The coincidence of gastric diseases and chronic duodenitis has been investigated on biopsies and preparations of stomach and duodenum by a quantitative histologic method. The histologic classification of duodenitis was made according to Whitehead et al. 9 of 40 patients with a chronic atrophic gastritis had a duodenitis grade 1. 10 of 40 patients with a chronic gastric ulcer had a duodenitis grade 1 and one patient grade 2. 14 of 40 patients with a gastric cancer had a duodenitis grade 1. By statistical investigations it has been found out that patients with an ulcer or cancer do not have duodenitis more often than patients with gastritis. A possible duodenitis concerning the investigated gastric diseases is mainly due to the chronic gastritis that also could be proved by all patients suffering from ulcer or cancer.

The distribution of collagen (determined as hydroxyproline) is compared with the histological index of fibrosis in 32 cadaver livers. The results indicate a partly heterogeneous distribution of collagen between three topographic different regions of the liver. The possible reasons for the different distribution of collagen were discussed and compared with data of the literature. Our results suggest that one biopsy only is not representative for the whole organ in every case.

Acute pancreatitis was produced in rats by a combination of dyschylic edema and short-term ischemia of the pancreas. The incidence of acute pancreatitis, reflected by peri- and extra-pancreatic fat necrosis, was found to be dependent on the duration of ischemia. Under most conditions there were no significant differences in enzyme activities between the animals with macroscopic signs and those without these. The time course of alpha-amylase and lipase release was investigated within 24 hours postoperatively. Additionally the quantitative correlation was estimated of the enzyme activities released from the pancreatic tissue and the activities determined in rat serum. The data show that the serum enzymes represent only a small and variable portion of the total amount of enzymes effused from the pancreas.

Trends of prevalence and incidence rates of non-insulin-dependent diabetes mellitus (non-insulin-treated diabetes mellitus) were assessed in the population of the GDR based upon the National Diabetes Register and the Official Statistical Year Book as sources for the calculations. Within the 25-year follow-up period 1960-1984 the prevalence rose from 4.39%; to 31.95%; the incidence rate from 1.04%; to 3.57%. Age-dependence of the specific rates is characterized by their continuous rise above the age of 30 years reaching the peak prevalence of 146.6%; in 75- to 80-year-olds, that of 14.1%, for the incidence in people aged 70 to 75 years. A significant male preponderance was confirmed between the ages of 30 and 50 years, a significant overwhelming of female NIDDM in the age groups 60 to 90 years. Based on demonstrated correlations between the changes of living standard parameters and the epidemiological trend of NIDDM the conclusion is drawn that overnutrition and reduced muscular activity mainly account for the rise of diabetes morbidity in the population of the GDR.

The relationship between an improvement of the carbohydrate metabolism due to a four-week intensified conventional insulin treatment and changes in fatty acid patterns of serum lipids was investigated in 12 insulin-dependent diabetic patients. At the commencement of the study the diabetic patients were characterized by higher relative linolic acid contents in the serum cholesterolesters and phospholipids in comparison with control subjects. The intensified insulin treatment was associated with a significant improvement of the glycaemic control, whereas the fatty acid patterns of the serum lipids remained unchanged. The results demonstrate that short-time improvement of the carbohydrate metabolism is not associated with qualitative changes of serum lipids in insulin-dependent diabetic patients. Remains to be established the long-term effect of improved metabolic control on fatty acid metabolism.

Also cholesterosis is more frequent among cases with cholesterol stones of little volume than among cases with stones with higher volume. From the 20. to the 49. year of life cholesterosis is more frequent among cases with gallstones than among cases without stones. By means of monoclonal antibodies against surface antigens of mononuclear cells we examined alterations of T-cell-subpopulations of peripheral blood and liver tissue in the course of acute HBsAg-negative hepatitis. Used methods for determination were the immunofluorescence for blood lymphocytes, respectively the peroxidase-anti-peroxidase-(PAP)-technique for round-cell-infiltrates of the liver based on paraffin fixed sections. The focus of attention was the relation of T4-(helper/inducer) to T8-(suppressor/cytotoxic) cells, the so-called immuno-regulatory quotient. With advancing improvement in the course of the disease we observed in peripheral blood a decrease of T4+/T8+ ratio from very high (3.6) to normal (1.9) values. By examination of the tissue round-cell-infiltrates we found a contrary behaviour to the peripheral blood with a low T4+/T8+ ratio in the liver, characteristic to more acute phases and due to an T8+-cell interchange between periphery and liver. This may be common for all virus diseases and without specificity for defined types of acute hepatitis. Beside this exists the possibility T4+/T8+ ratio has any prognostic value.

A case of endometriosis of the large intestine is reported in a 41 years old woman. The endometrioma involved the Bauhin-valve, the distal ileum and the right adnexa. The symptoms and signs were thought of a caecal carcinoma causing a mechanical ileus. The morphologic aspects and the management of the diagnostic and therapeutic procedure are discussed with respect to the endometriosis of the intestine.

In a small district hospital of the Factory Public Health the selective proximal vagotomy was introduced in conformity with the indicated standard operation technique in patients with gastroduodenal ulcer. This operation technique was based on the literature publications and on the own experiences within the methodical instructions in university hospitals. The importance of the selective proximal vagotomy (spV) was checked by means of 110 surgical operations, above, all for the reincorporation of the miners into the work procedure. Simultaneous the value of a continually dispensary care of the patients who underwent gastric operation was emphasized. The possibilities of the preoperative and postoperative diagnostic in a small district hospital were pointed out; at this a good cooperation and interdisciplinary collaboration with greater hospitals guarantees optimum investigation programmes. The regular medical supervision of operated patients is at the same time followed by a control of quality of the own performed work.

Since the first descriptions of liver tumours associated with the intake of steroid contraceptives, the literature has presented an increasing number of case reports as well as epidemiological studies on the relationship between benign liver tumours and the utilization of these drugs.
