Background

Washed microbiota transplantation (WMT) has been demonstrated as an effective treatment for IBS. However, limited research has assessed its efficacy in managing extraintestinal symptoms of IBS, particularly poor sleep and psychological disorders. This study aimed to evaluate the therapeutic effect of WMT on sleep disturbances, psychological and gastrointestinal symptoms among patients with IBS.

Methods

This is a prospective observational study involving patients with IBS who underwent WMT. The Gastrointestinal Symptom Rating Scale (GSRS) and IBS Severity Scoring System (IBS-SSS) were used to assess gastrointestinal symptoms and IBS severity, respectively. The Pittsburgh Sleep Quality Index (PSQI) was used to evaluate sleep quality. The Self-rating Depression Scale (SDS) and Self-rating Anxiety Scale (SAS) were used to evaluate depression and anxiety, respectively. All the questionnaires were evaluated at baseline and one month after WMT. A multiple logistic regression model was used to determine the predictive factors of sleep improvement.

Results

Seventy-three patients with IBS were included in the study. Sleep quality (P < 0.001), depression (P < 0.001), anxiety (P < 0.001), gastrointestinal symptoms (P = 0.004), and IBS severity (P < 0.001) significantly improved one month after WMT in all patients. The five components of PSQI in patients with poor sleep quality (PSQI scores > 5 suggests poor sleep quality) decreased one month after WMT compared to baseline scores: subjective sleep quality (P = 0.001), sleep latency (P < 0.001), sleep duration (P = 0.008), habitual sleep efficiency (P < 0.001) and sleep disturbances (P < 0.001) (IDDF2023-ABS-0066 Figure 1. Patients that experienced sleep improvement demonstrated greater alleviation in depression and IBS severity compared with patients without. Baseline PSQI scores were identified as an independent predictive factor of sleep improvement one month after WMT (OR 1.285 [95% CI = 1.004-1.643]; P = 0.046).

Conclusions

Our findings suggest that WMT may be a promising integrated therapy for IBS, improving both gastrointestinal and extraintestinal symptoms. Furthermore, WMT may produce better and more sustained clinical outcomes by improving the sleep of IBS patients.

Background

High-throughput sequencing has been widely used in characterizing microbial communities and revealing microbial-host interactions. However, the abundance produced by this technique is semi-quantitative, leading researchers to focus on the relative abundance of microbes and ignore the total microbial load. Moreover, changes in relative abundance do not accurately reflect the actual shifts in microbial load, which may hinder further microbiological research.

Methods

In this study, a bacterial absolute abundance quantitative method based on high-throughput 16S rRNA gene V4 sequencing was developed and used to reveal the changes in the gut microbiota of patients with ulcerative colitis (UC) who underwent fecal microbiota transplantation (FMT) (IDDF2023-ABS-0089 Figure 1 (a) Design and workflow of spike-in bacterium DNA). Samples were collected and analyzed before FMT (W0) and at 1 (W1), 4(W4) and 12 (W12) weeks after FMT.

Results

The spike-in bacterium Bacillus baekryungensis 700Cs for absolute abundance analysis was well distinguishable from the gut bacteria and did not affect the bacterial composition of fecal samples. The overall bacterial communities changed significantly (P<0.01) after FMT (IDDF2023-ABS-0089 Figure 1 (b) The absolute abundance of Donor, FMT (W0) and at 1 (W1), 4(W4) and 12 (W12) weeks after FMT), characterized by enrichment of 14 genera (eg Prevotella_9, Megamonas) and depletion of 29 genera (eg Veillonella, Escherichia-Shigella). Furthermore, gut bacterial communities of patients who achieved clinical remission (Rm) were shifted toward donors at W1, much earlier than these are not achieving clinical remission (NRm). The load of Ruminococcaceae_UCG-002, Ruminococcaceae_UCG-003 and Ruminococcaceae_UCG-005 were significantly (P<0.05) increased in Rm. The abundances of Eubacterium_coprostanoligenes_group, Coprococcus_3 and Holdemanella were significantly (P<0.05) increased for two consecutive timepoints post-FMT in Rm group, and that Christensenellaceae_R-7_group in the W1 was significantly (P<0.05) greater than that of W0 (IDDF2023-ABS-0089 Figure 1 (c) The significantly different taxa between W0 and W4 in patients with UC achieving clinical remission after FMT). Meanwhile, the abundances of un_f_Enterobacteriaceae, Haemophilus and Actinomyces were significantly decreased for 2 or 3 consecutive timepoints post-FMT in the Rm group.

Conclusions

Adopting this method, we identified significant shifts in gut bacteria of patients with UC after FMT, especially in the clinical remission group, including the increase of Ruminococcaceae_UCG-002, Coprococcus_3, Christensenellaceae_R-7 and decrease of Veillonella and Haemophilus. BackgroundThe COVID-19 pandemic has led to changes in dietary and lifestyle habits among children and adolescents. This study investigated the prevalence and factors associated with unhealthy dietary habits among 1,475 primary and secondary school students from 2021 to 2022.MethodsA cross-sectional study was conducted in randomly selected primary and secondary schools participating in a project promoting online health education and promotion named GoSmart using self-administered questionnaires. Logistic regression analyses were conducted to investigate the demographic and lifestyle habits associated with various unhealthy dietary habits. This study was approved by Survey and Behavioural Research Ethics (SBRE) [Reference No.: SBRE-21-0052] and funded by Quality Education Fund (QEF#2019/0883).ResultsThe prevalence of inadequate consumption of vegetables and fruits was high among both primary (81.5%) and secondary school students (89.5%) and associated with physical inactivity in both primary (aOR=3.35, 95%CI: 1.41-7.97, p=0.006) and secondary students (aOR=4.30, 95%CI: 1.51-12.25, p=0.006). A substantial prevalence of breakfast skipping was observed among primary students (22.9%) who spent two or more hours on video games or social media (aORs=1.84-2.27);and among secondary students (23.7%) who consumed two or more hours on social media, alcohol consumption, and consumption of unhealthy food (aORs=1.42-2.79). Consumption of unhealthy food was reported by 43.9% and 48.2% for primary and secondary students respectively, with primary and secondary students who spent two or more hours on video games or social media (aORs=1.62-2.27) and secondary students who perceived themselves as underweight (aOR=1.79, 95%CI: 1.13-2.83, p=0.012) having a higher frequency of consumption.ConclusionsThere was a high prevalence of unhealthy dietary habits among schoolchildren in Hong Kong. Alarming results were observed with physical inactivity being associated with a higher risk of unhealthy dietary habits among primary and secondary students, which may increase the risk of obesity. Moreover, the apparent interrelationship between unhealthy habits highlights the need for a comprehensive approach to promoting healthy habits through education in this population.
