The National Heart Foundation of Australia: The first 21 years 澳大利亚国家心脏基金会:前21年

The treatment of cardiovascular disease has been one of the late 20th century's great success stories. Rising death rates in Australia in the 1940s and 1950s gave impetus to the movement for the establishment of a community-based heart foundation in Australia, an organisation that could marshal resources and provide leadership to the community in its fight against what appeared to be an epidemic. The new organisation, named the National Heart Foundation, became a reality in 1959, the third such community-based heart foundation to be formed worldwide. The hard work and vision of interested, community-minded citizens and members of the medical profession, together with the support of the Federal Government and the press, resulted in a highly successful initial appeal for funds in 1961. Following further appeals in 1969 and 1975, an annual fund-raising program was adopted and financial resources allocated on the basis of merit, to scientific and medical research programs. Other major projects initiated by the Foundation in the 1960s and 1970s included: the Australian Therapeutic Trial in Mild Hypertension; the collection of data on the extent and nature of disease (including a register of heart attack for the city of Perth in 1971–1972, and a register of cardiac surgery for Australia); and education and rehabilitation programs.

The National Heart Foundation of Australia has been supporting cardiovascular research for more than 40 years. It has funded grants-in-aid to a wide range of researchers involved in basic, clinical and public health aspects of cardiovascular disease. It has also provided salary support for a large number of scholarships and fellowships. In addition, it has initiated and conducted projects of its own in the areas of epidemiology and public health research and, in partnership with industrial sponsors, has conducted large-scale clinical trials.[a/b]

Glucose, insulin, potassium (GIK) and the concept of metabolic support for the postischaemic heart 葡萄糖,胰岛素,钾(GIK)和脑缺血后心脏代谢支持的概念

Successful reperfusion has moved the frontier of treatment for acute myocardial ischaemia from coronary arteries to the heart muscle itself. Here we update the concept of metabolic support for the postischaemic heart which is based on an understanding of an earlier review on the same topic. The ischaemic heart is an energy-depleted organ. A mismatch of ATP production and utilisation leads to the uncoupling of ATP-dependent Ca2+-mediated excitation-contraction coupling necessary for normal contractile function. Sustained ischaemia leads to adaptive changes of either programed cell survival, cell death or cell necrosis. We are presenting a working hypothesis on the metabolic and ionic consequences of myocardial ischaemia that lead to progressive contractile dysfunction. Based on this hypothesis we develop a rationale for the use of GIK in the support of substrate metabolism by replenishing depleted energy stores to reverse the deleterious events in the postischaemic state. Possible mechanisms include replenishment of depleted cellular glycogen stores and restoration of efficient energy transfer through moiety-conserved cycles. The usefulness of this concept has been demonstrated in the improved survival of patients in cardiogenic shock after hypothermic ischaemic arrest of the heart and in patients with acute myocardial infarction treated with GIK.
