who has a genetic predisposition to hypercoagulation and comes with AIS clinic when multifactorial risk factors are added. The diagnosis, the underlying etiology, the risk factors affecting the process, the treatment of acute phase, the secondary treatment and the associated preventive approach on prognosis were discussed.

Objective: Interleukin 1 beta (IL-1β) is an essential proinflammatory mediator emerging early in bacterial infection. Urinary tract infections (UTI) are one of the most common bacterial infections during childhood. This prospective study aimed to explore whether urine levels of IL-1β could be used as a biomarker of UTI. The study also aimed to define the optimal cut-off level for urine IL-1β to detect UTI in children. Material and Method: Sixty patients with symptomatic UTI and 29 healthy controls were registered in the study. After taking informed consent, urine culture, urinalysis, serum urea and creatinine, C-reactive protein (CRP) and serum white blood cell count measurements were performed in both groups. Urine IL-1β was analyzed by enzyme-linked immunosorbent assay. Results: The mean urine IL-1β level was significantly greater in the UTI group than in the controls (21.14 pg/ml vs 2.60 pg/ml, p=0.0001), and using a cut-off of 6.11 pg/ml for urine IL-1β for early prediction of UTI, sensitivity and specificity were 100% and 93.1%, respectively (AUC: 0.971). The mean urine IL-1β/creatinine ratio (uIL-1β /Cr) was also significantly greater in the UTI group (46.87 pg/mgCr vs 3.29 pg/mgCr; p=0.0001), and using a cut-off of 9.45 pg/mgCr for uIL-1β/Cr for early prediction of UTI, sensitivity, and specificity were 96.7% and 89.7%, respectively (AUC: 0.963). Conclusion: Both urine IL-1β and uIL-1β/Cr can be used as a sensitive biomarker for early prediction of UTI. Further research with larger groups is necessary to support our results.

Amac: Ilkokul cagindaki cocuklarin bilgisayar, internet ve bilgisayar oyunlari kullanimini etkileyen faktorleri ve dusuncelerini belirlemek amaciyla yapildi. Gerec ve Yontem: Arastirma tanimlayici tipte olup Mart-Mayis 2018 tarihleri arasinda Zonguldak Ilkokulu’ndaki ogrenciler ile gerceklestirildi. Orneklem grubunun seciminde basit rastgele yontem ile bir ilkokul arastirma kapsamina alindi. Arastirmanin orneklemini belirtilen tarihlerde arastirmaya katilmayi kabul eden ogrenciler olusturdu. Veri toplama araci olarak, “Kisisel Bilgi Formu” ve “Literatur Isiginda Hazirlanan Soru Formu” kullanildi. Bulgular: Ogrencilerin %74,4’unun kendine ait bilgisayari oldugu, %88,7’sinin bilgisayari evde kullandigi, %62,2’sinin 5 yasindan sonra bilgisayar kullanmaya basladigi, %85,4’unun evinde internet oldugu, %67,7’sinin hic internet kafeye gitmedigi, %15,2’sinin her gun bilgisayar/ tablet kullandigi, %50’sinin gunluk ortalama oyun saatinin 0-1 saat oldugu, %50,0’inin bilgisayari oyun icin kullandigi ve %44,5’inin bilgisayari odev icin kullandigi saptandi. Ogrencilerin %37,2’si, bilgisayar oyunu oynamanin onlarin bazi bilgi ve becerilerini gelistirecegini dusunmektedir. Ogrencilerin %47,0’i oyun oynamanin zaman kaybi olmadigini dusunurken; %41,2’si bilgisayarda tek basina oyun oynamanin zevkli olmadigini belirtmektedir. Ogrencilerin cinsiyet ile bilgisayari daha cok ne icin kullandigi ile ve kendilerine ait bilgisayarinin olmasi ile her gun bilgisayar oynamasi ile arasinda anlamli sonuc bulundu (p<0,05). Sonuc: Teknolojinin ilerlemesiyle birlikte cocuklarda bilgisayar, internet kullaniminin ve bilgisayar oyunlarini oynama sikliginin arttigi sonucuna varildi. Konu ile ilgili denetimlerin ve ebeveyn kontrolunun saglanmasinin onemli oldugu ortaya koyuldu.
