Fentanyl products in the management of episodic pain in the cancer patient 芬太尼产品在治疗癌症患者阵发性疼痛中的应用

ABSTRACT Introduction: Breakthrough cancer pain (BTcP) has been defined as a pain exacerbation of short duration in patients who have a well controlled pain for most hours of the day. The principal treatment is represented by administration of a medication as needed. Areas covered: In the last years several short onset delivery systems have been developed. In comparison with oral opioids that often do not fit the typical pattern of a BTcP event, fentanyl products have been shown to produce an analgesic effect within 5–15 min after administration and a duration of about 2 h. These characteristics make these products indicated for BTcP, although concerns have been raised regarding the costs. The choice of the dose is controversial with regulatory studies suggesting the need to titrate the dose, and common clinical sense that suggests to use doses proportional to the basal opioid regimen. Expert commentary: The treatment of BTcP should depend on its characteristics. Pharmacokinetics of oral opioids indicate a late analgesic effect that in most cases is not overlapping the temporal pattern of BTcP, unless in determined circumstances. Dosing of fentanyl products is still matter of controversies.

Methods of controlled ovarian stimulation for embryo/oocyte cryopreservation in breast cancer patients 控制卵巢刺激用于乳腺癌患者胚胎/卵母细胞冷冻保存的方法

ABSTRACT Introduction: Controlled ovarian stimulation (COS) is a crucial step before oocyte collection and subsequent embryo/oocyte cryopreservation. However, in young breast cancer patients, the need for COS raises specific issues that should be taken into account during oncofertility counseling. Areas covered: We aim to perform an overview of COS protocols with a specific focus on the current knowledge and clinical challenges related to their use in young breast cancer patients. Expert commentary: Many protocols for COS are currently available to induce multiple follicular recruitment and maximize oocyte retrieval in women undergoing in vitro fertilization treatment. In cancer patients, to avoid the need to wait for a spontaneous menstruation, emergency protocols are also available so that COS can be started at any time during the menstrual cycle. COS protocols with the addition of tamoxifen or letrozole have been specifically designed for breast cancer patients to limit the potential negative impact of the temporary increase in estradiol levels during stimulation.

Frailty and quality of life in adult survivors of childhood cancer. 儿童癌症成年幸存者的虚弱和生活质量。

Introduction: Adult survivors of childhood cancer are at risk for frailty, a state of diminished physiologic reserve, typically seen in older adults, associated with morbidity and mortality. They are also at risk for poor quality of life. This review explores potential associations between frailty and quality of life and proposes potential interventions that may prevent or remediate frail health.

Areas covered and literature search: A literature search using MEDLINE and EMBASE was undertaken using key words: childhood cancer survivors and each of the following: frailty, physical performance, emotional health, and quality of life. 796 abstracts were reviewed for relevance. Reference lists for retrieved articles were consulted for pertinent manuscripts (N=40). Recent literature (from January 2013 to November 2016) was obtained using the key words: aging and frailty and quality of life. The final reference list includes 44 documents.

Expert opinion: Childhood cancer survivors are at risk for frailty and less than optimal quality of life. Poor emotional health may contribute to this association. Further research is needed to better explain these associations and to provide a foundation for the development of interventions designed to prevent and remediate frail health and suboptimal quality of life in this population.

Quality of Life in Palliative Care. 姑息治疗中的生活质量。

Introduction: A main goals of palliative care is to improve the health-related quality of life (QOL) of patients with advanced illnesses. The objective of this narrative review is to provide an updated synopsis on the use of QOL questionnaires in the palliative care setting.

Areas covers: Focusing on the palliative cares setting, we will define QOL, discuss how QOL instruments can be used clinically and in research, review approaches to validate these questionnaires, and how they can be used in utility analyses.

Expert opinion/commentary: Several QOL questionnaires, such as EORTC-QLQ-C30, McGill QOL questionnaire and EQ-5D have been validated in the palliative care setting. However, significant gaps impede their application, including lack of determination of their responsiveness to change and minimal clinically important differences, the need to conduct more psychometric validation on QOL questionnaires among patients at various stages of disease trajectory, and the paucity of studies examining utility and cost-effectiveness. Further research is needed to address these knowledge gaps so QOL questionnaires can be better used to inform clinical practice and research.

New Promising Strategies in Oncofertility. 肿瘤不孕不育领域前景广阔的新策略

Introduction: Approximately 70,000 adolescent and young adults (AYA) are diagnosed with cancer each year. While advancements in treatment have led to improved prognosis and survival for patients, these same treatments can adversely affect AYA reproductive capacity. Localized treatments such as surgery and radiation therapy may affect fertility by removing or damaging reproductive organs, and systemic therapies such as chemotherapy can be toxic to gonads, (ovaries and testicles), thus affecting fertility and/or endocrine function. This can be traumatic for AYA with cancer as survivors often express desire to have genetic children and report feelings of regret or depression as a result of infertility caused by cancer treatments.

Areas covered: Emerging technologies in the field of assisted reproductive technology offer new promise for preserving the reproductive capacity of AYA cancer patients prior to treatment as well as providing alternatives for survivors. The following review revisits contemporary approaches to fertility preservation as well newly developing technologies.

Expert commentary: There are several advances in ART that hold promise for patients and survivors. However there are challenges that inhibit uptake including poor communication between providers and patients about risks and fertility preservation options; high costs; and lack of insurance coverage for fertility preservation services.

Treating Nicotine Dependence and Preventing Smoking Relapse in Cancer Patients. 治疗尼古丁依赖和预防癌症患者复吸。

Introduction: Despite the well-documented harmful effects of smoking, many cancer patients continue to smoke. Smoking cessation is critical to address in this population given the associated increase in treatment toxicity, risk of second primary tumors, decrease in treatment response and higher disease-specific and all-cause mortality with continued smoking following a cancer diagnosis. This review seeks to summarize the latest recommendations and guidelines on smoking cessation treatment for patients diagnosed with cancer, and the evidence behind those recommendations.

Areas covered: We reviewed the latest evidence for smoking cessation treatments for cancer patients and the clinical guidelines and recommendation available for oncologists and health care providers. The unique aspects of nicotine dependence among patients diagnosed with cancer, and key challenges and barriers that cancer survivors and health care providers experience when considering smoking cessation treatments, and available clinical resources, are also discussed. Lastly, the authors summarize future directions in the field of smoking cessation treatment for cancer patients.

Expert commentary: While there are areas of improvement in research of smoking cessation treatment for cancer patients, critical under-explored areas remain. Nonetheless, providers should adhere to the NCCN guidelines and offer a brief counseling intervention to motivate patients to quit smoking when appropriate resources are not available.

This review seeks to summarize the latest recommendations and guidelines on smoking cessation treatment for patients diagnosed with cancer, and the evidence behind those recommendations.</p><p><strong>Areas covered: </strong>We reviewed the latest evidence for smoking cessation treatments for cancer patients and the clinical guidelines and recommendation available for oncologists and health care providers. The unique aspects of nicotine dependence among patients diagnosed with cancer, and key challenges and barriers that cancer survivors and health care providers experience when considering smoking cessation treatments, and available clinical resources, are also discussed. Lastly, the authors summarize future directions in the field of smoking cessation treatment for cancer patients.</p><p><strong>Expert commentary: </strong>While there are areas of improvement in research of smoking cessation treatment for cancer patients, critical under-explored areas remain. ABSTRACT Introduction: Physical exercise intervention programs show beneficial effects in early stage cancer patients during and after treatment. There is also growing evidence that physical exercise is safe and feasible for advanced cancer patients but knowledge is limited. Areas covered: This review provides an overview of randomized controlled exercise trials implemented in palliative care settings with advanced cancer patients. A comprehensive literature search was carried out in PubMed (MEDLINE), CINAHL, and the Cochrane Library. Six articles providing data of 590 patients met the predefined inclusion criteria. Mainly resistance exercise was applied and duration of interventions was between 8 and 16 weeks. Expert commentary: Beneficial effects for exercise patients were reported for both physical and psychological outcomes. However, general exercise recommendation for patients undergoing palliative treatment cannot be derived from the six analyzed studies. This is due to large heterogeneity of the applied exercise programs, included patient groups and performed assessment methods.

ABSTRACT Introduction: Ionizing radiation therapy is used as adjuvant, primary or palliative treatment of patients with primary tumors and metastatic bone disease. However, in cancer treatment, radiation also affects healthy tissues. IR spectroscopy can give important information on the behavior of radiation to bone structure. Areas covered: Radiation effects on matrix metabolism remain unclear, with a paucity of studies to identify the radiation effects at a molecular level. Spectroscopic studies have been prominent in the literature over the past two decades. In this article, we present our own data on the effect of radiation on the structure and properties of bone at a molecular level using FT-IR spectroscopy, correlate the data with the spectra and dose received, and discuss the related literature. Expert commentary: The interaction of cells with radiation results in enhanced production of free radicals leading to an alteration of the oxidative status and multiple cell damages. The products produced during radiation, free radicals and oxidative stress are similar to those during cancer development. The production of amyloid-proteins enhances the damage of radiation, while the hydrophilic environment inhibits the endogenous repairing mechanisms. The 1743 cm−1 band, attributed to malondialdehyde, can be used as ‘marker band’ for cancer progression.

ABSTRACT Introduction: The substitution of another opioid for a previous one in order to improve the clinical response has largely been reported as opioid switching, aimed to improve the opioid response in different clinical conditions. Areas covered: Existing data from literature have been reviewed to cover the different aspects of opioid switching in cancer patients. Expert commentary: Opioid switching has been reported to produce a clinical improvement in a large number of patients with chronic cancer pain presenting a poor response to one opioid. Despite the favourable effects reported with opioid switching, data are based on open studies, most of them being retrospective or small case series.
