We hope that our focus article as well as the commentaries by Huang, Christie, and Wessendorf will encourage the readers to consider the puzzle of serotonin's role in pain modulation. As highlighted in Huang's commentary, more research is particularly necessary with regard to serotonin's actions within the spinal cord. In designing and performing these experiments, we urge our colleagues to consider that: (1) serotonergic cells are likely to use a tonic, concentration-dependent code and (2) serotonin release is likely to modulate the effects of other neurotransmitters as well as have effects on its own.

Visceral pain 发自内心的痛苦

A painful experience may be brought about by an equilibrium shift in the kinetics of the central nervous system or by the information flow in separate interactive pathways and neural centers. A number of recent studies have addressed the role of spinal pathways, in particular the dorsal column, and other alternative mechanisms in the processing and integration of visceral information into a visceral pain experience. This article reviews newly reported experimental findings and some of the historical clinical evidence that point to a role of the dorsal column in visceral pain. It proposes a shift to a goal-oriented approach to pain issues that defers speculations about the nature of pain and keeps in mind the exigency of pain relief for those who are suffering.

Do medullary serotonergic neurons tonically modulate nociceptive transmission? 髓质5 -羟色胺能神经元是否张力调节伤害性传递?

Mason and colleagues have developed an analytical method to reliably identify serotonergic neurons in the rostral ventromedial medulla (RVM), including the raphe magnus, during extracellular recordings in vivo. Results of these studies strongly suggest that the serotonergic neurons do not belong to the major classes of opioid-sensitive neurons phasically activated or inhibited during noxious stimuli. However, their studies have not ruled out any involvement of serotonergic RVM neurons in supraspinal opioid antinociception, and they are at odds with detailed in vitro studies of opioid mechanisms in RVM neurons. It remains possible that tonic activity serotonergic RVM neurons plays an important role in descending opioid modulation of nociception.

It has been proposed that serotonergic neurons do not mediate opioid analgesia, based on the observation that the firing rates of serotonergic neurons are not modulated by antinociceptive treatments such as systemic opioids or periaqueductal gray (PAG) stimulation. However, several points need to be clarified regarding the data supporting this hypothesis. These include establishing whether anesthesia has influenced the results of these experiments, whether the serotonergic neurons recorded project to the dorsal horn, and whether presynaptic effects might allow opioids to change serotonin release without changing neuronal firing rate
