Expression, clinicopathologic, and prognostic significance of MMP15 in operable pancreatic ductal adenocarcinoma MMP15在可手术胰腺导管腺癌中的表达、临床病理及预后意义

Objective: Matrix metalloproteinase 15 (MMP15) has been previously reported to be involved in many cancers. However, its expression pattern in pancreatic ductal adenocarcinoma (PDAC) remains contradictory. In addition, its clinicopathologic and prognostic significance in this malignancy has not been elucidated. Methods: Expression of MMP15 was immunohistochemically detected in a tissue microarray of formalin-fixed paraffin-embedded samples from 95 patients of PDAC after surgery. Its expression pattern and relations with clinicopathologic factors and disease-specific survival (DSS) were then evaluated. Finally, its expression and prognostic value were measured in the on-line publically available database, GEPIA, using TCGA data. Results: In comparison of all samples and 75 paired ones, MMP15 expression in tumor tissues was all significantly higher than that in para-tumor ones (P = .037 and .016). Furthermore, tumoral MMP15 expression was associated with peri-neural invasion. Survival analysis showed that patients with high tumoral MMP15 expression had significantly poorer DSS than those with low MMP15 expression (P = .0059). In univariate and multivariate Cox regression tests, tumoral MMP15 expression was all significantly predictive for DSS. In the on-line publically available GEPIA database, MMP15 was also overexpressed in PDAC, but was not found to be prognostic. Conclusion: Our results indicated that MMP15 expression was elevated in operable PDAC and might have a prognostic impact.

Molecular alterations and therapeutic targets in pancreatic neuroendocrine tumors 胰腺神经内分泌肿瘤的分子改变和治疗靶点

Human pancreatic neuroendocrine tumors (PanNETs) are a rare, deadly tumor type that is sporadic or arises in the background of a hereditary syndrome. A critical genetic event in sporadic tumors is inactivation of the gene menin 1 (MEN1) on chromosome 11, and indeed, PanNETs occur in patients with the hereditary syndrome multiple endocrine neoplasia type 1 (MEN1) due to germline mutations in the gene. Here, we review the recent progress in the field of molecular genetics and therapeutic targets of PanNETs. The key genomic alterations, including MEN1, ATRX/DAXX, mammalian target of rapamycin (mTOR), DNA damage and repair associated genes, vascular endothelial growth factor receptor (VEGFR) and SSTRs, and epigenetic aberrations in PanNETs are discussed. In addition, the commonly used preclinical models for PanNETs are enumerated.

KCNH2 regulates the growth and metastasis of pancreatic cancer KCNH2调控胰腺癌的生长和转移

Objective: Due to the characteristics of insidious onset and early metastasis of pancreatic cancer (PC), patients are often diagnosed at an advanced stage and often delayed in completing surgical resection timely, resulting in poor prognosis. Therefore, this study aims to explore the expression of potassium voltage-gated channel subfamily H member 2 (KCNH2) in PC and its relationship with clinicopathological parameters and the related mechanisms. Methods: GEPIA database and immunohistochemical staining were used to analyze the difference in KCNH2 expression between PC and adjacent tissue in RNA and protein levels. Chi-squared test was used to evaluate the relationship between KCNH2 expression and clinicopathological features. The Cox regression model was used for multivariate analysis and univariate analysis. Histological diagnosis was performed according to World Health Organization (WHO) criteria to evaluate the relationship between KCNH2 expression and clinicopathological features. Results: KCNH2 expression was upregulated in PC compared with normal pancreatic tissue. In addition, the knockdown of KCNH2 inhibits PC cell proliferation, migration, invasion, and epithelial-mesenchymal transformation and promotes their apoptosis. In addition, clinical data showed that the abnormal expression of KCNH2 in PC was related to the tumor stage. Patients with high expression of KCNH2 had a poor prognosis. Conclusions: KCNH2 is expected to be a novel targeted molecule in treating PC.

Characteristics and outcomes of patients with autoimmune pancreatitis after pancreatectomy: a retrospective study 胰腺切除术后自身免疫性胰腺炎患者的特点和预后:一项回顾性研究

Objective: Autoimmune pancreatitis (AIP) is a rare and special type of pancreatitis, which is often difficult to distinguish from pancreatic cancer. We aimed to discuss the clinical characteristics, operation characteristics, and therapeutic effects of AIP patients who received surgical treatment based on retrospective analysis. Methods: The medical records of patients who underwent surgery at Peking Union Medical College Hospital from January 2012 to July 2021 and whose postoperative pathology was AIP were collected. Clinical symptoms, laboratory examinations, imaging features, surgical information, and prognostic data were analyzed. Results: Nineteen patients with AIP (median age 60 years old, 14 males) underwent surgery, whose main indication for surgery was suspected of a malignant lesion (n = 18, 94.7%). Ten patients underwent pancreaticoduodenectomy, 8 underwent distal pancreatectomy, and 1 underwent total pancreatectomy, the pathological result of whom were all AIP. The main postoperative complication was pancreatic fistula (n = 11, 57.9%), and one patient (5.3%) underwent a second operation due to gastrointestinal anastomotic bleeding. One patient (5.6%) had a recurrence of AIP, 3 patients (16.7%) were treated with glucocorticoid after surgery, and one patient (5.6%) was treated with immunosuppressors. Conclusions: Differential diagnosis of AIP from pancreatic cancer is complex. Operation difficulty and complication incidence for patients with AIP were similar to pancreatic cancer patients. Although glucocorticoid is the first-line treatment for AIP, surgery may still be necessary for patients who are strongly suspected of a malignant lesion.

Background: Pediatric pancreatoblastoma is an extremely rare malignant tumor, posing diagnostic and treatment difficulties for pediatric surgeons. Using the Surveillance, Epidemiology, and End Results (SEER) database, we present an up-to-date report of the epidemiology, clinicopathological features, survival rates, and prognosis of pancreatoblastoma in pediatric patients. Methods: All pediatric patients diagnosed with pancreatoblastoma between 1975 and 2018 were identified in the SEER registries (SEER 8 registries and SEER 17 registries). We conducted a survival analysis to assess overall survival and 1- and 5-year late mortality rates. Descriptive statistics and log-rank test were performed. Results: A total of 22 children and adolescents with pancreatoblastoma were identified. In this cohort, 12 of 22 were male (54.55%), 14 were White (63.64%), and 11 were diagnosed between the ages of 1 and 4 years (50.0%). Among the 22 patients, 11 (50.0%) had distant metastases, whereas 7 (31.82%) had localized, and 4 (18.18%) had a regional disease. A total of 5 children and adolescents died during the study period, with cumulative survival rates of 14 of 17 (82.35%) and 10 of 11 (90.95%) among 1- and 5-year survivors, respectively. Cancer-directed surgery was significantly associated with an increased life expectancy (log-rank test, P = .018). Conclusion: Pediatric pancreatoblastoma is a rare entity. Cases that underwent surgery had a greater likelihood of overall survival and reduced late mortality.

Perioperative risk factors for delayed gastric emptying after pancreaticoduodenectomy: a meta-analysis of the complications 胰十二指肠切除术后胃排空延迟的围手术期危险因素:并发症的荟萃分析

Objective: To evaluate the correlation between the incidence of delayed gastric emptying (DGE) in pancreaticoduodenectomy (PD) and the perioperative possible risk factors. Methods: We searched PubMed, Embase, Cochrane Library, and Medline for studies describing complications related to PD from 2018 to 2022. Statistical analysis was performed using Stata/SE16.0 software. Results: In 6 retrospective studies, 1 systematic review, and 1 randomized controlled trial (RCT) experiment, 12,419 patients (2174 and 10,245 patients, respectively, in DGE and no delayed gastric emptying [NDGE] groups). The preoperative American Society of Anesthesiologists (ASA) score between DGE and NDGE groups (log odds ratio [OR] = 0.23, 95% credible interval [CI]: 0.11–0.35, P < .01), operation time (mean diff. = 21.87, 95% CI: 15.96–27.78, P < .01), estimated intraoperative blood loss (EIBL; mean diff. = 70.67, 95% CI: 17.75–123.58, P < .05), postoperative pancreatic fistula (POPF) (log OR = 0.85, 95% CI: 0.24–1.46, P < .05) were statistically significant. No significant differences in preoperative body mass index (BMI), preoperative biliary drainage, and pancreatic texture were observed (P > .05). Conclusion: The incidence of DGE is significantly correlated with the operation time, preoperative ASA score, POPF, and EIBL,which suggests that the surgeon should thoroughly evaluate the patient through adequate preoperative examination before the operation. Finally, standardized practice and perfect technology are undoubtedly necessary to reduce complications.
