Hallux Valgus (HV) is a condition with high prevalence in the general population that can cause important consequences in terms of pain and limitation in activities of daily living. Besides, HV surgery is a frequent surgical procedure that impose significant surgical and medical costs including implants, hospital expenses, used drugs and period of sick leave, in addition to the increased overall costs that could involve its poor management. The management of this pathology is barely unified in the current literature whilst there are many and varied surgical techniques (more than 100 described), the indications of which overlaps in many cases, leaving unclear as to what is the best approach to the treatment of this problem globally. In addition, there is no level 1 evidence available to prove which is the best option for each particular patient. It is therefore necessary to create standard format to be adopted in each clinical scenario and set out the standards to guide that process. This article aims to propose a clinical practice guideline that covers all types of HV in the population as a whole and that can be used systematically in each case. A flow chart is detailed with the preference of the surgical technique on a case-by-case basis and a clinical pathway is drawn on the management of this condition including all professionals involved from the first assessment to the final discharge.

Children complex limb disconnection, most of them undergo amputation after evaluate MESS score and then mounting prosthesis. In this case,the mess score of right calf amputation is above 7, and we adopted short 10cm replantation. After the limb replantation survived smoothly, we performed bone lengthening by Ilizarov technique. After 4 years follow-up, satisfactory clinical results have been obtained.

Objective: Rheumatoid arthritis (RA) is a common chronic inflammatory arthritis with many flare ups in some patients. New studies showed the importance of Mediterranean fever gene mutations in certain inflammatory conditions. We investigated molecular mutation in exon 10 of MEFV gene in patients with refractory RA and healthy individuals as a control group in North-West of Iran. Methods: We conducted thirty four patients with refractory RA and fifty healthy controls in this study. Their DNA samples were analyzed with specific primers, standard PCR and Sequencing techniques. We studied the most common exon 10 variations on MEFV gene which include; M680I, M694V, M694I and V726A. Results: The mean ages of patients with refractory RA were 43.8 ± 7.7 years and disease duration was 49.8 ± 7.36 months. The mean of DAS 28 (Disease Activity score) of patients with refractory RA was about 4.2 ± 0.51. The mean ages of healthy control cases were 45.4 ± 8.27 years. The result of DNA sequencing showed no mutations in common exon 10 variations of MEFV gene in both groups. Conclusion: According to the result, we found no association between severity of RA and common exon 10 MEFV gene mutations.
