AT1-AA Infusion during Pregnancy Impairs CBF Autoregulation Postpartum. 妊娠期输注AT1-AA会影响产后CBF的自我调节。

Preeclampsia (PE), new-onset hypertension during pregnancy alongside organ dysfunction, is a leading cause of morbidity and mortality for the mother and fetus. PE women have activated B cells that produce agonistic autoantibodies to the angiotensin II type 1 receptor (AT1-AA). AT1-AA impairs cerebral blood flow (CBF) autoregulation during pregnancy. Although AT1-AA often remains elevated up to 8 years postpartum, AT1-AA's effect on CBF autoregulation postpartum is unknown. This study examined whether elevated AT1-AA during pregnancy impairs CBF autoregulation postpartum and if this was augmented by infusion of AT1-AA postpartum. AT1-AA was infused into 12-week-old timed-pregnant Sprague Dawley rats beginning on gestational day 14. Uterine artery resistance index (UARI) was measured on gestational day 18 as a measure of endothelial dysfunction associated with PE. Dams were allowed to deliver. One group was given a second infusion of AT1-AA (50% perinatal dose mimicking levels observed in postpartum PE women) at 9 weeks postpartum. After postpartum week 10, mean arterial pressure (MAP) was measured in conscious rats and CBF autoregulation was measured by laser Doppler flowmetry. AT1-AA during pregnancy increased UARI (P<0.05). AT1-AA during pregnancy did not affect MAP postpartum but did impair CBF autoregulation postpartum. Infusion of AT1-AA postpartum significantly elevated blood pressure (P<0.01) but did not further impair CBF autoregulation. This study demonstrates that circulating AT1-AA during pregnancy causes impairment of CBF autoregulation well into the postpartum period indicating that elevated AT1-AA leads to long-term cerebrovascular consequences. Targeting AT1-AA may prevent cerebrovascular effects associated with PE during pregnancy and postpartum.

Predictors of Fatal Outcome after Incident of Stroke among Patients from Three Hospitals in Kinshasa, Democratic Republic of the Congo 来自刚果民主共和国金沙萨三家医院的患者中风事件后致命结果的预测因素

Summary Objective: This study investigated the predictors of stroke fatal outcome after a one year follow-up. Methods: This was a historic cohort of 166 stroke patients from three hospitals in Kinshasa, capital of the Democratic Republic of Congo. Data were collected from patients as they were admitted. Vital outcome information was collected throughout the follow-up period. The Cox proportional hazard modeling was used to measure predictors. Results: The patients’ median age was 59 (IQR: 52-68) years. The majority of them were male (65.1%) and known hypertensive (79.5%). Around 31% patients died within the one year period of follow-up; the median duration between the occurrence of stroke and death was 37 (14-155.5) days. The Glasgow Coma Score and day 7 blood glucose showed a significant influence on vital outcome: an early disorder of consciousness (Glasgow score ˂13) multiplies by 2.5 the risk of death (HR: 2.46, 95% CI [1.3-4.6], p = 0.005) and the glycaemia level at day 7 greater than or equal to 180 mg / dl increases the risk of death by 2.4 times (HR: 2.4, 95% CI [1.03 -5.7], p = 0.04). Conclusion: Hyperglycemia in the acute phase of stroke and early consciousness disorder are predictors of fatal prognosis of stroke, easy to detect in routine clinical practice. They should therefore be used to identify patients with potentially poor prognosis in the short and in the long term for appropriate care.

Dysphagia Incidence and Comorbidity in Hospitalized Acute Stroke Patients 住院急性脑卒中患者吞咽困难发生率及合并症

Introduction: There has been limited literature examined in a general population sample of the incidence of dysphagia in acute stroke patients, stroke patients with pneumonia and gastrostomy comorbidity, and the increased costs associated with the complications. Methods: The current study used Illinois Hospital Discharge data for in-patients admitted during September 2012 to June 2016 to investigate the incidence of dysphagia among patients who had a primary diagnosis of stroke. Sub-analysis of dysphagia incidence among stroke patients with co-morbidity pneumonia and gastrostomy was also performed. Results: The study found a 14.9% incidence of dysphagia in a population of hospitalized acute stroke patients. The rate of dysphagia was greater in the elderly and in blacks and Hispanics than whites. In stroke patients diagnosed with dysphagia, the incidence of pneumonia is 23.1% and the incidence of percutaneous endoscopic gastrostomy (PEG) is 62.1%. Stroke patients with dysphagia had doubled length of hospital stay than those without dysphagia. With the co-morbid conditions of dysphagia and pneumonia or PEG tube placement, the length of hospital stay tripled. Increased length of hospital stay is associated with significantly higher healthcare cost.
