Heel ulceration – An exploration of the issues 足跟溃疡-问题的探讨

This article presents an exploration of some of the issues that require consideration in relation to the assessment and prevention of heel ulceration. The number of older people in the United Kingdom (UK) is rising with the fastest growing age group in the population being those aged 80 years. They currently constitute 4.5% (2,749,507) of the total population, this age group has increased by over 1.1 million between 1981 and 2007 (1,572,160 to 2,749,507), from 2.8% to 4.5% leading us to believe that the incidence of pressure ulcers may also rise. The prevention and management of pressure ulcers in orthopaedic patients, especially those on the heel, has been a long standing dilemma for nursing staff. The heel has been identified as the second most common site, accounting for up to 28% of all reported pressure ulcers. Authors have argued that heel ulcerations are notoriously difficult to heal and that management and treatment can be costly.

One of the most common orthopaedic injuries sustained by elderly women in the UK is a proximal fractured femur, often following a fall. This in itself is of great concern to both patient and caring relatives due to the morbidity and ‘year on’ mortality rates typically reported as 20–35% within one year (Goldacre, M.J., Roberts, S.E., Yates, D., 2007. Mortality after admission to hospital with fractured neck of femur: database study. British Medical Journal, 325, 868–869.). This article follows some of the issues experienced by an elderly female patient following a proximal fractured femur, compounded by the emergence of a Clostridium difficile infection during the rehabilitation period. Based on the events as told by the carer this article relays the frustrations of an elderly female patient who struggles to resume her normal life and the importance of relaying accurate information in care.

Fluid optimisation using a peripherally inserted central catheter (PICC) following proximal femoral fracture: Lessons learnt from a feasibility study 股骨近端骨折后采用外周插入中心导管(PICC)进行液体优化:来自可行性研究的经验教训

The aim of this study was to test the feasibility of using peripherally inserted central catheters (PICCs) in older people to enable fluid optimisation. Fourteen patients were randomised to three groups: (i) usual care, insertion of short peripheral intravenous cannulae and normal fluid prescription; (ii) PICC insertion and normal fluid prescription; (iii) PICC insertion and guided fluid prescription based on measurements of central venous pressure (CVP). A range of outcome measures were undertaken, plus two focus groups with ward staff and an interview with the research nurse to ascertain views concerning the implementation of the study. Descriptive findings identified that PICC use in this group of patients was extremely difficult. The practical issues affecting the feasibility of this study were: (i) the physical and psychological frailty of the patients, proxy consent, difficulties measuring outcomes, unsuccessful PICC placement due to aged veins, intolerance to lines; (ii) staff concerns relating to patient vulnerability, competent use of new technology, limited resources and work capacity. Most aspects of the trial were made more difficult due to the frailty of the patient group.
